Oral hairy leukoplakia--10 years on.

Oral hairy leukoplakia (OHL) was first observed in 1981 and reported in 1984. Initially, this entity was restricted to HIV-seropositive male homosexual patients, but the risk group has been expanded since 1986 to all patients infected with HIV. It has been recognized that OHL is caused by Epstein-Barr virus (EBV). In 1988 we reported the first HIV-negative immunosuppressed patient with documented EBV-positive OHL. In patients with OHL, continuous shedding of EBV from saliva is necessary to maintain the lesion. EBV shedding leads to repeated infection of the intraoral epithelium. This may expla

Oral hairy leukoplakia: diagnosis and management.

Oral hairy leukoplakia (HL) is a remarkable lesion associated with Epstein-Barr virus (EBV) found in persons infected with the human immunodeficiency virus. The clinical and histologic features of HL are characteristic and distinctive. However, none of these features are entirely specific for HL, and we consider that the presence of EBV is required for diagnosis in questionable cases. EBV can be demonstrated by means of electron microscopy, immunocytochemistry, or molecular biologic techniques. Therapy directed toward the HL lesion is sometimes indicated; acyclovir is the current drug of prefe

Hairy leukoplakia: a new clinical entity.

Recent advances in therapy mean that prompt diagnosis of AIDS can prolong the life of those infected. Hairy leukoplakia is a new clinical entity that is one of the early signs of HIV seropositivity, and the GDP is well placed to recognize this lesion and refer patients on for treatment. In this second article in the twice-yearly Aids Commentary series the author describes how to diagnose this new condition, and the treatments that are available.

[Epidermolytic leukoplakia: an unusual precancerous condition].

We report on a rare type of leucoplakia of the lip and the prepuce showing the morphological features of epidermolytic hyperkeratosis. The development of squamous cell carcinoma in one of the patients strongly suggests that epidermolytic leucoplakia may represent a precancerous disorder of the transitional epidermis.

Oral leukoplakia transplanted to nude mice.

Thirty-eight biopsies of homogeneous oral leukoplakias were transplanted to subcutaneous sites of nude mice and protected from overlying cutaneous tissue by Millipore filters. The transplants were recovered after 21 d with a rate of successful recovery of 66%. The histologic epithelial features of the transplants were compared with those of the original leukoplakias. None of the leukoplakias from patients without tobacco habits lost the preexisting keratinization, whereas all 10 transplants demonstrating loss of keratinization belonged to patients with tobacco habits. This difference in behavi

Malignant Transformation of Oral Leukoplakia and Proliferative Verrucous Leukoplakia and Its Biomarker Predictors: A Systematic Umbrella Review.

BACKGROUND: Oral leukoplakia (OL) represents the most common oral potentially malignant disorder globally, with highly variable reported malignant transformation (MT) rates creating challenges for evidence-based clinical management. OBJECTIVE: To systematically synthesize evidence on MT prevalence in OL and evaluate potential predictive biomarkers through an umbrella review of systematic reviews and meta-analyses. METHODS: Following PRISMA guidelines, we searched PubMed, MEDLINE, Scopus, and Embase databases through July 2025. Twenty-seven systematic reviews encompassing more than 125,000 pa

Prevalence and risk factors associated with leukoplakia, hairy leukoplakia, erythematous candidiasis, and gingival hyperplasia in renal transplant recipients.

The aims of this study were to determine the prevalence of intraoral lesions in renal transplant recipients and to identify possible risk factors. The oral mucosa of 159 renal transplant recipients and 160 control patients was examined. The most common lesion in renal transplant recipients was cyclosporin-induced gingival hyperplasia (prevalence 22%) and patients with gingival hyperplasia were found to be taking significantly more cyclosporin-A than those without (p < 0.001). The prevalence of hairy leukoplakia and leukoplakia in renal transplant recipients was 11.3% and 10.7%, respectively, c

Dyskeratosis congenita associated with leukoplakia of the tongue.

Dyskeratosis congenita (DC) is an inherited disease characterized by the triad of skin pigmentation, nail dystrophy, and oral leukoplakia. Among other abnormalities, bone marrow failure and a predisposition to cancer are recognized as the major causes of premature mortality in patients with DC. This disease is associated with short telomeres and mutations in 10 genes associated with telomerase and telomere components. The case of a 35-year-old male patient diagnosed with DC, who presented with leukoplakia of the tongue and had a high degree of hypoplastic marrow, but no haematological abnormal

Association of periodontitis with the risk of oral leukoplakia.

BACKGROUND: Oral leukoplakia is an oral lesion suspected to be of premalignant character. Besides smoking and alcohol, the risk factors for the development of this oral lesion are still less identified. The purpose of this study was the search for a possible influence of periodontitis on the risk of leukoplakia. METHODS: We used data from 4233 subjects (2116 women and 2117 men) who were recruited for the population-based Study of Health in Pomerania (SHIP) and finished a standard medical and dental examination. One hundred two-three cases with oral leukoplakia were 1:2 age and sex-matched wit

Glycogenic acanthosis presenting as leukoplakia on the tongue.

This report describes a rare case of glycogenic acanthosis (GA) located on the tongue of a 72-year-old man. The patient presented with a white plaque that had been on the right side of the tongue for 5 years. Histologically, the lesion was characterised by thickened squamous epithelium with abundant intracellular glycogen deposits. No epithelial dysplasia was noted. This is the first report of GA involving the tongue. This benign condition should be added to the large number of lesions responsible for leukoplakia in this anatomical location.

Oral hairy leukoplakia in a child with AIDS.

A case of a 9-year-old boy with AIDS and severe hairy leukoplakia on the tongue is reported. Clinically it appears as a bilateral whitish-grey, nonremovable lesion on the lateral margins of the tongue with characteristic vertical corrugations. The lesion failed to respond to topical and systemic antifungal treatment. In contrast, it completely disappeared after treatment with 600 mg acyclovir per day for 1 month. During the hairy leukoplakia development the CD4-lymphocytes count was 95/microL.

Hairy leukoplakia with involvement of the buccal mucosa.

Oral hairy leukoplakia occurs mainly on the tongue of human immunodeficiency virus (HIV)-infected persons. An HIV-infected patient with hairy leukoplakia involving the tongue and buccal mucosa was studied by light and electron microscopic methods, in situ hybridization, and polymerase chain reaction. Our findings indicate that hairy leukoplakia may involve the buccal mucosa and should be considered in the differential diagnosis of white oral lesions in HIV-positive patients. Epstein-Barr virus particles were found in the epithelial cells of both buccal and tongue mucosa.

[Therapy of oral hairy leukoplakia with acyclovir].

Oral hairy leukoplakia was treated in six patients with (a) acyclovir (i.v. or p.o.), (b) 0.1% vitamin-A acid solution or (c) human beta-interferon-gel (10(5) I.E./g) in a total of 23 therapeutic courses. In 5/6 patients, acyclovir (7.5 mg/kg every 8 h i.v. or 5 x 400 mg p.o. over 5-10 days) led to partial (n = 1) or complete (n = 4) remission. After 1-6 months, however, the leukoplakia recurred in all cases. Vitamin-A acid solution (n = 3) led to remission in one and to improvement in the others. Human beta-interferon gel (n = 3) had no visible effect. The efficacy of acyclovir is further evi
