Usefulness of artificial jump graft to portal vein thrombosis in deceased donor liver transplantation.

Severe portal vein thrombosis (PVT) is often considered a relative contraindication for living donor liver transplantation due to high associated risks and morbidity. Meanwhile, improvement in operative techniques, resulting in higher success rates has removed PVT from the list of contraindications in deceased donor liver transplantation (DDLT). In this report, we describe a surgical technique for DDLT using polytetrafluoroethylene graft from the inferior mesenteric vein for portal inflow in patient with portomesenteric thrombosis.

Artificial Liver Support Effectively Removes Immunosuppressive Bile Acids From Circulation in Patients With Severe Liver Failure: A Proof of Concept Study.

OBJECTIVE: Bile acids that induce immunosuppression through activation of the Takeda G protein-coupled receptor 5 (TGR5) have recently been identified in the circulation of patients with liver failure and sepsis. These bile acids are associated with increased susceptibility to infection and a high short-term mortality. Furthermore, TGR5 activation by bile acids has been implicated in the development of pruritus. However, therapies targeting TGR5-activating bile acids in patients have not yet been reported. This study investigates the effects of two artificial liver support systems, the Molecul

Artificial intelligence-aided steatosis assessment in donor livers according to the Banff consensus recommendations.

OBJECTIVES: Severe macrovesicular steatosis in donor livers is associated with primary graft dysfunction. The Banff Working Group on Liver Allograft Pathology has proposed recommendations for steatosis assessment of donor liver biopsy specimens with a consensus for defining "large droplet fat" (LDF) and a 3-step algorithmic approach. METHODS: We retrieved slides and initial pathology reports from potential liver donor biopsy specimens from 2010 to 2021. Following the Banff approach, we reevaluated LDF steatosis and employed a computer-assisted manual quantification protocol and artificial int

Vascular liver segmentation: a narrative review on methods and new insights brought by artificial intelligence.

Liver vessel segmentation from routinely performed medical imaging is a useful tool for diagnosis, treatment planning and delivery, and prognosis evaluation for many diseases, particularly liver cancer. A precise representation of liver anatomy is crucial to define the extent of the disease and, when suitable, the consequent resective or ablative procedure, in order to guarantee a radical treatment without sacrificing an excessive volume of healthy liver. Once mainly performed manually, with notable cost in terms of time and human energies, vessel segmentation is currently realized through the

Artificial ascites technique for percutaneous radiofrequency ablation of liver cancer adjacent to the gastrointestinal tract.

BACKGROUND: Percutaneous radiofrequency ablation (RFA) of liver tumours adjacent to the gastrointestinal tract is controversial. This study assessed the value of an intraperitoneal water infusion (artificial ascites) technique for percutaneous RFA of such tumours. METHODS: Before ablation in 52 patients (55 treatments, 58 tumours), between 250 and 3000 (mean 681) ml 5 per cent glucose solution was infused into the abdominal cavity using a 14-G needle, with the aim of preventing thermal injury by separating the liver from the gastrointestinal tract. RESULTS: There were no adverse events assoc

Favorable effect of new artificial liver support on survival of patients with fulminant hepatic failure.

The two most serious symptoms of fulminant hepatic failure are bleeding and hepatic coma. To overcome these problems, we developed an artificial liver support system comprising a combination of plasma exchange and hemodiafiltration using a high performance membrane. We treated 67 patients with fulminant hepatic failure. Of these, 65 patients (97.0%) regained normal consciousness, and 55 patients (80.9%) were kept alert as long as we continued to apply this system. All 7 patients (100%) with fulminant hepatitis caused by hepatitis A virus infection and 9 of 12 patients (75%) with fulminant hepa

Subacute liver failure caused by immunotherapy in a patient with gastric cancer was improved after multimodal treatment with artificial liver combined with liver-protective drugs: a case report.

The emergence of the anti-tumor immunotherapy era has led to the widespread adoption of immune checkpoint inhibitors (ICIs) in clinical practice. While ICIs exhibit substantial anti-tumor efficacy across a broad range of malignancies, they may also induce a diverse spectrum of immune-related adverse events (irAEs), involving multiple organ systems and, in severe instances, resulting in life-threatening complications. This report describes a rare case of severe immune-mediated hepatotoxicity (IMH) in a 76-year-old male patient with postoperative gastric cancer who was undergoing adjuvant therap

CT-based artificial intelligence system complementing deep learning model and radiologist for liver fibrosis staging.

Noninvasive methods for liver fibrosis staging are urgently needed due to its significance in predicting significant morbidity and mortality. In this study, we developed an automated DL-based segmentation and classification model (Model-C). Test-time adaptation was used to address data distribution shifts. We then established a deep learning-radiologist complementarity decision system (DRCDS) via a decision model determining whether to adopt Model-C's diagnosis or defer to radiologists. Model-C (AUCs of 0.89-0.92) outperformed models based on liver (AUCs: 0.84-0.90) or spleen (AUCs: 0.69-0.70)

Predictive Model of Oxaliplatin-induced Liver Injury Based on Artificial Neural Network and Logistic Regression.

BACKGROUND AND AIMS: Identifying potential high-risk groups of oxaliplatin-induced liver injury (OILI) is valuable, but tools are lacking. So artificial neural network (ANN) and logistic regression (LR) models will be developed to predict the risk of OILI. METHODS: The medical information of patients treated with oxaliplatin between May and November 2016 at 10 hospitals was collected prospectively. We used the updated Roussel Uclaf causality assessment method (RUCAM) to identify cases of OILI and summarized the patient and medication characteristics. Furthermore, the ANN and LR models for pre
