Morphological changes of the ligamentum flavum as a cause of nerve root compression.

The ligamentum flavum is considered to be one of the important causes of radiculopathy in lumbar degenerative disease. Although there have been several reports anatomically examining the positional relationship between the ligamentum flavum and nerve root, there are few reports on ventral observation. The purpose of this study is to clarify the shape of the ligamentum flavum seen ventrally, and to obtain anatomic findings related to nerve root compression. The subjects were 18 adult embalmed cadavers, with an average age of 78 years at the time of death. The ventral shapes of the ligamentum fl

Ossification of the ligamentum flavum causing thoracic myelopathy: a case report.

Ossification of the ligamentum flavum is a well reported clinicopathologic entity causing narrowing of the spinal canal and subsequent spinal cord compression. The patient described in this case report complained of 9 mo of middle and lower back pain, difficulty with balance, progressive gait disturbance, and recent onset of bladder retention. Magnetic resonance imaging and computed tomographic scan revealed a bone density mass at the T2-3 level causing 25% cord compression and edema. A decompressive laminectomy was performed at T-2. The etiology of the compression was found to be attributable

Tongluo Huoxue formula reduces ligamentum flavum fibrosis in lumbar spinal stenosis.

OBJECTIVE: Lumbar spinal stenosis (LSS) caused by ligamentum flavum hypertrophy (LFH) is a common degenerative spinal condition leading to spinal cord and nerve root compression. This study aims to investigate the therapeutic effects and underlying mechanisms of Tongluo Huoxue Formula (TLHXF) in treating LSS by modulating the TGF-β/Smad signaling pathway. METHODS: The active compounds of TLHXF were identified using HPLC-MS/MS (high-performance liquid chromatography-mass spectrometry). Network pharmacology was used to predict potential targets, followed by molecular docking to evaluate interac

CCN5 Reduces Ligamentum Flavum Hypertrophy by Modulating the TGF-β Pathway.

Ligamentum flavum hypertrophy (LFH) is the most important component of lumbar spinal canal stenosis. Although the pathophysiology of LFH has been extensively studied, no method has been proposed to prevent or treat it. Since the transforming growth factor-β (TGF-β) pathway is known to be critical in LFH pathology, we investigated whether LFH could be prevented by blocking or modulating the TGF-β mechanism. Human LF cells were used for the experiments. First, we created TGF-β receptor 1 (TGFBR1) knock out (KO) cells with CRISPR (clustered regularly interspaced short palindromic repeats)/Cas9 bi

Proteomic analysis of ligamentum flavum from patients with lumbar spinal stenosis.

Lumbar spinal stenosis (LSS) is a syndromic degenerative spinal disease and is characterized by spinal canal narrowing with subsequent neural compression causing gait disturbances. Although LSS is a major age-related musculoskeletal disease that causes large decreases in the daily living activities of the elderly, its molecular pathology has not been investigated using proteomics. Thus, we used several proteomic technologies to analyze the ligamentum flavum (LF) of individuals with LSS. Using comprehensive proteomics with strong cation exchange fractionation, we detected 1288 proteins in these

Inflammatory cytokines induce fibrosis and ossification of human ligamentum flavum cells.

STUDY DESIGN: In vitro experiment using degenerated human ligamentum flavum (LF) and various inflammatory cytokines. OBJECTIVES: To examine the effect of inflammatory cytokines on LF cells and to identify their roles in the pathogenesis of LF hypertrophy and ossification. SUMMARY OF BACKGROUND DATA: Spinal stenosis is caused, in part, by hypertrophy and ossification of the LF, which are induced by the degenerative processes (ie, increased collagen synthesis and chondroid metaplasia) of ligament fibroblasts. Degenerated intervertebral disk spontaneously produces inflammatory cytokines, which

Unilateral ossified ligamentum flavum in the high cervical spine causing myelopathy.

High cervical ossified ligamentum flavum (OLF) is rare and may cause progressive quadriparesis and respiratory failure. Our two patients had unilateral OLF between C1 and C4 levels. MR showed a unilateral, triangular bony excrescence with low signal and a central, intermediate or high signal on all pulse sequences due to bone marrow within. There was Type I thecal compression (partial deficit of contrast media ring). The first patient had a linear and nodular OLF with calcification within tectorial membrane, C2-3 fusion and unilateral C2-facetal hypertrophy; and the second patient, a lateral,

Removal of ossified ligamentum flavum via a minimally invasive surgical approach.

Ossified ligamentum flavum (OLF) in the thoracic spine is a rare cause of myelopathy, often presenting with progressive symptomatology over an extended period of time. Surgical decompression via wide laminectomy has been the mainstay of treatment for this symptomatic disease phenomenon, but complications such as kyphotic deformity have developed due to extensive bone removal and release of the posterior tension band. The authors present a case of OLF excised via a minimally invasive microsurgical approach in which an expandable tubular retractor system was used. This approach enables complete

Two-level ligamentum flavum hematoma in the lumbar spine. Case report.

Ligamentum flavum hematoma is a rare cause of spinal root or cord compression that usually occurs at a single level. No case of multiple-level ligamentum flavum hematoma has previously been reported. We report an extremely rare case of double, contiguous ligamentum flavum hematomas in the lumbar spine. A 71-year-old man with hypertension and degenerative lumbar scoliosis presented with pain and muscle weakness in the left lower extremity after physical exertion. Magnetic resonance imaging of the lumbar spine showed severe spinal stenosis caused by two-level ligamentum flavum hematoma (L3-L4 an

Case reports: a rare cause of radicular complaints: ligamentum flavum hematoma.

Hematomas in the lumbar ligamentum flavum are rare and may generate signs and symptoms of lumbar nerve root compression or neurogenic claudication similar to the far more common degenerative diseases. The pathogenesis of these hematomas is unclear. Ruptured irregular vessels of the degenerated and hypertrophic ligamentum flavum were assumed. We diagnosed three patients with a flavum hematoma intraoperatively leading to radicular complaints and spinal claudication. One occurred as a complication of local infiltration therapy, and the other two patients reported minor trauma as the releasing fac

Ligamentum flavum hypertrophy animal models: methodological trade-offs and future directions.

Ligamentum flavum hypertrophy (LFH) represents a key pathological factor in lumbar spinal stenosis (LSS), which is characterized by abnormal collagen deposition, reduced elastin fibers, and other degenerative changes. LFH compresses the spinal cord and nerve roots, causing symptoms like low back pain, numbness, and intermittent claudication, and may lead to central spinal stenosis, with significant consequences for quality of life. The pathophysiology of LFH involves extracellular matrix remodeling, inflammatory mediator release, and biomechanical stress, which contribute to spinal canal narro

Single-level ossified ligamentum flavum causing a holocord syrinx: illustrative case.

BACKGROUND: Syringomyelia is a neurological disorder that is caused by abnormal cerebrospinal fluid flow or circulation. It is an incidental finding in most cases, predominantly presenting with sensory symptoms of insensitivity to pain and temperature. Spinal ossified ligamentum flavum (OLF) leading to syringomyelia is one of the rare causes. The authors report an unusual case of syringomyelia due to a thoracic OLF. OBSERVATIONS: A 54-year-old female presented with backache, difficulty walking, spasticity in the bilateral lower limbs, tingling sensation in the bilateral lower limbs, and parap
