Face-to-face, confidential and health worker-led: Understanding the preferences for behavioral health services among people with HIV in Lesotho.

Behavioral treatments can help people with human immunodeficiency virus (HIV) improve their quality of life and treatment adherence. In Lesotho, where a fifth of adults live with HIV and depression and harmful alcohol use is prevalent among this group, little is known about the local suitability of established behavioral treatment strategies. We explored preferences regarding two strategies evaluated in other settings: involving phones and trusted individuals in treatment. We thematically analyzed 28 semi-structured interviews with potential service users receiving routine HIV care in the Buth

Rapid antiretroviral therapy initiation in patients with advanced HIV disease: 6-month outcomes of an observational cohort evaluation in Lesotho.

For adults and adolescents, the World Health Organization defines advanced HIV disease (AHD) as a CD4 (cluster of differentiation 4) count of <200 cells/mm3 or a clinical stage 3 or 4 event. We describe clinical outcomes in a cohort of AHD patients at two regional hospitals in Lesotho. From November 2018-June 2019, we prospectively enrolled eligible patients (≥15 years) not on antiretroviral therapy (ART) presenting with WHO-defined AHD into a differentiated model of care for AHD (including rapid ART initiation) and followed them for six months. All patients received Tuberculosis (TB) symptom

Diabetes mellitus screening and its associated factors among women in Lesotho: evidence from the 2023-2024 demographic and health survey.

BACKGROUND: Diabetes screening is one of the simpler methods for preventing and lowering the disease's morbidity and mortality. Screening for gestational diabetes is a crucial component of obstetric care, as it negatively affects the life of both the mother and the fetus. There is a high prevalence of obesity, diabetes, high blood pressure, and tobacco use in Lesotho. This highlights the significance of bolstering prevention and treatment initiatives in the country. Therefore, this study is intended to assess diabetes screening and its associated factors among women aged 15-49 years using the

Integrating clinical decision support and mobile health for differentiated HIV service delivery in Lesotho (VITAL): a cluster-randomised non-inferiority trial.

BACKGROUND: The World Health Organization conditionally endorses digital interventions to strengthen health systems, while acknowledging limited evidence on their benefits and harms. We assessed the effectiveness of digital health-supported differentiated service delivery on HIV treatment outcomes in rural southern Africa. METHODS: The VITAL pragmatic, open-label, parallel-group, non-inferiority, cluster-randomised controlled clinical trial enrolled adults with HIV taking antiretroviral therapy (ART) at 18 rural, nurse-led clinics in Lesotho. Clinics were randomised to receive either digital

Polio vaccination and marginal effects among children aged 12-23 months in Lesotho using 2024 LDHS data: A multilevel analysis approach.

BACKGROUND: Infectious diseases that have affected countless newborns, children, and adults can be avoided with polio vaccination. The WHO and Lesotho's health strategy placed a strong emphasis on vaccination as a means of preventing and controlling infectious diseases. The current study's goal was to indicate marginal effects and identify the determinants of polio vaccination status in children between the ages of 12 and 23 months. METHODS: The polio vaccination status in Lesotho was evaluated using LDHS data collected from 490 children aged 12-23 months. The prevalence of polio vaccination

Associations Between Substance Use, Sexual Violence, Mental Distress, and Sexual HIV Risk Behaviors Among Adolescent Girls and Young Women in Lesotho.

Adolescent girls and young women (AGYW) are disproportionately burdened by new HIV infections in eastern and southern Africa. Data on co-occurrence of behavioral risk factors and associations with sexual HIV risk behaviors can inform layered and targeted HIV prevention efforts. Using nationally representative data of AGYW aged 13-24 years from the 2018 Lesotho Violence Against Children and Youth Survey, we categorized the population into distinct adversity groups comprised of different pattens of sexual violence exposure, substance use, and moderate/severe mental distress. We estimated associa

Preventing tuberculosis among HIV-infected pregnant women in Lesotho: the case for rolling out active case finding and isoniazid preventive therapy.

BACKGROUND: The Lesotho Ministry of Health issued guidelines on active case finding (ACF) for tuberculosis (TB) and isoniazid preventive therapy (IPT) in April 2011. ACF has been recommended in maternal and child health (MCH) settings globally, however, the feasibility of implementing IPT within MCH in countries with high concurrent HIV and TB epidemics is unknown. DESIGN/METHODS: The study evaluated the implementation of ACF and IPT guidelines in MCH settings in 2 health facilities in Lesotho. This descriptive prospective study analyzed data collected during routine services. Categorical dat

Impact of the DREAMS Program on New HIV Diagnoses in Adolescent Girls and Young Women Attending Antenatal Care - Lesotho, 2015-2020.

Lesotho is a small, landlocked country in southern Africa with a population of approximately 2 million persons, approximately two thirds of whom live in rural areas (1). Lesotho has the second highest prevalence of HIV infection in the world (2). In 2017, 25.6% of persons aged 15-59 years living in Lesotho were HIV-positive (3). Strategies implemented in recent years to control HIV include efforts to reduce mother-to-child transmission and improve coverage with antiretroviral therapy, as well as increasing testing for HIV. Among persons aged 15-24 years, the HIV prevalence among females in 201

Prevalence and determinants of cigarette smoking and smoking frequency among people with tuberculosis in Lesotho: Evidence from a nationwide cross-sectional survey.

BACKGROUND: Low- and middle-income countries face a significant dual burden of tuberculosis (TB) and cigarette smoking. Nevertheless, studies examining the prevalence and determinants of smoking status and frequency among people with TB (PWTB) are scarce in sub-Saharan Africa, including Lesotho. Therefore, this study evaluated the prevalence and determinants of smoking status and frequency among people with TB in Lesotho. METHODS: Data on people with TB from the 2023 Lesotho Demographic and Health Survey were used in this study (n = 326). The data were adjusted for sampling weight, stratifica

A cross-sectional study on domestic use of biomass fuel and the prevalence of respiratory illnesses in a rural community in Thaba-Tseka district of Lesotho.

The domestic utilization of biomass fuel for purposes such as cooking, space heating, and water heating has been linked to a number of respiratory ailments, particularly when burned inefficiently. However, there is an existing knowledge gap on the impact of this practice on the health of Basotho. This study aims to explore the impact of biomass fuels use on the prevalence of respiratory illnesses among residents of two rural communities in Thaba-Tseka. A quantitative, cross-sectional design was adopted, using a structured questionnaire, to assess the correlation between biomass fuel use and th

Medically, Traditionally, and Dually Circumcised Men in Lesotho: Population-Based Measurements of HIV/STI Infections, Sexual Risk Behaviors, and Service Use Patterns.

Voluntary medical male circumcision (VMMC) is an HIV prevention priority in Lesotho, but uptake remains suboptimal. We analyzed the 2014 Lesotho Demographic and Health Survey to assess population-level social, behavioral, and serological correlates of circumcision status, specifically traditional and/or medical circumcision. Among 2931 men, approximately half were traditionally circumcised, and fewer than 25% were medically circumcised. Only 4% were dually (traditionally and medically) circumcised. In multivariate analysis, only medical circumcision emerged as significantly (p < 0.05) protecti

Same-day versus rapid ART initiation in people with HIV and symptoms of tuberculosis (SaDAPT): a randomised, non-inferiority trial in Lesotho and Malawi.

BACKGROUND: Whether same-day antiretroviral therapy (ART) should be offered to people with HIV with tuberculosis symptoms or whether ART should be initiated only after tuberculosis diagnostic results are available is unknown. This randomised trial aimed to determine whether same-day ART initiation for people with tuberculosis symptoms is non-inferior to deferral until tuberculosis diagnostic results are available in achieving viral suppression at 6 months. METHODS: We did an open-label, individually randomised, non-inferiority trial among people with HIV who were outpatients at 11 primary or

Removal of helminth eggs by centralized and decentralized wastewater treatment plants in South Africa and Lesotho: health implications for direct and indirect exposure to the effluents.

Wastewater may contain contaminants harmful to human health; hence, there is the need for treatment before discharge. Centralized wastewater treatment systems are the favored treatment options globally, but these are not necessarily superior in reduction of pathogens as compared to decentralized wastewater treatment systems (collectively called DEWATS). This study was therefore undertaken to assess the soil-transmitted helminth (STH) and Taenia sp. egg reduction efficiency of selected anaerobic baffled reactors and planted gravel filters compared to centralized wastewater treatment plants in S

Depressive symptoms and substance use as mediators of stigma affecting men who have sex with men in Lesotho: a structural equation modeling approach.

PURPOSE: Research on the relationship between sexual orientation-related stigma and risks for HIV among men who have sex with men (MSM) is limited. This study tests a hypothesis that substance use and depressive symptoms mediate the relationship between stigma in the health care system and HIV-related risk practices among MSM in Maseru, Lesotho. METHODS: In 2014, we conducted a cross-sectional study among MSM in Lesotho accrued via respondent-driven sampling including a survey and biological testing for HIV. The hypothesis was tested using structural equation modeling. RESULTS: Of the 318 pa

Cost and cost-effectiveness of switching from d4T or AZT to a TDF-based first-line regimen in a resource-limited setting in rural Lesotho.

BACKGROUND: Latest World Health Organization guidelines recommend shifting away from Stavudine (d4T)-based regimens due to severe side effects. However, widespread replacement of d4T by Tenofovir (TDF) or Zidovudine (AZT) is hampered by cost concerns. METHODS: We established the cost-effectiveness of alternative first-line regimens using primary utilization, cost, and outcome data from a program in a rural district in Lesotho. We calculated cost per patient-year, incremental costs, and incremental cost-effectiveness ratios per life year, and per Quality Adjusted Life Year gained. Uncertainty

Comparison of clinical diagnosis and standard laboratory and molecular methods for the diagnosis of genital ulcer disease in Lesotho: association with human immunodeficiency virus infection.

A multiplex polymerase chain reaction (M-PCR) assay for Haemophilus ducreyi, Treponema pallidum, and herpes simplex virus (HSV) was compared with clinical and standard laboratory methods for the diagnosis of genital ulcer disease (GUD) in 105 patients; 36% were human immunodeficiency virus (HIV)-seropositive. Chancroid (80%), syphilis (8%), and genital herpes (8%) were the most frequent diagnoses. H. ducreyi and HSV were isolated from ulcers of 43% and 18% of patients, respectively; in 35%, all cultures were negative and the laboratory diagnosis indeterminate. M-PCR detected H. ducreyi, T. pal

Investigating the Factors Influencing Traditional Male Circumcision and Its Contribution to HIV Transmission Amongst Men in Lesotho: A Multilevel Binary Logistic Regression Approach.

BACKGROUND: Traditional Male Circumcision (TMC) has been practiced in many parts of the world. However, the impact thereof on reducing HIV transmission is still unclear. This study aimed to examine the prevalence and determinants of TMC and the associated risk of HIV transmission in Lesotho. METHOD: Using data from the 2023-24 Lesotho Demographic and Health Survey, the analysis included a weighted sample of 3202 men aged 15-59 years. Missing data was addressed through multiple imputations, and multilevel logistic regression was used to assess the factors associated with TMC, incorporating int

An integrated, multidisciplinary management team intervention to improve patient-centeredness, HIV, and maternal-child outcomes in Lesotho: formative research on participatory implementation strategies.

BACKGROUND: Reducing perinatal HIV transmission and optimizing maternal and child health (MCH) outcomes in high HIV prevalence settings is an urgent, but complex, priority. Extant interventions over-emphasize individual-level provider and patient behaviors, and neglect critical health systems-level changes. The 'Integrated Management Team to Improve Maternal-Child Outcomes (IMPROVE)' study implemented a three-part, patient-centered, health-systems-level intervention to improve MCH and HIV outcomes in Lesotho. Ensuring intervention fit within the health systems context is important, but often o
