Temporal lobe epilepsy after experimental prolonged febrile seizures: prospective analysis.

Experimental prolonged febrile seizures (FS) lead to structural and molecular changes that promote hippocampal hyperexcitability and reduce seizure threshold to further convulsants. However, whether these seizures provoke later-onset epilepsy, as has been suspected in humans, has remained unclear. Previously, intermittent EEGs with behavioural observations for motor seizures failed to demonstrate spontaneous seizures in adult rats subjected to experimental prolonged FS during infancy. Because limbic seizures may be behaviourally subtle, here we determined the presence of spontaneous limbic sei

Unilateral limbic epileptic status activity: stereo EEG, behavioral, and cognitive data.

We report data from four patients with unilateral epileptiform status activity within different structures of the temporal lobe, recorded during stereoelectroencephalographic presurgical evaluation. The ictal clinical symptoms accompanying neocortical and mesiobasal limbic discharges (two patients with complex partial status epilepticus) consisted of various psychosensory and vegetative signs, which can be understood on the basis of the spatiotemporal analysis of the discharges. The other two patients with circumscribed long-lasting mesiobasal limbic epileptiform activity represent unilateral

SPECT follow-up study of cerebral blood flow changes during Donepezil therapy in patients with Alzheimer's disease.

BACKGROUND AND PURPOSE: Treatment with acetylcholinesterase inhibitors (AchEIs) is beneficial for patients with Alzheimer's disease (AD). But the clinical response varies. Functional neuroimaging techniques might allow objective evaluation of the topographic correlates of the effect of therapy on brain functioning. We attempted to evaluate the possible correlation between regional cerebral blood flow (rCBF) differences in patients with AD and response to donepezil hydrochloride (Donepezil) therapy. METHODS: The subjects of this study were 51 consecutive patients with AD receiving Donepezil wh

Autoimmune and Paraneoplastic Disorders.

Autoimmune encephalitis (AE) is a disorder in which the immune system targets the central nervous system. AE can be paraneoplastic or postinfectious, though many cases lack a clear trigger. AE typically presents acutely or subacutely with altered mental status and psychiatric symptoms, with clinical features varying by the targeted autoantigen. MR imaging is the imaging modality of choice and may show T2 and fluid attenuation inversion recovery (FLAIR) hyperintensities in the medial temporal lobes, characteristic of limbic encephalitis, though imaging can be normal or may extend beyond the lim

Clinical, Radiological, and Immunohistological Distinctions Between Limbic-Predominant and Typical Alzheimer's Disease: A Systematic Review.

BACKGROUND: Alzheimer's disease (AD) is the most common cause of dementia worldwide and one of the leading causes of morbidity and mortality among elderly people. It is characterized by generalized brain atrophy, especially affecting the hippocampus and medial temporal lobe. In this context, new subtypes of AD have been documented, including a limbic-predominant subtype (LP), and the current literature is insufficient to clarify the similarities and differences between these subtypes and the typical presentation. Recently, new studies have proposed a clinical criterion for LP amnestic syndrome

Neuronal signatures of anger and fear in patients with psychosis.

The present study investigated the functional neuroanatomy in response to sentence stimuli related to anger-provoking situations and fear of negative evaluation in patients with psychosis. The tasks consisted of four active conditions, Self-Anger (SA), Self-Fear, Other-Anger (OA), and Other-Fear (OF), and two neutral conditions, Neutral-Anger (NA) and Neutral-Fear (NF). Several relevant clinical measures were obtained. Under all contrasts, significantly higher activation in the left inferior parietal gyrus or superior parietal gyrus and the left middle occipital gyrus or superior occipital gyr

Mapping of neurofibrillary degeneration in Alzheimer's disease: evaluation of heterogeneity using the quantification of abnormal tau proteins.

A biochemical mapping of neurofibrillary degeneration was performed in Brodmann areas of the brains of five patients with senile dementia of the Alzheimer type (AD). To quantify the degenerating process, we used an immunoblot method with antibodies directed against the abnormally phosphorylated tau proteins named Tau 55, 64 and 69, known to be early and reliable markers of the degenerating process in AD. The detection intensities were assessed using a numerical rating scale for each area and scores were grouped by lobe. In all cases, the detection of Tau 55, 64 and 69 was positive in all areas

Neurobiological Effects of Binge Drinking Help in Its Detection and Differential Diagnosis from Alcohol Dependence.

The prevalence of binge drinking in the general population is 3-4 times higher than that of alcohol dependence. Neuroimaging studies show that binge drinking in adolescence impairs brain development and white matter integrity. Regions with reduced functional activity include the limbic system, ventral diencephalon, frontal lobe, and middle and inferior temporal lobes, whereas the right superior frontal and parietal lobes are typically hyperactivated. The observed activation of the frontoparietal areas might reflect the alternative memory system operating, whereas the reduced occipito-hippocamp

Antemortem basal forebrain atrophy in pure limbic TAR DNA-binding protein 43 pathology compared with pure Alzheimer pathology.

BACKGROUND AND PURPOSE: Currently, the extent of cholinergic basal forebrain atrophy in relatively pure limbic TAR DNA-binding protein 43 (TDP-43) pathology compared with relatively pure Alzheimer disease (AD) is unclear. METHODS: We compared antemortem magnetic resonance imaging (MRI)-based atrophy of the basal forebrain and medial and lateral temporal lobe volumes between 10 autopsy cases with limbic TDP-43 pathology and 33 cases with AD pathology on postmortem neuropathologic examination from the Alzheimer's Disease Neuroimaging Initiative cohort. For reference, we studied MRI volumes from

Increased suicide attempts in young depressed patients with abnormal temporal-parietal-limbic gray matter volume.

BACKGROUND: Suicide is a major cause of death throughout the world. Approximately 60% of all suicides have a history of depression. Previous studies of structural brain imaging have shown that suicide is often associated with abnormal fronto-limbic networks. However, the mechanism underlying suicide in depression remains poorly understood. METHOD: Twenty sex- and age-matched suicidal unipolar patients were compared with 18 non-suicidal unipolar patients and 28 healthy controls. High-resolution T1-weighted 3T magnetic resonance imaging (MRI) scans were acquired. Hamilton Depressive Rating Scal

Effective connectivity of the insula as measured by cortico-cortical evoked potentials.

Effective connectivity of the human insula, mainly assessed at rest using cortico-cortical evoked potentials (CCEPs), is not yet fully characterized at high-resolution. Here, we significantly extend prior CCEP studies of the insula by leveraging an extensive multicenter CCEP database and fine-grained anatomical atlases of the insula. We analyzed CCEP datasets from 897 patients with refractory focal epilepsy (459 females, age: 26±14 years) explored by stereo electroencephalography and with at least one electrode contact in the insula. Efferent and afferent effective connectivity measures of nin

Anemia-associated smaller brain volume and sex differences: a cross-sectional study of magnetic resonance imaging in brain health checkups.

INTRODUCTION: Anemia is a risk factor for dementia development. However, few studies have examined the relationship between brain volume and anemia. This study aimed to analyze the association between anemia and brain volume using magnetic resonance imaging data from brain health checkups. METHOD: Participants underwent brain health checkups between January 2015 and March 2022. Blood samples were collected to measure hemoglobin concentrations and mean corpuscular volumes. The modified Mini-Mental State Examination (MMSE) was used to evaluate cognitive function. Magnetic resonance images were

Limbic Interference During Social Action Planning in Schizophrenia.

Schizophrenia is characterized by social interaction deficits contributing to poor functional outcome. Hand gesture use is particularly impaired, linked to frontal lobe dysfunction and frontal grey matter deficits. The functional neural correlates of impaired gesturing are currently unclear. We therefore investigated aberrant brain activity during impaired gesturing in schizophrenia. We included 22 patients with schizophrenia and 25 healthy control participants matched for age, gender, and education level. We obtained functional magnetic resonance imaging data using an event-related paradigm t

A Case of Anti-LGI1 Limbic Encephalitis Presenting With Non-convulsive Status Epilepticus and Requiring a Long Period of Hospitalization.

An 81-year-old man was brought to our hospital with tonic-clonic seizures and a gradual progression of cognitive dysfunction. Four months prior, he experienced transient episodes of stiffness and unresponsiveness. One month later, the patient began to exhibit disorientation to time and place. Upon admission, fluid-attenuated inversion recovery (FLAIR) brain magnetic resonance imaging (MRI) revealed hyperintense lesions in the right limbic cortex. Electroencephalography (EEG) revealed lateralized periodic discharges in the right temporal lobe. Serum anti-LGI1 antibody was detected. Based on the

Altered Brain Glucose Metabolism Assessed by 18F-FDG PET Imaging Is Associated with the Cognitive Impairment of CADASIL.

Recurrent stroke and cognitive impairment are the primary features of patients with cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL). The cognitive deficits in these patients are known to be correlated with structural brain changes, such as white matter lesions and lacunae, and resting-state functional connectivity in brain networks. However, the associations between changes in brain glucose metabolism based on 18F-2-fluoro-2-deoxy-D-glucose (18F-FDG) positron emission tomography (PET) imaging and cognitive scores in CADASIL patients remain u

Early childhood abuse and limbic system ratings in adult psychiatric outpatients.

The authors investigated the hypothesis that early abuse might affect the development of the limbic system. During initial psychiatric evaluation, 253 outpatients completed a self-report scale, the Limbic System Checklist-33 (LSCL-33), designed to measure somatic, sensory, behavioral, and memory symptoms suggestive of temporal lobe epilepsy, along with a questionnaire about physical or sexual abuse. Physical abuse was associated with a 38% increase in LSCL-33 scores (P < 0.01), sexual abuse with a 49% increase (P < 0.02), and combined abuse with a 113% increase (P < 0.0001). Physical or sexual

Neuropsychological and Structural Neuroimaging Outcomes in LGI1-Limbic Encephalitis: A Case Study.

OBJECTIVE: Anti-leucine-rich glioma-inactivated 1 limbic encephalitis (LGI1-LE) is a rare autoimmune condition that affects the structural integrity and functioning of the brain's limbic system. Little is known about its impact on long-term neuropsychological functioning and the structural integrity of the medial temporal lobe. Here we examined the long-term neuropsychological and neuroanatomical outcomes of a 68-year-old male who acquired LGI1-LE. METHODS: Our case patient underwent standardized neuropsychological testing at two time points. Volumetric analyses of T1-weighted images were und

Identification of adenylate kinase 5 antibodies during routine diagnostics in a tissue-based assay: Three new cases and a review of the literature.

Antibodies against adenylate kinase 5 (AK5) have been described in patients with non-paraneoplastic limbic encephalitis, mainly in men around 70 years of age. Routine testing with specific cell-based assays is not yet available. Three patients with episodic anterograde memory problems and depression had extensive limbic lesions and developed severe atrophy, mainly of the medial temporal lobes. The antibodies were identified in serum and CSF based on the typical staining pattern of AK5 antibodies on a tissue-based assay (here, unfixed mouse brain). Subsequently, they were confirmed by a researc

[Recognition of temporal lobe epilepsy in adults].

Temporal lobe epilepsy has a subtle and diverse symptomatology, and therefore temporal lobe seizures can initially be misdiagnosed. Here we discuss 3 patients with this type of epilepsy, but with completely different presentations. The first, a 67-year-old woman suffered from episodes of confusion, and later she developed tonic-clonic seizures. The second patient, a 58-year-old man, had auras followed by impaired consciousness and oral automatisms and also developed a type of seizure resembling panic attacks. The third patient, a 65-year-old man, presented with isolated auras. A few years late
