A case of Gitelman syndrome with severe hyponatraemia and hypophosphataemia.

Gitelman syndrome (GS) is a renal tubular disorder of the thiazide-sensitive sodium chloride cotransporter, which is located in the distal tubule of the loop of Henle. We present a rare case of GS complicated by severe hyponatraemia and hypophosphataemia. A 17-year-old boy was admitted to our institution with fever and lethargy. The workup revealed typical features of GS, i.e. hypokalaemia, hypomagnesaemia and metabolic alkalosis. In this report, we discuss the differential diagnoses and rationale for accepting GS as the most likely diagnosis. This case was complicated by severe hyponatraemia

Acute B-cell lymphoblastic leukaemia in a 5-month-old boar.

CASE HISTORY: A 5-month-old mixed-breed boar presented with lethargy, weakness and anorexia of 1 week's duration. CLINICAL FINDINGS AND DIAGNOSIS: The boar had an elevated heart rate, slow capillary refill time and cyanotic skin over the ears; it was also severely anaemic. A necropsy revealed scattered petechial haemorrhages, small subcapsular white nodules on the kidneys, a nodule in the parenchyma of one testis, creamy-white bone marrow, a preputial diverticulum, and an ulcer of the pars oesophagea of the stomach. Cytology of peripheral blood and bone marrow identified large numbers of lymp

[Hypertensive manifestation of an acute intestinal intussusception].

BACKGROUND: Hypertension may be associated with intussusception. CASE REPORT: An 8-month-old infant showed the following symptoms: lethargy, vomiting and hypertension. Abdominal ultrasound suggested the diagnosis of intussusception, which was confirmed by barium enema. The hypertension resolved after the intussusception was reduced. CONCLUSION: Intussusception should be considered a diagnostic possibility in infants who show a history of vomiting and in whom lethargy and systematic hypertension are noted. This case re-affirms the diagnostic usefulness of abdominal ultrasonography.

Tuberculous meningitis presenting with unusually severe hyponatremia.

We report a patient with tuberculous meningitis who presented with unusually severe hyponatremia, an electrolyte disorder that may cause symptoms similar to those of tuberculous meningitis. The hyponatremia was probably due to the syndrome of inappropriate antidiuretic hormone secretion, and resolved after instituting water restriction and antituberculous medication.

Clinical and Laboratory Factors Associated with Severe Dengue: A Case-Control Study of Hospitalized Children.

BACKGROUND: More than half of the hospitalizations because of dengue in Brazil occurred in children <15 years of age in 2007 and 2008, an unexpected change in the epidemiological pattern. We sought to determine clinical and laboratory parameters associated with severity. METHODS: A case-control study was conducted in three pediatric hospitals in Rio de Janeiro, Brazil; 233 laboratory-confirmed dengue patients were included: 69 cases and 164 controls. Specific clinical and laboratory factors were assessed using univariate and multivariate logistic regression models. RESULTS: Lethargy [adjuste

Investigating the relationship between the intoxicated drug and clinical profile of children with drug poisoning: a cross-sectional study in a tertiary referral hospital.

BACKGROUND: Pediatric drug poisoning is a significant public health concern, with opioids being the most common agents in poisoning cases. Despite its importance, limited data are available on the relationship between clinical presentations, laboratory findings, and the age of affected children. OBJECTIVES: This study aimed to evaluate the clinical and laboratory profiles of pediatric drug poisoning cases and to assess age-related variations in symptoms and laboratory markers. METHODS: A retrospective analytical cross-sectional study was conducted in the emergency department, from April to O

Electrocardiographical case. A tale of tall T's. Hyperkalaemia.

A 63-year-old woman presented at the emergency department (ED) with a history of increasing lethargy and drowsiness. The electrocardiogram (ECG) showed tall peaked T waves with broadening of the QRS interval, suggestive of hyperkalaemia. This patient had an elevated serum potassium level due to diabetic ketoacidosis. She was treated with intravenous calcium chloride and insulin with 50% dextrose. The ECG changes associated with hyperkalaemia are discussed, with illustrations from a second 48-year-old male patient with renal failure who presented with malaise, lethargy and generalised weakness.

Management of perianal squamous cell carcinoma with permanent colostomy in a mare.

A 28-year-old Pinto mare was evaluated for multiple recurrences of a perianal squamous cell carcinoma. Clinical signs included lethargy, inappetence, weight loss, dyschezia, and hematochezia. The tumor involved the proximal portion of the vulvar labia, the perineum, and the anus, and there was a rectal fistula just left of the anus. Permanent end-on colostomy without further treatment of the carcinoma was the management option chosen. The colostomy functioned satisfactorily, and the mare's condition was stable for the next 14 months. Recurrence of lethargy and inappetence and development of we

Magnesium sulfate treatment for juvenile ferrets following induction of hydrocephalus with kaolin.

BACKGROUND: Previous work with 3-week hydrocephalic rats showed that white matter damage could be reduced by the calcium channel antagonist magnesium sulfate (MgSO4). We hypothesized that MgSO4 therapy would improve outcomes in ferrets with hydrocephalus induced with kaolin at 15 days. METHODS: MRI was performed at 29 days to assess ventricle size and stratify ferrets to treatment conditions. Beginning at 31 days age, they were treated daily for 14 days with MgSO4 (9 mM/kg/day) or sham saline therapy, and then imaged again before sacrifice. Behavior was examined thrice weekly. A case report of fatal feline babesiosis caused by Babesia canis in north western Spain.

BACKGROUND: In Europe, Babesia infections in cats are sporadic and only partial knowledge is currently available since the number of described cases including both the clinical presentation and the molecular identification of the Babesia species involved is limited. In the present case report, the clinical signs, the epidemiological data and the molecular results suggest that this is the first reported fatal case of feline babesiosis caused by Babesia canis. CASE PRESENTATION: A six month old female European shorthair cat from north-western Spain died after being hospitalized for two days. Th

A Rare Cause of Pancytopenia in an Exclusively Breastfed Infant.

Vitamin B12 (B12) deficiency in infancy can present with nonspecific symptoms. We report a 5-month old exclusively breastfed full-term infant with emesis, lethargy, progressive pancytopenia, hemolysis, hypofibrinogenemia, elevated lactate dehydrogenase and a hypercellular bone marrow with dyserythropoiesis. The B12 level in the serum was undetectable. The infant's lethargy resolved within 48 hours of intramuscular B12 injection, followed by rapid improvement of pancytopenia. The asymptomatic mother had a normal hemoglobin and mean corpuscular volume, but undetectable B12 level and positive ant

Within- and across-day patterns of interplay between depressive symptoms and related psychopathological processes: a dynamic network approach during the COVID-19 pandemic.

BACKGROUND: In order to understand the intricate patterns of interplay connected to the formation and maintenance of depressive symptomatology, repeated measures investigations focusing on within-person relationships between psychopathological mechanisms and depressive components are required. METHODS: This large-scale preregistered intensive longitudinal study conducted 68,240 observations of 1706 individuals in the general adult population across a 40-day period during the COVID-19 pandemic to identify the detrimental processes involved in depressive states. Daily responses were modeled usi

Vasculitis secondary to presumptive leptospirosis treated with long-term corticosteroids in a captive lesser kudu (Tragelaphus imberbis australis).

A 2-yr-old female lesser kudu (Tragelaphus imberbis australis) was presented for lethargy. Empirical antibiotic treatment appeared to improve its clinical signs, although no etiology for the symptoms was determined. The kudu again presented with lethargy, diffusely swollen limbs, and subcutaneous ecchymoses of 1 day's duration after completion of the initial therapy. Vasculitis secondary to presumptive leptospirosis infection was diagnosed based on a skin biopsy and decreasing paired serologic titers for Leptospira grippotyphosa. The vasculitis was responsive to intramuscular antibiotic therap

Severe lactic acidosis due to thiamine deficiency in a patient with B-cell leukemia/lymphoma on total parenteral nutrition during high-dose methotrexate therapy.

An 11-month-old girl with B-cell leukemia/lymphoma developed profound lethargy due to severe lactic acidosis during chemotherapy and total parenteral nutrition (TPN). Initial treatment with NaHCO3 was ineffective. Treatment with a vitamin cocktail (OH-cobalamin, pyridoxine, thiamine, riboflavine, biotin, carnitine) at pharmacologic doses rapidly improved the child's clinical and laboratory status. Lactic acidosis was caused by an impairment of pyruvate dehydrogenase complex, which was due to lack of its necessary cofactor thiamine in the TPN. This case report indicates that lactic acidosis may
