Adhesion molecules and cerebral microvascular hemodynamic abnormalities in sickle cell disease.

Cerebrovascular abnormalities are a common feature of sickle cell disease that may be associated with risk of vaso-occlusive pain crises, microinfarcts, and cognitive impairment. An activated endothelium and adhesion factors, VCAM-1 and P-selectin, are implicated in sickle cell vasculopathy, including abnormal hemodynamics and leukocyte adherence. This study examined the association between cerebral expression of these adhesion factors and cortical microvascular blood flow dynamics by using in-vivo two-photon microscopy. We also examined the impact of blood transfusion treatment on these marke

[Immunoreaction and blood transfusion--chairmen's introductory remarks].

Although blood transfusion is an extremely important therapeutic procedure that usually proceeds without complications, there are some risks associated with donated blood. Investigations into the causes of transfusion reactions and their prevention are important issues for transfusion therapy. In addition to nucleic acid amplification testing (NAT) for infectious diseases and the irradiation of blood to prevent post-transfusion GVHD, prestorage leukocyte reduction and diversion of the first part of the donation of blood were recently introduced into transfusion therapy. This symposium, entitle

Transfusion-related acute lung injury: transfusion, platelets and biological response modifiers.

Transfusion-related acute lung injury (TRALI) may be induced by plasma, platelet concentrates and red blood cell concentrates. The mechanism leading to TRALI is thought to involve two steps. The priming step consists of previous inflammatory pathological conditions or external factors attracting leukocytes to lung vessels and creating conditions favorable for the second step, in which anti-HLA or anti-HNA antibodies or biologically active lipids, usually in transfused blood products, stress leukocytes and inflame lung epithelia. Platelets may be involved in the pathogenesis of TRALI because of

Efficacy of red blood cell transfusion in the critically ill.

This article has evaluated the published data regarding the efficacy of RBC transfusions in the critically ill. Taken together, these studies generally support conservative RBC transfusion strategies in critical care to reduce the risk of transfusion-related adverse effects. The TRICC trial has established the safety ofa restrictive transfusion strategy, suggesting that physicians could minimize exposure to allogeneic RBCs by lowering their transfusion threshold. Further research will add to the generalizability of this study and explore the possible mechanism to explain why RBC transfusions d

Approaches to preventing or reversing platelet alloimmunization using animal models.

Animal transfusion models were established to assess treatment programs for preventing or reversing platelet alloimmunization. Five control baboons given weekly transfusions of radiolabeled platelets from a single unrelated donor became immunized after an average of 2.4 +/- 2.1 transfusions. Similarly, 18 of 21 (86%) dogs given up to eight platelet transfusions from a single unrelated donor became immunized after an average of 2.3 +/- 1.7 transfusions. In six of seven baboons, prednisone or antithymocyte globulin alone or in combination effectively delayed platelet alloimmunization. In contras

Minor histocompatibility antigens on transfused leukoreduced units of red blood cells induce bone marrow transplant rejection in a mouse model.

When successful, human leukocyte antigen (HLA)-matched bone marrow transplantation with reduced-intensity conditioning is a cure for several nonmalignant hematologic disorders that require chronic transfusion, such as sickle cell disease and aplastic anemia. However, there are unusually high bone marrow transplant (BMT) rejection rates in these patients. Rejection correlates with the number of transfusions before bone marrow transplantation, and it has been hypothesized that preimmunization to antigens on transfused blood may prime BMT rejection. Using a novel mouse model of red blood cell (RB

TRALI risk reduction: donor and component management strategies.

Transfusion-related lung injury (TRALI) occurs in approximately 1 in 5,000 transfusions and may cause considerably more morbidity and mortality that is not recognized in clinical practice. Based on the current understanding of the etiology of TRALI, blood centers have implemented or are evaluating various donor and component management strategies in an effort to mitigate the risk of TRALI. Many cases of TRALI are likely caused by antibodies to leukocyte antigens (HLA or HNA) in blood components. Approximately 10 to 20% of female blood donors with a history of pregnancy and 1 to 5% of male bloo

A comparison of different filters for white cell reduction.

Removal of white blood cells (WBCs) from blood components before transfusion by filters with at least 3 log10 depletion may prevent or delay leukocyte-associated transfusion reactions such as HLA alloimmunization, non-hemolytic febrile reactions, transmitted infections (e.g., CMV, HTLV-1), and immunomodulation. The aim of this study was to compare the leukocyte removal efficiency (LRE) of six commercial bedside filters that are said to achieve 3 log10 (Bio R-01), Leucostop 4LT-1, Pall RC 50) and 4 log10 (Bio R-01 Plus, Pall RC 400, Pall RC XL-1) WBC depletion. Partial embolization of the spleen in patients with thalassemia.

PURPOSE: The role of partial splenic embolization in alleviating hyperslenism was evaluated in children with thalassemia. PATIENTS AND METHODS: Five children with thalassemia (three with alpha-thalassemia, two with beta-thalassemia) underwent embolization with polyvinyl alcohol particles. In two patients, a second splenic embolization was required. RESULTS: Of the four patients who were transfusion dependent before embolization, two no longer needed transfusion after embolization and two had decreased requirements. In fifth patient who was not transfusion dependent, the hemoglobin level incr

Transfusion reactions in thalassemia. A survey from the Cooleycare programme. The Cooleycare Cooperative Group.

A survey on transfusion reactions in thalassemia was carried out within the COOLEYCARE Programme, a cooperative enterprise aimed at improving quantity and quality of life in thalassemia through a program of quality assurance of treatment delivered to patients. Reactions were reported in 1,225 of 111,590 red cell transfusions (1.1%) given during 40 months (September 1985-December 1987) to 3,755 thalassemics in Italy and Greece. About 90% of red cell units were leukocyte-poor. Filtration was the most commonly used technique for leukocyte removal. Chills, fever, urticaria, headache and chest pain

Microchimerism in transfused trauma patients is associated with diminished donor-specific lymphocyte response.

BACKGROUND: Blood transfusion can result in long-term survival of donor leukocyte subpopulations, or microchimerism, in the peripheral blood of injured patients. Neither injury severity nor the number of transfusions is associated with its occurrence. We sought to determine whether changes in general or antigen-specific lymphocyte activation may be associated with the subsequent development of microchimerism. METHODS: We evaluated 63 transfused trauma patients, which we compared with 10 non-transfused trauma patients and 10 healthy control subjects. Of the 63 transfused patients, 31 were know

Filtering out the confusion about leukocyte-poor blood components.

During the past decade, intensive investigation into the adverse clinical potential of leukocytes in red cell and platelet products has been undertaken. Clinical data indicated several benefits associated with the delivery of leukocyte-depleted blood products. These benefits include a reduction in nonhemolytic febrile transfusion reactions, human leukocyte antigen alloimmunization, and viral transmission. In response to such findings, several procedures and devices have been designed to remove leukocytes from blood components.

Transfusion-related acute lung injury following coronary artery bypass graft surgery.

Blood transfusion is sometimes a necessary procedure during or following coronary artery bypass graft (CABG) surgery. However, transfusion-related acute lung injury (TRALI)/possible TRALI is a rare and fatal complication and characterized by acute hypoxemia and non-cardiogenic pulmonary edema that occurs within 6 hours following a transfusion. Anti-leukocyte antibodies or, possibly, other bioactive substances cause inflammation and capillary endothelial destruction in susceptible recipients' lungs. Prompt diagnosis and mechanical ventilatory support are important. A successful treatment of two

Possible insensitivity of the polybrene antibody screen to detect anti-Jka.

An acute hemolytic transfusion reaction (AHTR) occurred in a 28-yr-old gravida immediately after transfusion with leukocyte-reduced red cells. The patient gave no history of prior transfusion. Initial serologic testing by the polybrene method was negative for both antibody screening and cross-matching. Further testing by the indirect anti-globulin test (IAT) demonstrated the presence of anti-Jka antibodies. These observations suggest a limitation in polybrene testing for Jka antibodies associated with hemolytic transfusions. Caution is advised when the polybrene test is used as the sole determ

Leukocyte depletion of cellular blood components.

Clinical studies have indicated that the use of leukocyte-reduced cellular blood components produced in the laboratory may prevent febrile reactions and delay or prevent alloimmunization to HLA antigens and refractoriness to platelet transfusion. Additional investigations regarding the effects of the use of leukocyte-reduced blood components were reported during the past year. A recent study in patients with hematologic malignancy that employed the commonly used bedside leukocyte-reduction filters failed to confirm a decrease in the rate of alloimmunization, except in a subgroup of patients wi

Immunosuppressive effects of allogeneic blood transfusions: implications for the patient with a malignancy.

A considerable amount of information has accumulated over the past decade indicating that the transfusion of allogeneic blood products may be associated with adverse effects to the recipient. These include the development of transfusion reactions, TA-GVHD, HLA-alloimmunization, and immunomodulatory effects. The latter might be beneficial for recipients of kidney allografts, in reducing the relapse rate in patients with Crohn's disease, and in ameliorating the rate of recurrence of spontaneous abortion in affected patients; however, the immunosuppressive effects associated with perioperative AB

In vitro study on the effect of fibrinogen γ-chain peptide-coated ADP-encapsulated liposomes on postcardiopulmonary bypass coagulopathy using patient blood.

BACKGROUND: Fibrinogen γ-chain peptide-coated, adenosine 5'-diphosphate (ADP)-encapsulated liposomes (H12-ADP-liposomes) are potent hemostatic adjuvants that promote platelet thrombi formation at bleeding sites. Although we have reported the efficacy of these liposomes in a rabbit model of cardiopulmonary bypass coagulopathy, we are yet to address the possibility of their hypercoagulative potential, especially in human beings. OBJECTIVES: Considering its future clinical applications, we herein investigated the safety of using H12-ADP-liposomes in vitro using blood samples from patients who ha

Immunologic consequences of blood transfusion and their clinical manifestations.

Aspects of clinical immunology, in the context of transfusion medicine, became highlighted in the last decade as a consequence of the accelerating expansion of basic immunology, immunogenetics and molecular biology. In addition sophisticated new technologies, which were capable of producing pure and safe blood products, attracted more attention to research and monitor the consequences of transfusion. These technologies also had obvious effects on supportive hematological therapy. The transfusion of blood components follows the rules of organ transplantation: when there is a mismatch between th

Pulmonary consequences of transfusion: TRALI and TACO.

Transfusion-related acute lung injury and transfusion-associated circulatory overload are important, life-threatening complications of transfusion. Each adversely impact hospital length of stay and cost of healthcare. TRALI is clinically indistinguishable from the adult respiratory distress syndrome but it has a more favorable prognosis. Approximately 10% of TRALI patients die from this complication. The at-risk patient for TRALI has not been identified. The most commonly cited incidence is 1:5000 plasma-containing blood component transfusions. Although several pathways may lead to TRALI, pass

Filtration of platelet concentrates.

The transfusion of leukocyte-containing blood products causes a number of side reactions, which among others induces alterations of the immune system of the recipient leading to alloimmunization or immunosuppression. Antibodies against antigenic determinants of the major histocompatibility complex-proteins of class I are the major cause of alloimmunization. Though platelets are easy targets for HLA antibodies, they cannot induce the antibody production themselves. This requires leukocytes, carrying the MHC-proteins of the class II. Standard platelet concentrates for the clinical routine have m
