A case of pulmonary edema secondary to dislocation of laryngeal mask during awakening from anesthesia is described. The complication has been ascribed to physiopathological changes of the alveolo-capillary membrane caused by inspiratory efforts secondary to dislocation of laryngeal mask during awakening from anesthesia. Other possible causes of pulmonary edema are discussed, especially the cardiogenic one, and among the non cardiogenic edema, the Mendelson's syndrome. The quick identification and the intensive care of post obstructive pulmonary edema lead to a rapid resolution of the pathology

Fiberoptic bronchoscopy was performed on 11 children aged from 6 months to 3 years and weighing between 2.1 and 12.5 kg using the laryngeal mask airway (LMA). The LMA allows observation of subglottic stenosis which can not be seen under tracheal intubation. The smaller prototype of the fiberoptic bronchoscope whose diameter is 1.8mm has no provision for angulation at the tip. However, using the LMA as a guide it was easy to reach the subglottic stenosis even using this smaller bronchoscope.

In daily anesthetic practice, the insertion of a Laryngeal Mask--airway is facilitated by means of a lubricant. Many different products are in widespread clinical use, according to personal preference or what's in or on the anesthesia trolley. This study compares four different lubricants: Xylocaine 10% spray, Silkospray, Endosgel and water. When comparing the ease of insertion of the Laryngeal Mask, the genuine lubricants proved to be superior to water. However, no single lubricant was associated with a statistically significant lower incidence of postoperative sore throat.

We have evaluated the reinforced laryngeal mask airway (LMA) for use during dento-alveolar surgery in 100 ASA I and II day-case patients allocated randomly to receive either a nasotracheal tube or reinforced LMA. We recorded ease of airway insertion, airway complications, quality of recovery and replies to a 24-h postoperative questionnaire. In addition, a fibreoptic assessment was made of laryngotracheal soiling, and the effect of head movement and the position of the reinforced LMA. There were no significant differences in difficulty in airway positioning or perioperative oxygen desaturation

We have compared insertion of a tracheal tube and laryngeal mask airway (LMA) both with and without the presence of a tube in the oesophagus in 20 ASA I and II patients undergoing elective laparoscopy. After induction of anaesthesia and neuromuscular block, we measured the times for an experienced anaesthetist to correctly position both an LMA and a tracheal tube with and without a tube in the oesophagus. The time to intubation was significantly less with the LMA than with the tracheal tube, both with and without an oesophageal tube in place (P < 0.05). We conclude that if a tracheal tube is p

The laryngeal mask airway was inserted in 10 cadavers. At postmortem the chest was opened and an infusion set primed with a dilute barium solution was inserted into the oesophagus and ligated in place. A cricoid force of 43 N was then applied and the infusion set was positioned so that when the clamp was opened it generated a pressure of 7.8 kPa within the oesophagus. The cricoid pressure was able to stop the flow of fluid into the oesophagus. This demonstrates that cricoid pressure is effective in preventing reflux at intragastric pressures which are encountered clinically and the presence of

Observations were recorded from two hundred consecutive insertions of the laryngeal mask airway (LMA). They were undertaken by 27 anaesthetists most of whom had no previous experience of its use. A clinically patent airway was obtained in 94% of patients and in the majority of these (76%) the LMA was positioned correctly at the first attempt. There were twelve cases of failed insertion. Nine of these were as a result of an inability to pass the LMA correctly into the hypopharynx while in the remaining three cases complete airway obstruction was reported. The insertion technique was easily acqu

OBJECTIVE: To evaluate the feasibility of a video flexible laryngeal mask airway (VFLMA) for simultaneous ventilation and direct visual RLN monitoring during thyroid surgery.
METHODS: This prospective single-arm study enrolled 104 consecutive patients undergoing thyroid surgery, in whom the VFLMA provided real-time glottic views after induction. The primary outcome was successful ventilation with RLN monitoring. Secondary outcomes included diagnostic performance of visual RLN monitoring (vocal cord movement upon stimulation) and perioperative respiratory adverse events.
RESULTS: Effective ve

BACKGROUND: The Laryngeal Mask Airway Vision Mask (LMA VM) is a supraglottic airway device (SAD) with a vision guidance system. The ideal head and neck position for direct laryngoscopy is known, but the ideal position for placing a LMA is not. The objective of this study is to evaluate and compare the optimal position for placement of a video laryngeal mask airway.
METHODS: This prospective, observational, transversal, and analytical study was performed in 72 consecutive patients. In the same patient, laryngeal vision was first assessed with the head and neck in the sniffing position and then

With the popular use of the laryngeal mask airway (LMA), recurrent laryngeal nerve injury associated with the LMA, which might cause serious complications, has recently been reported. The true mechanism, however, remains unclear, which makes effective prevention and treatment of this disorder difficult. On the basis of the anatomical relationship of the laryngopharynx and the laryngeal mask and the progressive recovery in most reported cases, we hypothesize that demyelinating neuropraxia resulting from direct mechanical compression contributes to the recurrent nerve injury associated with the

Airway management in anesthetized pigs is known to be technically demanding, and the 'gold standard' technique of endotracheal intubation is particularly difficult to master. The authors investigated the feasibility of the laryngeal tube as an alternative technique for airway management in German Landrace pigs (n = 5). They compared this method with the laryngeal mask, which is considered to be an effective yet relatively straightforward tool for porcine airway management. One after the other, investigators attempted to establish an airway in each anesthetized, artificially ventilated pig usin

PURPOSE: We report an unusual case of vocal cord edema associated with the use of a ProSeal laryngeal mask airway (PLMA) in an adult patient with an undiagnosed upper respiratory tract infection (URTI).
CLINICAL FEATURES: A 55-yr-old woman had fixation of a radial fracture under general anesthesia with muscle relaxation. She developed audible wheezing 30 min after PLMA insertion. Bronchoscopic examination revealed significant vocal cord edema. Adequate ventilation was possible at increased airway pressures, and the administration of dexamethasone 4 mg iv produced clinical resolution of the st

OBJECTIVES/HYPOTHESIS: A critical step in thyroidectomy involves definitive identification of the recurrent laryngeal nerve (RLN). Using the laryngeal mask airway, identification of the RLN can be facilitated by stimulation of the nerve while monitoring vocal cord movement with a fiberoptic laryngoscope. We present this technique as an effective and safe means to identify the RLN during thyroid surgery, with significant advantages over existing techniques in appropriately selected patients.
STUDY DESIGN: Retrospective case series.
METHODS: We performed thyroidectomy on 8 patients (13 RLN ide

BACKGROUND: Face-mask ventilation is the most common resuscitation method for birth asphyxia. Ventilation with a cuffless laryngeal mask airway (LMA) has potential advantages over face-mask ventilation during neonatal resuscitation in low-income countries, but whether the use of an LMA reduces mortality and morbidity among neonates with asphyxia is unknown.
METHODS: In this phase 3, open-label, superiority trial in Uganda, we randomly assigned neonates who required positive-pressure ventilation to be treated by a midwife with an LMA or with face-mask ventilation. All the neonates had an estim

The TotaltrackTM is a new video laryngeal mask, which combines a supraglottic airway with a videolaryngoscope. We evaluated the performance of this device in an observational study in 300 adult patients. The Totaltrack was inserted at the first attempt and the glottis seen in all cases. In 249 patients (83%; 95%CI 78.7-87.3%) the glottis was seen at the first attempt. Adequate ventilation and tracheal intubation were achieved in all patients. The median time to visualisation of vocal cords, to confirmation of ventilation and to successful tracheal intubation was 5 s, 13 s and 24 s, respectivel

Hiccup is an involuntary contraction of the diaphragm and intercostal muscles resulting in sudden inspiration and closure of the glottis. The presence of hiccup in the perioperative period can be a challenging problem. Sudden movements of the patient from hiccups can interfere preoperative diagnostic procedures, intraoperative hiccup may delay the beginning of surgery, interfere with the surgical process, and affect intraoperative monitoring, and postoperative hiccup may affect would healing and hemodynamic stability. Hiccup can lead to have increased aspiration risk. Hiccup are is an incomple
