Closure of post-laryngectomy pharyngocutaneous fistulae.

BACKGROUND: Closure of salvage laryngectomy defects with vascularized tissue remains controversial. METHODS: We evaluate outcomes in patients who required repair of a fistula after attempted primary closure of salvage laryngectomy defect and assess risk factors for persistent fistula. Between 2001 and 2010, 20 patients were treated for pharyngocutaneous fistulae after primary closure of a salvage laryngectomy. All patients required free flap repair for definitive fistula management. RESULTS: Patients presented with fistulae from one to 18 months in duration; median time to closure was seven

Modified frontolateral partial laryngectomy without tracheotomy.

OBJECTIVE: To investigate the feasibility of modified frontolateral partial laryngectomy without tracheotomy for patients with early-stage laryngeal cancer or dysplasia of the true vocal cord. STUDY DESIGN: After frontolateral partial laryngectomy, the inner side of the sternohyoid muscle was drawn into the laryngeal lumen to suture it to the incisal margin of the uninjured side and to the false vocal cord on the side of the lesion to ensure the safety without tracheotomy. A reverted sternohyoid fascial flap was used to cover the anterior area to form a new laryngeal lumen in the shape of a l

Early oral feeding following total laryngectomy.

OBJECTIVES: To determine whether, in a developing world context, early oral feeding after laryngectomy is safe, cost-effective and appropriate. STUDY DESIGN: A prospective study of early oral feeding after laryngectomy, compared with retrospective, historical delayed feeding controls. METHOD: Forty patients underwent total laryngectomy for advanced carcinoma of the larynx with or without hypopharyngeal involvement, not requiring tongue base resection or myocutaneous flaps, and were commenced on oral feeding on the second post-operative day. Thirty-nine laryngectomy patients previously manage

[Lipoid pneumonia in patients after laryngectomy].

Three patients with exogenous lipoid pneumonia are presented. All of them had laryngectomy because of the cancer of larynx. In a period of time ranging from a few months up to a few years after the operation they started to have dyspnoea, cough, sometimes hemoptysis and slightly elevated temperature. The chest X-ray revealed massive opacities in the lower zones of both lungs suggesting lung cancer or metastases of the laryngeal cancer. HRCT showed ground glass lesions. Histological examination of the lungs detected accumulation of multiple macrophages with vacuolated foamy cytoplasms in the al

Laryngostroboscopic evaluation after supracricoid partial laryngectomy.

BACKGROUND: Supracricoid partial laryngectomy (SCPL) results in laryngeal preservation in more than 95% of patients with T2 glottic carcinoma. Postsurgical glottic function is characterized by an absence of vocal cords, and phonation quality is a key post-SCPL quality-of-life factor. OBJECTIVE: This investigation was designed to enhance post-SCPL vocal function, study anatomic function of the post-SCPL larynx, and correlate anatomic findings with perceptual and instrumented measurements of voice. METHOD: Twenty-five patients were included. All had undergone SCPL with cricoepiglottopexy for T

[Sleep apnea syndrome after total laryngectomy].

INTRODUCTION: Tracheostomy was the treatment of choice for sleep apnoea syndrome before nasal continuous positive airway pressure was developed. The occurrence of sleep apnea syndrome after total laryngectomy is unusual. CASE REPORT: The authors report the case of a 73 year-old man presenting with the symptomatology of sleep apnoea syndrome, several years after a total laryngectomy for cancer. The polysomnographic recording confirmed severe sleep apnoea syndrome with an apnea-hypopnea index of 89. Clinical examination showed a marked reduction in the diameter of the tracheostomy orifice durin

Vocal analysis after vertical partial laryngectomy.

It is generally believed that a reconstruction of the glottic region after a vertical partial laryngectomy (VPL) can improve the glottic and supraglottic function. However, there is a paucity of reports on secondary healing without a glottic reconstruction after a VPL. The aim of this study was to obtain objective phonatory data after a VPL without a glottic reconstruction. From 1993 to 2001, 13 patients, who had been treated with VPL without a glottic reconstruction, were enrolled in this study. Patients with a postoperative follow up of less than 12 months were excluded. Seven lesions were c

[Results of supraglottic partial horizontal laryngectomy].

INTRODUCTION: Supraglottis is a part of larynx comprising two subregions: epilarynx (suprahyoid epiglottis--including lingual and laryngeal surface, aryepiglottic folds--laryngeal surface, and arytenoids) and supraglottis without epilarynx (infrahyioid epiglottis and ventricular folds). MATERIALS AND METHODS: A total of 234 patients with supraglottic squamous cell carcinoma undergoing primary surgery were analyzed in the period 1976-1996. The tumor was localized in epilarynx in 84 (25%) patients, and in supraglottis without epilarynx in 261 (75%) cases. RESULTS: T1 tumor was present in 145 (

[Functional outcomes after supracricoid partial laryngectomy].

OBJECTIVES: We evaluated functional outcomes in patients undergoing supracricoid partial laryngectomy (SCPL). PATIENTS AND METHODS: The study included 20 male patients (mean age 61.5 years; range 43 to 76 years) who underwent SCPL for advanced laryngeal carcinoma. Correlations were sought between variables (age, medical history, reconstruction techniques such as cricohyoidopexy or cricohyoidoepiglottopexy, arytenoid resection) and decannulation time, duration for oral feeding, weight change, and complications. The mean follow-up was 20.9 months (range 7 to 39 months). RESULTS: All patients w

Laryngectomy alone for T3 glottic cancer.

BACKGROUND: The long-term survival of 81 patients with T3 squamous cell carcinoma of the glottic larynx treated with laryngectomy alone is presented and pathologic predictors for cancer recurrence above the clavicles and cancer death are identified. METHODS: Clinical records, operative notes, and pathologic slides were reviewed. The major end points were failure above the clavicles, cause-specific survival, and overall survival. All patients were followed until death or a minimum of 10.8 years. RESULTS: The main pattern of treatment failure was within an undissected ipsilateral or contralate

Vertical partial laryngectomy for glottic carcinoma.

Vertical partial laryngectomy is recommended as primary treatment for selected T2 glottic cancers. Some irradiation failures are also suitable but the indications are more restricted. A careful assessment of the cancer is mandatory. Of 12 patients undergoing primary surgery, no patient has died from their disease. Five of seven irradiation failures were salvaged by vertical partial laryngectomy.

Supraglottic laryngectomy: 30 years of experience.

A review of 467 cases of supraglottic laryngectomy operated during the last 30 years is presented. Cases have been subdivided into groups according to stage and TNM classification. By far the most numerous in this series were T2 cases. No cases prevailed over N+ by 4:1. Stage II cases represented about 60% of the total. Results are analyzed according to TNM and staging. The average overall 5-year cure rate is 75%. The influence of factors other than TNM and staging upon results is also considered. Supraglottic laryngectomy may be extended to include one arytenoid, the base of the tongue, or bo
