External fixation of fractures in children.

We report a series of 20 children with 24 fractures that were treated using the AO tubular external fixator in a paediatric orthopaedic unit. There were no major complications related to fixator use. There were two cases of superficial pin track sepsis and one case of non-union. There were no cases of refracture after fixator removal or of leg length inequality.

Rehabilitation of patients after lower limb replantations by the bone distraction method.

The length of the lower extremities was equalized by the bone distraction method in four patients after replantation of lower legs. A Volkov-Oganesian or Ilizarov distraction apparatus was applied on the replanted lower leg after corticotomy of the tibia and osteotomy of the fibula, and distraction was carried out over 6 to 11 months in small, measured doses. The lower extremities were lengthened from 4 to 8 cm. The lengths of the lower extremities were fully equalized in two patients; they were partially equalized in the other two patients. This made it possible to achieve successful rehabili

Severe growth retardation following limb lengthening: a case report.

The first limb lengthening was reported by Codivilla in 1905. Common complications reported with this procedure include pain tract infections, angulation deformities, joint contractures, and residual limb length discrepancies. Growth arrest or severe growth retardation in the lengthened bones has rarely been reported. We report a case of a skeletally immature patient with right sided hemihypertrophy who underwent lengthening of the left lower extremity by the method of DeBastiani using Orthofix external fixators. Postoperatively he developed severe growth retardation in the lengthened limb, as

Extramedullary Lengthening After a Distal Femoral Replacement to Improve Bone Stock and Restore Leg Length: A Case Report.

CASE: A 10-year-old boy with a left femur osteosarcoma treated with a growing distal femoral prosthesis developed a 6.5 cm limb-length discrepancy with erosion of the remaining femur. He underwent revision of the replacement with femoral transport and lengthening using an extramedullary lengthening nail. After treatment, his bone stock increased by 14 cm and his discrepancy decreased to 3.6 cm. He ambulates without assistive device using a shoe insert. CONCLUSIONS: Extramedullary lengthening can increase bone stock and improve leg lengths. Multiple procedures may be required, and deformities

Review of Long-Term Sequelae and Management in Paediatric Lower Limb Replantation.

Major limb amputation in a child is a rare but devastating injury associated with significant psychosocial and financial burden. This, combined with the higher growth and remodelling potential despite segmental limb loss, decreases the threshold for replantation in children. Advances in microsurgical techniques and a better understanding of such injuries have led to lower limb replantation becoming a feasible option, with good long-term functional outcomes, even in bilateral amputations. However, long-term sequelae such as limb length discrepancy (LLD) are common, and patients are often subjec

The efficacy of two-hole tension band plates in the treatment of lower extremity limb length discrepancy.

The main purpose of our study was to investigate the efficacy of dual two-hole tension band plates in the treatment of lower extremity limb length discrepancy (LLD). We retrospectively reviewed patients who underwent epiphysiodesis utilizing dual two-hole tension band plates due to lower extremity LLD between January 2012 and June 2018. The efficacy of epiphysiodesis was defined as the inhibited percentage of the expected growth of the physis between two time intervals and was calculated between 0-6 months and 6-18 months as the primary outcome. The relationship of efficacy of epiphysiodesis w

Percutaneous epiphysiodesis using transphyseal screws (PETS) versus tension-band plating (TBP): comparative study of outcomes for correcting limb length discrepancy.

OBJECTIVES: The best effective treatment strategy for limb length discrepancy (LLD) is still being debated. The goal of this study was to compare the efficacy and results of tension-band plating (TBP) and percutaneous epiphysiodesis using transphyseal screws (PETS) for LLD correction. METHODS: From June 2008 to January 2019, children who had lower extremity epiphysiodesis with either TBP or PETS were reviewed retrospectively. At the conclusion of treatment, LLD, angular deformity, and complications were reviewed. The t-test or Wilcoxon rank sum test was used to compare continuous variables. C

Monomelic Maffucci syndrome.

Maffucci syndrome is a rare congenital, non-hereditary condition characterised by presence of multiple enchondromas and haemangiomas. Enchondromatous lesions affecting epiphysial growth plates can lead to angular deformities and leg-length discrepancy in the lower limb. We describe a 12-year-old girl with monomelic Maffucci syndrome affecting her left lower limb. She presented with progressive genu valgus deformity of her left knee. This caused her to limp during her gait and was a cosmetic dissatisfaction. The deformity affected her quality of life. She underwent a supracondylar distal femora

Asymmetric limb lengthening in the treatment of tibial hemimelia caused by osteomyelitis: A case report.

RATIONALE: Tibial hemimelia is known as a rare congenital lower limb deficiency. It has been classified into different types based on Jones classification, and the traditional treatment of tibial hemimelia is amputation. Here we present a variant and unclassified case of tibial hemimelia, which was caused by osteomyelitis. And the lower limb with tibial hemimelia was salvaged by asymmetric limb lengthening. PATIENT CONCERNS: 19-year-old girl had the shortened and curved left lower extremity with walking abnormalities. DIAGNOSIS: The patient's deformity was caused by osteomyelitis of tibia oc

Magnitude of limb lengthening after primary total knee arthroplasty.

Patients will often perceive a change in lower limb length after total knee arthroplasty (TKA). From this observed finding, we asked how frequently does a change in limb length occur after TKA. Preoperative and postoperative full-length standing radiographs were obtained for 102 knees in 98 patients who underwent TKA. Digital radiography software was used to measure the mechanical axis and limb length of the operative and nonoperative legs. Overall, 83% of the knees measured showed an increase in limb length after TKA. Preoperative varus alignment was associated with an average lengthening of

Case reports: Epiphysiodesis in a patient with osteopetrosis.

We describe a 14-year-old girl with intermediate-type osteopetrosis who presented with a 4.8-cm leg-length discrepancy secondary to premature distal femoral growth plate closure. She had a projected leg-length discrepancy of 7 cm at maturity, which was predicted using Moseley charts. The patient was treated successfully with an epiphysiodesis. Her remaining growth was predicted by Moseley charts. A percutaneous epiphysiodesis can treat leg-length discrepancies in patients with osteopetrosis.

Measuring leg-length discrepancy by the "iliac crest palpation and book correction" method: reliability and validity.

OBJECTIVE: To determine the reliability and validity of a clinical measurement of leg-length discrepancy (LLD), by using the iliac crest palpation and book correction (ICPBC) method. DESIGN: Intra- and interrater reliability and validity determinations. SETTING: Rehabilitation center. PARTICIPANTS: Thirty-four healthy subjects, none of whom had an apparent LLD, as determined by iliac crest palpation. INTERVENTIONS: We induced a simulated LLD (7-53 mm) for each subject. To measure the LLD, the examiner performed the ICPBC method by palpating the iliac crests and correcting identified differ

[Leg length differences and rotation defects after femoral shaft fractures in childhood. Therapeutic influence and spontaneous correction].

A follow-up study of 149 pediatric fractures of the femoral shaft (146 patients had a radiographic control) demonstrated a significant influence of multiple reductions and residual axial deviations on the incidence and extent of posttraumatic overgrowth. The presented group of patients did not show a positive correlation between therapeutic shortening at the time of the initial reduction and reduced incidence of long term overgrowth. Neither extent nor incidence of overgrowth could be prevented by this method. We therefore recommend to reduce fractures of the femoral shaft without forshortenin

Clinically Detected Leg Length Discrepancy in Patients With Idiopathic Clubfoot Deformity: Prevalence and Outcomes.

BACKGROUND: This study evaluates the prevalence and outcomes of patients with idiopathic clubfoot and clinically detected limb length discrepancy (LLD). METHODS: This is a retrospective cohort study of idiopathic clubfoot patients in a Research Ethics Board-approved clubfoot registry. Patients with LLD ≥0.5 cm (LLD+) were compared with those with no or <0.5 cm LLD (LLD-). LLD was determined by documented clinical examination. Exclusion criteria included nonidiopathic clubfoot deformity, <2-year follow-up, and incomplete records. RESULTS: Of the 300 patients included, 27 (9.0%) had an LLD, of

Clinical validation of the multiplier method for predicting limb length at maturity, part I.

To validate the accuracy of the multiplier method in predicting bone and limb maturity lengths, radiographs of 60 patients treated for lower limb length discrepancy were measured. Longitudinal limb length data were used to predict maturity lengths of non-epiphysiodesed normal bones and short bones. Mean errors for predictions were 1.1 cm (SD = 0.9) and 1.5 cm (SD = 1.3) for the multiplier method using chronologic age and skeletal age, respectively. Regression correlation values between multiplier method predictions and actual measurements were 0.93 using chronologic age and 0.90 using skeletal

Limb length and girth discrepancy of unilateral congenital clubfeet.

There have been few reports about hypoplasia of the femur and tibia in congenital clubfeet. The subjects of this study were 38 patients with unilateral congenital clubfoot. All the measurements were significantly less in the affected side than in the normal side. Although the decrease in length itself was higher in the distal segment, the tibial length contributed mostly to limb length shortening. There was no significant difference between male and female patients. Only the tibial length and the calf girth were significantly less in patients who had undergone surgery than in those without.
