[Initial management of acute leukemia and its complications].

Acute leukemias are rare diseases requiring emergency specialized hematological care. However, other specialists may be involved: general practitioners, emergency physicians, internists and intensivists most often. The initial phase of the management of acute leukemia can be marked by the occurrence of complications with a significant impact on morbidity and mortality. The aim of this article is to guide the clinician when facing a suspected acute leukemia for the diagnosis and management of complications that may occur in the first hours. The article reviews the initial assessment, the diagno

A report of swine erysipelas in a litter of piglets.

Out of a litter of 7 two-week old Landrace piglets, 6 developed cutaneous haemorrhages especially on the limbs and ears. Two of these piglets died within 24 hours of the haemorrhages appearing whilst the other 4 recovered following penicillin therapy. The histopathological lesions were centred around the smaller vessels of the dermis and hypodermis. These included hyperaemia, leukostasis and intravascular fibrin coagulation or thrombosis. Bacterial emboli were present within the vessels of the skin, spleen, liver and kidney and loose in the areolar tissue of the dermis and hypodermis. Other le

Acute pulmonary complications in patients with hematologic malignancies.

Various acute pulmonary complications may occur in patients with hematologic malignancies because they are in an immunocompromised state due to systemic disease or to chemotherapy or hematopoietic stem cell transplantation. Pulmonary complications may arise from other treatment regimens, as well, or from direct pulmonary involvement in the malignant disease process. The differential diagnosis of pulmonary opacities in patients with hematologic malignancies is broad and includes both infectious and noninfectious causes. Pulmonary hemorrhage, edema, leukostasis, and pneumonia are well-known and

[Pathophysiologic and anesthesiologic characteristics of patients with leukemia].

Leukaemia and its associated therapy result in pathophysiological peculiarities relevant to anaesthesia. Leukaemic patients suffer from anaemia, coagulation disorders, and the consequences of immunosuppression. In addition, some patients show infiltrations of the oropharynx, potentially resulting in difficult intubation and/or pharyngeal haemorrhage. Mediastinal masses can induce complete airway obstruction during general anaesthesia. Patients with a white blood cell count (WBC) greater than 100,000/mm3 (hyperleukocytosis) can suffer from the leukostasis syndrome with acute respiratory failure

Acute nonlymphocytic leukemia in a dog.

Acute nonlymphocytic leukemia was diagnosed in a 1-year-old Rhodesian Ridgeback. Clinical signs of disease included weight loss, anorexia, lethargy, lymphadenopathy, splenomegaly, and hepatomegaly. Doxorubicin was administered IV on day 4 at a dosage of 30 mg/m2 of body surface, followed 2 days later by oral administration of cyclophosphamide at a dosage of 100 mg/m2. The cyclophosphamide was given for 4 consecutive days (days 8, 9, 10, and 11), but the WBC count did not respond. The dog was administered 500 ml of blood; but on day 12, it died. Necropsy was not performed, but the presumptive c

Intraoperative Collapse During First-Trimester Abortion: Hyperleukocytosis Crisis, Thrombotic Pulmonary Embolism, or Amniotic Fluid Embolism?

BACKGROUND: Hyperleukocytosis in patients with hematologic malignancies poses significant perioperative risks, including leukostasis, coagulopathy, tumor lysis syndrome and hyperinflammatory syndrome. Additionally, pregnancy and hyperleukocytosis predispose patients to a hypercoagulable state, increasing the risk of thrombosis. On the other hand, amniotic fluid embolism (AFE) is a rare but serious complication of pregnancy. Although distinct in etiology, thrombotic pulmonary embolism (PE), AFE, and hyperleukocytosis crisis can present with a strikingly similar clinical picture of pulmonary hyp

Chest radiographic and CT findings in hyperleukocytic acute myeloid leukemia: A retrospective cohort study of 73 patients.

Hyperleukocytic acute myeloid leukemia (AML) is associated with pulmonary complications and high early mortality rate, but given its rarity, data on chest radiographic presentation are scarce. We retrospectively reviewed the charts of 73 AML patients admitted with white blood cell count >100 × 10/L between 2003 and 2014 in order to describe the chest radiographic and computed tomography (CT) findings and to correlate them with AML subtype and respiratory symptoms.Forty-two of the 73 patients (58%) overall and 36 of the 54 patients (67%) with clinical signs of pulmonary leukostasis had abnormal

Ocular application of the kinin B1 receptor antagonist LF22-0542 inhibits retinal inflammation and oxidative stress in streptozotocin-diabetic rats.

PURPOSE: Kinin B(1) receptor (B(1)R) is upregulated in retina of Streptozotocin (STZ)-diabetic rats and contributes to vasodilation of retinal microvessels and breakdown of the blood-retinal barrier. Systemic treatment with B(1)R antagonists reversed the increased retinal plasma extravasation in STZ rats. The present study aims at determining whether ocular application of a water soluble B(1)R antagonist could reverse diabetes-induced retinal inflammation and oxidative stress. METHODS: Wistar rats were made diabetic with STZ (65 mg/kg, i.p.) and 7 days later, they received one eye drop applic

Leukapheresis in management hyperleucocytosis induced complications in two pediatric patients with chronic myelogenous leukemia.

Complications caused by elevated white blood cell count in pediatric patients with CML could be a presenting feature of the disease. Here, we present two adolescents, aged 16 and 17years, who were admitted for investigation of extremely elevated leukocytes and complications of leucostasis. Initial manifestations were priapism and blurred vision, respectively. Diagnosis of chronic phase of chronic myeloid leukemia is established, and conventional measures for leucoreduction began. However, since there were no improvements, a leukapheresis procedure was initiated. After undergoing 3 daily proced

Cross-complementation of Clostridium perfringens PLC and Clostridium septicum alpha-toxin mutants reveals PLC is sufficient to mediate gas gangrene.

Clostridium perfringens and Clostridium septicum are the most common causes of clostridial myonecrosis or gas gangrene. Although they mediate a similar disease pathology, they elaborate functionally very different alpha-toxins. We used a reciprocal complementation approach to assess the contribution of the primary toxin of each species to disease and found that C. perfringens alpha-toxin (PLC) was able to mediate the gross pathology of myonecrosis even in a C. septicum background, although it could not induce vascular leukostasis. Conversely, while C. septicum alpha-toxin restored some virulen

Cannabinoid Receptor 2 Agonism Demonstrates Therapeutic Potential in Experimental Models of Relevance to Diabetic Retinopathy.

An effective treatment strategy for early-stage diabetic retinopathy (DR) could delay or prevent disease progression and limit or eliminate irreparable retinal damage and vision loss associated with progression to later stages of DR. Cannabinoid receptor 2 (CB2) activation strategies may provide a promising therapeutic approach to address the chronic retinal inflammation that encourages DR progression. CB2 activation therapeutically addresses leukocyte binding in human retinal microvascular endothelial cells and retinal leukostasis in diabetic mice, presumably by inhibiting nuclear factor κB-d

Granulocyte aggregation in adult respiratory distress syndrome (ARDS)--serial histologic and physiologic observations.

Although a number of studies have suggested that granulocyte sequestration is an important pathophysiologic event in ARDS, histologic evidence of aggregated granulocytes in the pulmonary microvasculature is limited, and serial histologic data have not been reported with physiologic measurements. We report a patient with ARDS who demonstrated microvascular granulocyte aggregation and lung edema in sections of a lung biopsy obtained seven days after the onset of symptoms. Pulmonary vascular resistance and pulmonary capillary wedge pressure were normal immediately before the biopsy. A second biop

MyD88-dependent pathways in leukocytes affect the retina in diabetes.

BACKGROUND: Previous studies by us and other have provided evidence that leukocytes play a critical role in the development of diabetic retinopathy, suggesting a possible role of the innate immune system in development of the retinopathy. Since MyD88 is a convergence point for signaling pathways of the innate immune system (including Toll-Like Receptors (TLRs) and interleukin-1ß (IL-1ß)), the purpose of this study was to assess the role of MyD88 and its dependent pathways on abnormalities that develop in retina and white blood cells related to diabetic retinopathy. METHODS: C57BL/6J mice were

Intracerebral hemorrhage as the first symptomatic manifestation of chronic myeloid leukemia (chronic phase): A case report and literature review.

BACKGROUND: Chronic myeloid leukemia (CML) is mostly asymptomatic at diagnosis. Intracerebral hemorrhage (ICH), as the first presentation of CML in its chronic phase (CP) has only once been reported in the literature. In addition, CML (CP) patients developing ICH are equally rare, with only eight cases reported. ICH is more commonly associated with CML progressing to its end stage (accelerated phase [AP] and blast crisis [BC]). The pathophysiology of ICH in CML-CP is postulated to be due to leukostasis, unlike in the CML-AP/BC, where thrombocytopenia and coagulopathy are the underlying mechani

Effects of methylprednisolone and dexamethasone on polymorphonuclear leukocyte adherence in vivo.

It has been proposed that corticosteroids may be effective in the treatment of shock and the adult respiratory distress syndrome (ARDS) by inhibiting complement-induced granulocyte aggregation and by disaggregating granulocyte aggregates in vitro. In the present investigation we have examined the effects of sera from patients who had received comparable doses of methylprednisolone (MP) and dexamethasone (DEX) on the adherence of polymorphonuclear leukocytes (PMNs). Sera obtained from patients that had received MP markedly reduced PMN adherence with maximum effect noted by 8 h. In contrast, ser

Out with the bad and in with the good; red cell exchange, white cell reduction, and platelet reduction.

Automated techniques for red cell [red blood cell (RBC)] exchange or depletion of malignant cells from the peripheral blood have allowed patients with life-threatening conditions to survive long enough to receive definitive treatment. Examples of such conditions include acute chest syndrome in sickle cell disease (SCD) or acute respiratory insufficiency due to leukostasis in acute leukemia. Conversely, other patients with SCD undergo RBC exchanges on a chronic basis to maintain a reasonable quality of life and prevent another stroke. In this review, we will discuss the techniques as well as in

Continuous intravenous infusion of elastase in normal and agranulocytic minipigs--effects on the lungs and the blood coagulation system.

In order to investigate the acute effects on lung morphology, lung function, hemodynamic and blood coagulation system, elastase (330 units (U) kg-1 h-1) was continuously infused into 16 anesthetized and mechanically ventilated minipigs. Elastase infusion induced a disturbance of blood coagulation leading to hypocoagulability, a pulmonary leukostasis, interstitial edema, a progressive respiratory failure with prompt increase in pulmonary vascular resistance, decrease in systemic vascular resistance, increased venous admixture, and increased dead space ventilation. Agranulocytosis prevented inte

Leukapheresis and Hyperleukocytosis, Past and Future.

Hyperleukocytosis is a hematologic crisis caused by excessive proliferation of leukemic cells and has a relatively high early mortality due to a series of severe complications. Therefore, prompt and effective intervention is required. Leukapheresis performed using apheresis equipment to separate leukocytes from peripheral blood, at the same time returns autologous plasma, platelets and erythrocytes to the patient, is applied clinically for the treatment of hyperleukocytosis. Leukapheresis not only removes excessive leukocytes rapidly and corrects metabolic abnormalities but also alleviates the

[Fatal pulmonary leukoagglutination after administration of fresh frozen plasma].

A case is reported of a 73 year old female patient who died during surgery for enlarged nephrectomy as a result of a massive non-cardiogenic pulmonary oedema. She had been given 2 red cell concentrates (450 g) and 3 fresh frozen plasma units (900 g). A postmortem examination did not reveal any pulmonary embolus, acute myocardial infarct, haemorrhage or cardiac tamponade. Further tests on the sera of the 5 plasma donors revealed a neutrophil-specific anti-NA2 antibody. Alveolar leukostasis was confirmed on the postmortem lung slices. This type of transfusion accident occurs for about 1 in 5,000

Review of Hematology-Oncology Emergencies for Internal Medicine Residents.

The prevalence of cancer continues to grow globally every year. With therapeutic advances over the recent decades, the prevalence of individuals living with cancer continues to increase. Internal medicine residents can see patients admitted to the hospital for cancer-related emergencies. Early identification and appropriate management of these emergencies have been shown to improve mortality and morbidity. In this article, we aim to review the recent updates in the management of commonly encountered oncologic emergencies in the practice of internal medicine residents. This review will cover sp
