Enduring dysmetria and impaired gain adaptivity of saccadic eye movements in Wallenberg's lateral medullary syndrome.

Saccadic eye movements and the adaptive control of their amplitudes were examined in patients with Wallenberg's lateral medullary syndrome. Half of the patients had permanent saccadic dysmetria. Their primary saccades had asymmetric amplitudes: those made in response to an ipsilateral target step (i.e. to the lesion side) tended to be hypermetric and saccades made in response to a contralateral target step were strongly hypometric. Multiple correction saccades were needed for target fixation. The adjustment of the amplitude of artificially induced hypermetric saccades, called gain adaptivity,

An unexpected Holter monitor result: multiple sinus arrests in a patient with lateral medullary syndrome.

A 52-year-old man presented with vomiting, dysphagia, left-sided ataxia and dissociated sensory loss. Diffusion-weighted MRI showed evidence of acute infarct in the left lateral medulla and left medial cerebellar hemisphere, probably secondary to thromboembolism from left vertebral artery dissection. While making an uneventful recovery as an inpatient, a routine 24 h ECG was performed 2 weeks after the stroke to investigate possible paroxysmal atrial fibrillation. The recording instead revealed 56 asymptomatic episodes of sinus arrest, necessitating implantation of a permanent pacemaker to pre

Dysphagia in lateral medullary infarction (Wallenberg's syndrome): an acute disconnection syndrome in premotor neurons related to swallowing activity?

BACKGROUND AND PURPOSE: We have investigated the pathophysiological mechanisms of dysphagia in Wallenberg's syndrome (WS) that are due to lateral medullary infarction (LMI). METHODS: Twenty patients with WS were evaluated by means of clinical and electrophysiological methods that measured the oropharyngeal phase of voluntarily initiated swallowing. For comparison, 22 patients with unilateral hemispheric infarction were investigated during the acute stage of stroke, and 4 patients with unilateral peripheral 9th and 10th cranial nerve palsies were studied. Age-matched 30 healthy control subject

Outcome of Surgery to Improve Swallowing in Patients with Lateral Medullary Syndrome: A Retrospective Cohort Study.

The study investigated surgical interventions to improve dysphagia in patients with lateral medullary syndrome (LMS), along with optimal scheduling of surgery and prognostic factors. In this retrospective, single-center cohort study, dysphagia patients with LMS who underwent surgery between January 2010 and December 2021 were enrolled. The National Outcomes Measurement System (NOMS) was used to classify swallowing function (level 1: only tube feeding, level 5: without tube feeding). Patients were divided into four groups. Groups 1 and 2, LMS onset within 1 year, and groups 3 and 4, onset after

Rehabilitation and Physiotherapy Action Strategy for an Acute Case of Lateral Medullary Syndrome: A Case Report.

Lateral medullary syndrome (LMS) may result from a failure in either the vertebral artery or the posterior inferior cerebellar artery. Stroke is the most common cause of LMS. To achieve bronchial hygiene and improve the patient's condition, chest physiotherapy was initiated due to his confined condition during the acute stage. As a result, a four-week physiotherapy program was established and administered twice daily to the patient, with noticeable results. A physical examination revealed that the patient exhibited tachycardia, dyspnea, intracranial nerve palsies on the left side, thermoanesth

Restricted dissociated sensory loss in a patient with a lateral medullary syndrome: A clinical-MRI study.

BACKGROUND: Various sensory syndromes in lateral medullary infarctions are described. A small variation in the location of a lesion may lead to very different clinical features, owing to the complex anatomy of the medulla oblongata. MRI may identify the location and extent of the ischemic lesions, allowing a clear clinical-anatomical correlation. CASE DESCRIPTION: We describe a man with an ischemic lesion in the right portion of the lower medulla that presented a contralateral impairment of spinothalamic sensory modalities and an ipsilateral impairment of lemniscal modalities with a restricte

Treatment approach and outcomes to severe and chronic dysphagia in lateral medullary syndrome: A case series.

INTRODUCTION: Dysphagia is more profound in lateral medullary syndrome (LMS) patients. LMS patients are susceptible to be dependent on tube feeding for months to years post stroke. The objectives of this study are to characterize and understand the extent of functional impairment in the swallowing phases, to strategize the noninvasive methods as per the deficits present, and to document swallowing outcomes of severe and chronic dysphagia following rehabilitation in patients with LMS. MATERIALS AND METHODS: This is an ambispective observational study on chronic LMS (≥1 year) patients with comp

Vertebral artery dissection presented as lateral medullary syndrome in a patient with migraine: a case report.

PURPOSE: Migraine and artery dissection are both rare causes of ischemic stroke. The mechanism of migraine-related intracranial artery dissection is still unknown. It is proposed that the repeated attack of migraine would make the involved artery more vulnerable to tearing and lead to dissection. CASE REPORT: We describe a 42-year-old female suffering from basilar-type migraine for more than 20 years. The patient complained severe dizziness with hyperventilation while watching television. Initially anxiety and migraine attack were impressed in the emergency room, but dizziness accompanied wit

Cortical Activation and Functional Connectivity Changes during Swallowing in Patients with Dysphagia in Lateral Medullary Syndrome.

Swallowing function is affected in patients with lateral medullary syndrome (LMS) due to impaired swallowing central pattern generator, but it remains unclear whether their cortical function is affected. To determine the level of cortical involvement during swallowing in LMS dysphagia patients. This is a cross-sectional study carried out from May 2023 and January 2024 in China. 21 patients with LMS dysphagia and 20 age-matched healthy controls were recruited. Functional near-infrared spectroscopy with 39 channels was utilized to detect the cortical hemodynamic changes when repeated salivary sw

Opalski Syndrome: An Intricate Case of Lateral Medullary Infarction With Unconventional Neurological Features and Diagnostic Challenges.

Opalski syndrome is a rare variant of Wallenberg syndrome characterized by ipsilateral hemiparesis. We present a case involving a 50-year-old male who exhibited symptoms of right-sided upper and lower limb weakness, dizziness, and ataxia. T2-weighted magnetic resonance imaging (MRI) identified an infarction in the right lateral medulla. This case displayed several unique and atypical features, including the negative Babinski sign, absence of dysphagia, and dysarthria. The diagnosis was confirmed through meticulous clinical examination and advanced radiological investigations, such as MRI. This

Lateral medullary syndrome in a resources limited hospital: A rare clinical anatomical variation of ischemic stroke.

KEY CLINICAL MESSAGE: Although it is rare, physicians should be familiar with the presentation of lateral medullary syndrome (LMS). Urgent neuroimaging is crucial to distinguish LMS from other causes of stroke. The majority experience significant improvement within months. ABSTRACT: Lateral medullary syndrome is a rare type of stroke resulting from a vascular event in the lateral part of the medulla oblongata. Loss of pain and temperature in the ipsilateral side of the face, and contralateral side of the body along with ipsilateral ataxia, vertigo, nystagmus, dysphagia, and hiccups are the ha

[Features of MRI diffusion weighted image in early stage of lateral medullary infarction presenting Wallenberg syndrome].

We described early features of MRI diffusion weighted images (DWIs) in three patients with lateral medullary infarction. Patient 1. A 65-year-old woman who complained of vertigo was admitted. Then, DWIs showed a high signal intensity in the right lateral medulla, though T2 weighted images (T2WIs) did not show any abnormalities. On the next day, when the typical symptoms and signs of lateral medullary infarction appeared, the lesion was recognized on both DWIs and T2WIs. Patient 2. A 48-year-old man was admitted 9 hours after he had dizziness, nausea, and repeated vomitings. When a diagnosis of

[The correlation between dysphagia and involvement of the ambiguous nucleus on MRI in acute-phase lateral medullary syndrome].

In this study, the clinical features and MRI findings of 21 patients admitted for acute lateral medullary syndrome, including 10 patients with dysphagia, were examined. According to Cytoarchitecture of the Human Brain Stem (Olszewski, J & Baxter, D), MRI-identified lesions were classified into four groups based on their location (upper, middle-upper, middle-lower, and lower parts of the medulla oblongata). We also examined whether each lesion involved the ambiguous nucleus (AN). We then studied the correlation between dysphagia and involvement of the AN. Ten patients had dysphagia, which impro
