Modified Cow-Hitch Suture for Repositioning of Subluxated Scleral-Fixated Rigid Intraocular Lens.

BACKGROUND AND OBJECTIVE: To describe a new technique for repositioning a subluxated scleral-fixated rigid intraocular lens (IOL). PATIENTS AND METHODS: The authors present a modified intraocular threading technique to tie a cow-hitch knot around the eyelet on the dislocated haptic of a scleral-fixated rigid IOL. This technique uses three small corneal incisions to eliminate the need for IOL externalization and minimized the size and number of wounds. RESULTS: Three consecutive cases of subluxated haptics in two patients underwent this procedure. Postoperative IOL centration and alignment we

[Iris hooks and modified capsular tension ring for subluxation lens in patients with Marfan's syndrome].

OBJECTIVE: To evaluate clinical efficiency and safety of the iris hooks and modified capsular tension ring (MCTR) for the treatment of subluxation lens in patients with Marfan's syndrome. METHODS: This study comprised 5 patients (7 eyes) with Marfan's syndrome and subluxation lens. After 3.2 mm clear cornea incision and continuous curvilinear capsulorhexis (CCC), 2 - 4 disposable nylon iris hooks were inserted through the stab incisions and placed in the capsulorhexis to support the capsule and enlarged the CCC. Following irrigation/aspiration, a MCTR with suture in its eyelet was placed in t

Incision capsular bag herniation during subluxated lens surgery: mechanisms, management, and prevention.

PURPOSE: To investigate the mechanism, intraoperative characteristics, management, and prevention of incision capsular bag herniation (ICBH), a previously unreported complication during cataract surgery in eyes with lens subluxation. METHODS: A retrospective observational case series was conducted on five male patients who developed ICBH during phacoemulsification with intraocular lens (IOL) implantation between January 2019 and December 2024. Among 867 subluxated-lens surgeries performed during this period, the estimated incidence of ICBH was 0.58%. Clinical data was reviewed to identify pre

Scleral fixation of a dexamethasone intravitreal implant in a case of subluxated bag-intraocular lens complex.

Macular edema is a condition of retinal tissue treated with anti-inflammatory agents including placement of an intravitreal sustained-release dexamethasone device, designed to deliver a controlled amount of the medication for a prolonged time, representing an excellent therapy. Nonetheless, the implantation cannot be carried out without an anatomical barrier, such as the presence of posterior capsular support, lens, or intraocular lens. The absence of these barriers could lead to several complications, due to migration of the device from the vitreous cavity to the anterior chamber, causing cor

Surgical Management of a Patient with Anterior Megalophthalmos, Lens Subluxation, and a High Risk of Retinal Detachment.

The early development of lens opacities and lens subluxation are the most common causes of vision loss in patients with anterior megalophthalmos (AM). Cataract surgery in such patients is challenging, however, because of anatomical abnormalities. Intraocular lens dislocation is the most common postoperative complication. Patients with AM also seem to be affected by a type of vitreoretinopathy that predisposes them to retinal detachment. We here present the case of a 36-year-old man with bilateral AM misdiagnosed as simple megalocornea. He had a history of amaurosis in the right eye due to reti

Intraocular lens subluxation obstructing a glaucoma tube shunt after a Valsalva maneuver: a case report.

BACKGROUND: Late subluxation of an intraocular lens (IOL) within the capsular bag is a rare complication of cataract surgery, primarily associated with progressive zonular weakening in conditions such as pseudoexfoliation syndrome, high myopia, prior vitreoretinal surgery, connective tissue disorders, or ocular trauma. While IOL-capsular bag subluxation is well-documented, its impact on glaucoma drainage devices remains poorly understood. We present a rare case of Valsalva-induced IOL subluxation obstructing an Ahmed glaucoma valve, resulting in acute intraocular pressure (IOP) elevation in a

Unilateral fronto-temporal headache with ocular pain caused by lens subluxation due to spontaneous zonulysis.

A 52-year-old man presented to the emergency department (ED) for left frontotemporal headache and left eye pain. In the ED, visual exam with extra-ocular movement was performed to rule out ocular pathology and brain imaging to rule out cerebral pathology. No abnormal finding was noted after evaluation of ED. Initial exam by an ophthalmologist was also negative with normal intraocular pressure. Left lens dislocation was found only after brain imaging. In subsequent second exam by an ophthalmologist with iridodilator, lens dislocation due to spontaneous zonulysis was finally confirmed. Hence, ED

Ciliary epithelium detachment during lens subluxation extraction surgery after blunt eye trauma: A case report.

PURPOSE: To document the ciliary epithelial detachment identified intraoperatively during lens-subluxation extraction in a patient with blunt eye trauma. METHOD: A 60-year-old Chinese man presented 14 days after right eye blunt trauma with blurry vision, elevated intraocular pressure (IOP), temporal shallowing of the anterior chamber, and iris bombé 1. To relieve pupillary-block-related IOP elevation, phacoemulsification was performed. Intraoperatively, the lens capsule ruptured, and portions of the nucleus and cortex dropped into the vitreous cavity. The surgeon switched to a cutter to remov

Visual outcomes of lens subluxation surgery with Cionni modified capsular tension rings in Marfan syndrome.

Marfan syndrome (MFS) is a hereditary disease with an incidence of 0.3% in the general population. Approximately 60% of MFS patients with FBN1 gene mutation will suffer ectopia lentis (EL) from the age of 3. With the development of EL, severe loss of vision will accrue because of lens tilt and glaucoma. Cionni modified capsular tension rings (MCTR) has been applied in the surgery for EL in MFS patients. To evaluate visual acuity and safety of using MCTR during lens subluxation surgery in MFS patients, 66 MFS patients (110 eyes) were included in our study, with the mean duration of follow-up of

Corneal Curvature, Astigmatism, and Aberrations in Marfan Syndrome with Lens Subluxation: Evaluation by Pentacam HR System.

Marfan syndrome (MFS) is associated with abnormalities of corneal biometric characteristics. We conducted a retrospective case-control study including 55 eyes of the MFS patients with lens subluxation and 53 normal eyes of the control subjects to evaluate the corneal curvature, astigmatism and aberrations using a rotating Scheimpflug camera (Pentacam HR). Compared with the control group, the anterior, posterior, and total corneal curvature were flatter in the MFS group. The anterior and total corneal astigmatism were higher in the MFS patients, whereas the posterior corneal astigmatism was not

Intraocular lens subluxation and decompensated pseudoexfoliation glaucoma in a patient's only reading eye May consultation #1.

An 89-year-old man had cataract surgery in 2005 in both eyes. At that time, he already had glaucoma that was well regulated with topical therapy and drusen maculopathy with incipient areal atrophy. Fifteen years later, the patient presented with a decrease in visual acuity of his only reading eye, his right eye. Visual acuity 1 year before presentation was reported as 0.5, Jg2 in his right eye, 0.2, Jg15 in his left eye with drusen maculopathy in both eyes, and an atrophic macular scar in the left eye. Slitlamp biomicroscopy showed an inferotemporally decentered and wobbling intraocular lens (

Late spontaneous bilateral intraocular lens subluxation accompanied with intraocular pressure elevation in a patient with acromegaly.

A 53-year-old male with newly diagnosed acromegaly came to our clinic with the chief complaint of diplopia. He had the past ocular history of uneventful phacoemulsification cataract surgery with intraocular lens (IOL) implantation in the right eye 17 years ago and left eye 15 years ago. Postoperative examination showed remarkable improvement in visual acuity. Two years ago, he developed elevated intraocular pressure (IOP) in both eyes, which was well-controlled with the use of travoprost 0.004%/timolol 0.5%. At the clinic, slit-lamp examination revealed inferiorly subluxated IOL bilaterally. T
