Posterior capsular opacification (PCO) is a common complication of cataract surgery. The development of PCO is due to a combination of the processes of proliferation, migration, and transdifferentiation of residual lens epithelial cells (LECs) on the lens capsule. PURPOSE: To report the unusual finding of ectopic lens material in an otherwise healthy 5-week-old infant.
METHODS: Case report and literature review.
RESULTS: An asymptomatic 5-week-old female infant was found to have unilateral ectopic lens material in the retrolental space of the left eye associated with a posterior capsular defect.
CONCLUSION: The abnormality is likely embryological in origin, and the established progression for similar conditions means long-term monitoring is required to ensure the best possible visual outcome.

The histochemical staining properties of the abnormal material deposited in the anterior segment of the eye in the pseudoexfoliation syndrome and the zonular ligaments of the lens are essentially the same. Both present the staining characteristics of oxytalan, the microfibrillar component of elastic tissue. Reasons are advanced for regarding the pseudoexfoliative material as a product of the ciliary and lenticular epithelium.

The ultrastructure of lens capsule abnormalities in Alport's syndrome is reported. An anterior lens capsule from a 29-year-old patient with lenticonus who was affected by Alport's syndrome was obtained at phacoemulsification. Continuous (central, circular) curvilinear capsulorhexis (CCC), first introduced by Gimbel and Neuhann, has become increasingly popular, especially for endolenticular phacoemulsification with intracapsular intraocular lens implantation. The principles of simple mechanics are used to facilitate teaching the procedure and to reduce the incidence of complications. A safe and simple method of performing CCC based on these principles is described.

PURPOSE: Posterior capsule opacification (PCO) is a complication after cataract surgery, particularly in children. Epithelial-mesenchymal transition (EMT) of lens epithelial cells, mediated by transforming growth factor beta (TGFβ), contributes to PCO. However, its pathogenesis in children is poorly understood. We correlated cell growth in culture with patient characteristics, studied gene expression of pediatric lens epithelial cells (pLEC), and examined the effects of TGFβ-2 on these cells in vitro.

Clinical echographic examinations were performed in 15 cases of traumatic cataract. As a result certain criteria were established which give some information on the condition of the posterior capsule of the lens. Exfoliation fibrils have a characteristic ultramicroscopic appearance. They are 20-30 nm thick with 10 nm subunits and may be 800 or 900 nm long. They sometimes have a characteristic banding periodicity of about 50 nm. The fibrils have been located in the lens capsule, zonular threads, iris, ciliary body and trabecular meshwork. Extraocular locations have also been found in conjunctival vessels and retroorbital tissue.

We describe a new technique of aspiration of subluxated lens in young patients with Marfan's syndrome. Two small circular anterior capsulorhexis (1.5 mm to 2.0 mm) openings were created, and bimanual irrigation/aspiration was performed by introducing the irrigation cannula through 1 capsular opening and the aspiration cannula through the other.
