Capsule-augmented scleral fixation of intraocular lenses.

In-the-bag placement of intraocular lenses (IOLs) generally yields optimal results in terms of alignment and minimizes decentration or tilt. However, this approach can be counterproductive if the capsular bag is compromised, potentially leading to IOL malposition or inability to secure the lens. Repurposing the compromised capsular bag to its maximum potential can help place the initially intended IOL. This surgical series showcases techniques and options for scleral fixation of the same or a newer IOL, using the compromised bag as augmented support. The capsular bag provides alignment guidanc

Toric intraocular lenses: advancements in astigmatism correction.

PURPOSE OF REVIEW: Toric intraocular lenses (IOL) predictably correct corneal astigmatism and are an important part of the cataract and refractive surgeon's toolbox. RECENT FINDINGS: Advances in toric IOL technology, preoperative biometry, IOL power calculations, and IOL alignment have resulted in excellent visual outcomes and achievement of spectacle independence. Despite these advancements, the primary challenges faced in toric IOL use remain choosing the optimal IOL toricity and properly aligning the IOL. SUMMARY: Toric IOL are an effective way to correct corneal astigmatism, and advancem

Clinical outcomes of trifocal toric intraocular lenses.

The purpose of this report is to summarize the visual and refractive outcomes of patients with trifocal toric intraocular lens (IOL) implants. A peer-reviewed literature search in different databases was carried out to identify clinical publications reporting outcomes of patients with this type of implant. The analysis considered information on the sample of eyes, type and power of the trifocal toric IOLs, biometric preoperative data, postoperative refraction, rotational stability, visual acuity at different distances and other analyses undertaken, such as contrast sensitivity or quality of vi

Refractive accuracy with light-adjustable intraocular lenses.

PURPOSE: To evaluate efficacy, predictability, and stability of refractive treatments using light-adjustable intraocular lenses (IOLs). SETTING: University Hospital Virgen de la Arrixaca, Murcia, Spain. DESIGN: Prospective nonrandomized clinical trial. METHODS: Eyes with a light-adjustable IOL (LAL) were treated with spatial intensity profiles to correct refractive errors. The effective changes in refraction in the light-adjustable IOL after every treatment were estimated by subtracting those in the whole eye and the cornea, which were measured with a Hartmann-Shack sensor and a corneal top

Pseudoexfoliation material on posterior chamber intraocular lenses.

The presence of pseudoexfoliation material on the surface of an intraocular lens (IOL) is a rare finding. We report a series of seven cases with different patterns of pseudoexfoliation material deposition on the posterior chamber IOLs, recognized 2-20 years after cataract surgery. Six patients had an IOL implanted in the capsular bag and one in the ciliary sulcus. Two patients had undergone posterior capsulotomy. Although the pathophysiological mechanisms and clinical significance of this finding remain unknown, the careful follow-up of pseudophakic patients with known or suspected pseudoexfol

[Anterior chamber intraocular lenses for aphakia correction].

The intraocular correction of aphakia in the anterior chamber currently consists of two options: an angle supported intraocular lens (IOL, Kelman Multiflex) and an iris fixated IOL (Artisan/Verisyse). The angle supported IOL is fixated with four haptic points in the anterior chamber and the iris fixated IOL is enclavated to the anterior iris surface. Acceptable results of both IOLs are reported in the literature; however both IOLs show some long-term complications in some eyes. Alternatives to these anterior chamber IOLs to correct aphakia are the retropupillary fixated Artisan and sclera fixa

Interlenticular opacification of piggyback acrylic intraocular lenses.

A dense interlenticular opacification developed 18 months after cataract surgery in a hyperopic patient with 2 acrylic piggyback intraocular lenses (IOLs) placed in the capsular bag. The patient developed a hyperopic shift with decreased vision, requiring explantation of the lenses. The dense interlenticular opacification did not allow separation of the lenses intraoperatively because of the tight adherence of the 2 IOLs.

[Implantation of bifocal intraocular lenses. Initial experience].

Twelve patients with bifocal intraocular lenses (3M, Suncoast and IOLAB) were evaluated. In 4 patients authors were not able to evaluate bifocality because of macular disorders and optic atrophy. In all remaining 8 patients the bifocality was strongly expressed. The main problem is a postoperative ammetropia. Implantation of bifocal lenses is connected with side effects which are not usually seen in implantations of monofocal intraocular lenses.

Management of dislocated posterior chamber intraocular lenses.

Management options for posteriorly dislocated posterior chamber intraocular lenses include observation, removal, exchange, and repositioning. Many microsurgical techniques have been developed for repositioning posterior chamber implants. These include repositioning into the ciliary sulcus without suturing if adequate posterior capsule support remains, iris fixation suturing techniques, and scleral fixation suturing techniques. The indications, timing, and techniques for intervention are reviewed in a series of 32 cases with posteriorly dislocated posterior chamber implants. A final visual acui

Corneal endothelial loss with new intraocular lenses.

When we compared the effect of a hydrogel intraocular lens on the corneal endothelium of rabbits to the damage produced by uncoated methylmethacrylate intraocular lenses and methylmethacrylate lenses coated with sodium hyaluronate or methylcellulose, we found that the endothelial damage produced by the hydrogel lenses in a standard 0.25 mm2 of contact was 3.6%. This value was not significantly different from that for the control corneas (0.4%). Uncoated methylmethacrylate lenses caused 62% endothelial loss but coating them with sodium hyaluronate or methylcellulose reduced the loss to 27% and

[Complication profiles of posterior chamber intraocular lenses IOL). An analysis of 586 foldable and 2077 rigid explanted intraocular lenses].

BACKGROUND: Foldable intraocular lenses (IOL) have become increasingly preferred choice for IOL implantation after cataract removal. However, both foldable as well as rigid IOLs are not yet complication-free and may need explanation. MATERIAL AND METHODS: A total of 2663 explanted posterior chamber IOLs (PCIOLs) were accessioned at the Center for Research on Ocular Therapeutics and Biodevices between January 1988 and September 2000. The lenses were examined grossly using a Leitz/Wild M-8-Zoom stereomicroscope. The clinical reasons for explanation were documented for foldable as well as for ri

BCLA CLEAR presbyopia: Management with intraocular lenses.

Cataract surgery including intraocular lens (IOL) insertion, has been refined extensively since the first such procedure by Sir Harold Ridley in 1949. The intentional creation of monovision with IOLs using monofocal IOL designs has been reported since 1984. The first reported implantation of multifocal IOLs was published in 1987. Since then, various refractive and or diffractive multifocal IOLs have been commercialised. Most are concentric, but segmented IOLs are also available. The most popular are trifocal designs (overlaying two diffractive patterns to achieve additional focal planes at int

Phakic intraocular lenses: Recent advances and innovations.

Phakic intraocular lenses (pIOL) are recommended when counselling refractive surgery candidates presenting with high ametropia or ocular surface and/or corneal conditions that contraindicate corneal refractive surgery. This review aims to present the state-of-the-art regarding pIOL models currently available in Europe, addressing their newer indications and recent design innovations. These include, in the case of posterior chamber pIOLs, the addition of a central hole to improve aqueous humour circulation, the availability of larger optical zones, and multifocal optics for the compensation of

Scleral-Fixated Intraocular Lenses: Past and Present.

Intraocular lenses (IOLs) can have inadequate support for placement in the capsular bag as a result of ocular trauma, metabolic or inherited conditions such as Marfan's syndrome or pseudoexfoliation, or complicated cataract surgery. Surgical options for patients with inadequate capsular support include alternative placement in the anterior chamber (ACIOLs), fixation to the iris, or fixation to the sclera. The surgical techniques for each of these approaches have improved considerably over the last several decades resulting in improved visual and ocular outcomes. If no capsular or iris support
