Percutaneous Left Atrial Appendage Closure: The CHAMPION-like vs CLOSURE-like Patient Phenotype Framework.

AIMS: The near-simultaneous publication of CHAMPION-AF and CLOSURE-AF produced apparently discordant evidence regarding percutaneous left atrial appendage closure (LAAC) for stroke prevention in atrial fibrillation. We aimed to reconcile these findings through a phenotype-based framework and identify the determinants of net clinical benefit. METHODS AND RESULTS: This narrative review integrates randomized trials, registries, consensus documents, and guidelines, focusing on patient selection, procedural risk, post-implant antithrombotic therapy, endpoint construction, competing mortality, and

Innovative Surgical Left Atrial Appendage Closure Technique: Early Experience of Inverted Spiral Closure Technique.

OBJECTIVE: Simultaneous closure of the left atrial appendage (LAA) during cardiac procedures has become a common preventive measure against cardiogenic embolic events associated with atrial fibrillation. However, this strategy encounters limitations during minimally invasive mitral valve surgery through a right minithoracotomy because access to the LAA is limited. The use of endocardial sutures for surgical exclusion of the LAA is also well established but has a notable rate of closure failure. We introduced a new surgical LAA closure technique called the inverted spiral closure technique (ISC

Left Atrial Spontaneous Echo Contrast and Ischemic Stroke in Patients Undergoing Percutaneous Left Atrial Appendage Closure.

Objectives: Spontaneous echo contrast (SEC) in the left atrium (LA) is frequently observed in atrial fibrillation (AF) patients and may lead to thromboembolic events. We aimed to investigate both periprocedural and long-term stroke risks associated with LA SEC in AF patients undergoing percutaneous left atrial appendage closure (LAAC). Methods: A total of 408 consecutive AF patients treated with LAAC between March 2015 and February 2019 were divided into two groups based on preprocedural transesophageal echocardiography: the study group (moderate/severe LA SEC; n = 41) and the control group (n

The Case for Intracardiac Echo to Guide Left Atrial Appendage Closure.

Left atrial appendage closure (LAAC) has become a commonly used alternative to anticoagulation for stroke prevention in patients with atrial fibrillation. There is a growing interest in adopting a minimally invasive procedural approach using intracardiac echocardiography (ICE) and moderate sedation. In this article, we review the rational for and the data supporting ICE-guided LAAC and discuss the pros and cons of this approach.

Early Dislodgment and Migration of a Left Atrial Appendage Closure Device.

A 68-year-old man underwent pulmonary vein isolation with cryoballoon combined with left atrial appendage closure using a LAmbre device. The device was dislodged and embolized early after implantation with no symptoms, and it was retrieved percutaneously. An early in-hospital check of the device position after implantation is important for early recognition of any possible device-related complication.

Prevention and Management of Complications of Left Atrial Appendage Closure Devices.

Atrial fibrillation is associated with an ongoing risk of thromboembolic stroke and systemic embolism due to stasis and thrombus formation within the left atrial appendage (LAA). Transcatheter occlusion or ligation of the LAA represents a potential paradigm shift in the management of stroke prevention in at-risk patients with atrial fibrillation. This review summarizes the types and rates of procedural complications that have been observed with LAA occlusion and ligation; describes strategies that can be implemented to minimize these complications; and discusses management approaches that may

DOAC Score for Predicting Clinical Outcomes After Left Atrial Appendage Closure.

BACKGROUND: The direct-acting oral anticoagulant (DOAC) score has been validated for assessing the bleeding risk in patients with atrial fibrillation (AF). However, data on DOAC scores in patients undergoing percutaneous left atrial appendage closure (LAAC) is limited. This study aimed to evaluate the predictive impact of the DOAC score on clinical events following LAAC and compare it with that of the HAS-BLED (Hypertension, Abnormal renal and liver function, Stroke, Bleeding history or predisposition, Labile international normalized ratio [INR], Elderly [age ≥65 years], Drugs and alcohol conc

Intraprocedural Thrombus Formation During Left Atrial Appendage Closure: A Conservative Approach.

BACKGROUND: Left atrial appendage closure with the Watchman FLX Pro device (Boston Scientific) is an alternative to anticoagulation in patients at high bleeding risk with atrial fibrillation. Intraprocedural thrombus formation is rare and poses a unique challenge. CASE SUMMARY: A 57-year-old man with paroxysmal atrial fibrillation and prior intracranial hemorrhage underwent elective left atrial appendage closure. The Watchman FLX Pro was recaptured twice and was successfully deployed on the third attempt. Shortly after deployment, thrombus was noted at the device's threaded insert. The patien

Displacement of occluder after left atrial appendage closure: A case report.

KEY CLINICAL MESSAGE: Atrial fibrillation is closely associated with thrombotic events. In non-valvular atrial fibrillation, 90% of thrombi are formed by the left atrial appendage. Left atrial appendage occlusion (LAAC) can effectively prevent the detachment of left atrial appendage thrombus during atrial fibrillation, thereby reducing the risk of long-term disability or death caused by thromboembolic events. However, the identification and management of complications in LAAC are also very important. ABSTRACT: The efficacy and safety of left atrial appendage occlusion (LAAC) in preventing non

[A complicated left atrial appendage closure : percutaneous management of LAA rupture].

An 81-year-old patient was referred for left atrial appendage closure. Anatomical LAA analysis by CT scan showed an inverted chicken wing morphology. The procedure was performed through i an infero-anterior transseptal puncture and led to "sandwich" closure strategy using an AMPLATZER AMULET 25 mm device. Despite successful deployment of the occluder, a hemopericardium soon developed related to an iatrogenic LAA perforation/partial rupture and leading to major hemodynamic instability. After pericardocentesis, it was decided to inject activated thrombin into the pericardial sac to achieve in si

Early and late hospital readmissions after percutaneous left atrial appendage closure.

INTRODUCTION AND OBJECTIVES: Percutaneous left atrial appendage closure (LAAC) has emerged as a nonpharmacological alternative for the prevention of thromboembolic events in patients with nonvalvular atrial fibrillation. However, there are few data on readmissions after LACC. The aim of this study was to determine the rate of early (≤ 30 days) and late (31-365 days) readmission after LAAC, and to assess the predictors and clinical impact of rehospitalization. METHODS: This multicenter study included 1419 consecutive patients who underwent LAAC. The median follow-up was 33 [17-55] months, and

Appropriate use criteria of left atrial appendage closure devices: latest evidences.

Atrial fibrillation is the most common arrythmia and it is linked to an increased risk of stroke. Even if anticoagulation therapy reduces the rate of stroke the benefits of this therapy have to be balanced with the increased risk of hemorrhagic event. Left atrial appendage closure is a valid alternative to long-term anticoagulation in patients with atrial fibrillation and high hemorrhagic risk. Actually new devices with different features have been tested and introduced progressively in the clinical practice. Improvements in preprocedural imaging evaluation and the learning curve of the operat

Leak closure following left atrial appendage exclusion procedures: A multicenter registry.

BACKGROUND: Though infrequent, incomplete left atrial appendage closure (LAAC) may result from residual leaks. Percutaneous closure has been described though data is limited. METHODS: We compiled a registry from four centers of patients undergoing percutaneous closure of residual leaks following LAAC via surgical means or with the Watchman device. Leak severity was classified as none (no leak), mild (1-2 mm), moderate (3-4 mm), or severe (≥5 mm). Procedural and clinical success was defined as the elimination of leak or mild residual leak at the conclusion of the procedure or follow-up, respec
