Analysis of the Bacterial Diversity in Liver Abscess: Differences Between Pyogenic and Amebic Abscesses.

Several recent studies have demonstrated that virulence in Entamoeba histolytica is triggered in the presence of both pathogenic and nonpathogenic bacteria species using in vitro and in vivo experimental animal models. In this study, we examined samples aspirated from abscess material obtained from patients who were clinically diagnosed with amebic liver abscess (ALA) or pyogenic liver abscess (PLA). To determine the diversity of bacterial species in the abscesses, we performed partial 16S rRNA gene sequencing. In addition, the E. histolytica and Entamoeba dispar species were genotyped using t

Temporal Trends in Antimicrobial Resistance and Risk Factors in Patients with Pyogenic Liver Abscess: A Four-Year Cross-Sectional Study : Resistance Trends in Pyogenic Liver Abscess.

BACKGROUND: Pyogenic liver abscess (PLA) is a serious liver infection, particularly common in patients with immune deficiencies or chronic liver diseases. Given the rising antimicrobial resistance in PLA-associated pathogens, this study aimed to investigate the temporal trends in antimicrobial resistance and associated risk factors in PLA patients over a four-year period. MATERIALS AND METHODS: This cross-sectional study was conducted at Tianjin First Center Hospital, China, by reviewing the medical records of PLA patients from January 2019 to December 2023. Patients were categorized into two

Hepatic Abscess in a Returning Traveler with Crohn's Disease: Differentiating Amebic from Pyogenic Liver Abscess.

Liver abscess is a rare but serious complication of Crohn's disease. Patients with Crohn's disease are at risk for pyogenic liver abscesses due to immunosuppressive therapy, fistulous disease, and intraabdominal abscesses. Inflammatory bowel disease patients are also known to have a greater prevalence of amebiasis compared to the rest of the population; however, a higher incidence of amebic liver abscess has not been reported. We describe a case of a liver abscess in a patient with Crohn's disease that was initially presumed pyogenic but later determined to be amebic in origin. Epidemiology, c

Amebic liver abscess: a study of 11 cases compared with a series of 38 patients with pyogenic liver abscess.

Amebic liver abscess is an uncommon disease in the northern states of North America with 11 cases seen among approximately 500,000 Mount Sinai Hospital admissions over a 16-year period. Five of 11 cases originated in, or had recently visited South America. In three of these, and two patients with concomitant intestinal amebiasis, the diagnosis was suspected on admission. Diagnosis after admission was rapid, mean 5 days, compared with a mean of 13 days in pyogenic liver abscess. There was a higher incidence of male patients, nine males versus two females which was greater than the excess found

When free air misleads: pneumoperitoneum from a ruptured pyogenic liver abscess.

Pyogenic liver abscess (PLA) is a severe condition with mortality rates of 10%-19%. Spontaneous rupture occurs in about 5% of cases and carries much higher mortality. Rarely, ruptured PLA presents with pneumoperitoneum, mimicking hollow viscus perforation. We report a 48-year-old man with chronic gastric ulcer disease who presented with fever, malaise, nausea, and abdominal distension two weeks after endoscopic hemostasis. He rapidly deteriorated with septic shock and peritoneal signs. Ultrasound was inconclusive, and repeat X-ray revealed pneumoperitoneum. Laparotomy found purulent peritoniti

Gas Forming Pyogenic Liver Abscess Diagnosed by Point of Care Ultrasound.

Gas-forming pyogenic liver abscess (GFLPA) carries a high mortality rate. Early identification of the source of infection in sepsis results in better survival. Bedside point of care ultrasound (POCUS) can be used to help localize a source of infection. A 59-year-old man presented with systemic inflammatory response syndrome (SIRS) and was diagnosed with GFLPA on the initial encounter via clinical assessment and POCUS examination. After commencing antibiotics, optimal glucose control, adequate fluid resuscitation, and early infective source control, he achieved full recovery and was followed up

Prognostic risk factors for pyogenic liver abscess caused by Klebsiella pneumoniae.

BACKGROUND: Pyogenic liver abscess (PLA) caused by Klebsiella pneumoniae can vary in severity, and several risk factors for the development of organ dysfunction in PLA have been implicated. However, few studies to date have explored the most common risk factors for clinical severity. METHODS: We conducted a study on patients with PLA caused by Klebsiella pneumoniae between February 2013 and December 2022.Using logistic regression analysis, we sought to identify factors associated with positive blood culture, septic shock, and intensive care unit (ICU) admission. RESULTS: After included 200 p

Hepato-Duodenal Fistula Complicating a Pyogenic Liver Abscess: An Unusual Presentation.

An elderly male with type 2 diabetes mellitus was admitted to the emergency ward with fever and pain over the right hypochondrium since one month. An abdominal ultrasound revealed an ill-defined hypoechoic lesion with multiple air foci measuring 8 x 6 x 4 cm within the left lobe of the liver implicating segments III b and IV. A contrast-enhanced computed tomography (CECT) scan of the abdomen showed a similar lesion with leakage of oral contrast into the dependent areas of the collection from a rent in the antero-inferior aspect of the first part of the duodenum. Pyogenic liver abscess caused by Salmonella Enteritidis: a rare case report.

Salmonella Enteritidis is one of the most important serovars transmitted from animals to humans and a serovar most commonly reported worldwide. Infection with Enteritidis is mainly limited to the intestinal tract, but under certain circumstances may cross the mucosal barrier to disseminate and get established as some localized infectious focus. Although cited as one of the very uncommon causes, Enteritidis may involve the liver and evolve into an overt abscess. Pyogenic liver abscess by a gas forming organism like Enteritidis usually follow a serious fulminant course and associated morbidity a

Pyogenic liver abscess related to dental disease in an immunocompetent host.

A 59-year-old man with poor oral hygiene presented to our hospital because of fever and chills. Abdominal ultrasonography and enhanced computed tomography (CT) revealed a liver abscess. The patient had no history of immunodeficiency and we confirmed the patient had no immunologic abnormalities. Blood culture revealed Fusobacterium nucleatum, a bacterium commonly found in the oral cavity. Even if a patient is immunocompetent, poor oral hygiene might be an independent risk factor for a pyogenic liver abscess. Professional mechanical tooth cleaning (PMTC) and appropriate self-care are recommended

Hepatocellular carcinoma presenting as pyogenic liver abscess: characteristics, diagnosis, and management.

We performed a 17-year retrospective analysis of 10 cases of hepatocellular carcinoma presenting as pyogenic liver abscess. Spontaneous tumor necrosis and biliary obstruction caused by tumor thrombi, superimposed with bacterial infection, were the two major pathogeneses. Exact diagnosis of the underlying hepatocellular carcinoma was made for five of the 10 patients before management was attempted. Main clinical manifestations included fever, chills, right-upper-quadrant pain, malaise, anorexia, jaundice, and hepatomegaly. Characteristics such as middle age and male sex, seropositivity for hepa

Iron Metabolism Disorders Associated With the Severity of Pyogenic Liver Abscess.

Pyogenic liver abscess (PLA) is a serious intra-abdominal infection with high morbidity and mortality. Current diagnostic methods are time-consuming, inefficient and high-cost. Rapid identification/assessment of PLA and early recognition of bacterial liver abscess (BLA) improve prognosis. Iron metabolism disorders play a critical role in infections, but their role in PLA (especially ferritin) remains unclear. Notably, dietary nutrition, as a modulator of iron metabolism/immune function, may influence PLA progression, with limited research. In this study, we collected clinical data of 125 PLA p

Primary hepatic lymphoma presenting as pyogenic liver abscess: A case report.

BACKGROUND: Primary hepatic lymphoma (PHL) is a lymphoproliferative disorder confined to the liver without peripheral lymph node involvement and bone marrow invasion. PHL is extremely rare in clinical practice. The etiology and pathogenesis of PHL are largely unknown. There are no common standard protocols or guidelines for the treatment of PHL. CASE SUMMARY: We report the case of a 66-year-old man who presented with fever and abdominal pain for three weeks. Computed tomography and magnetic resonance imaging scans showed a pyogenic liver abscess. The patient underwent a right posterior hepate

Incidence of Colorectal Cancer in Patients Diagnosed With Pyogenic Liver Abscess.

IMPORTANCE: Colorectal cancer (CRC) can compromise the mucosal barrier and subsequently allow a route for bacterial invasion into the portal system or systemic circulation. Despite preliminary data suggesting that patients who experienced pyogenic liver abscess (PLA) have higher CRC incidence rates, data from outside the Southeast Asian population are sparse. OBJECTIVE: To investigate whether there is an association between PLA and the subsequent incidence of diagnosed CRC using the Veterans Health Administration (VHA) cohort. Septic Shock Secondary to a Pyogenic Liver Abscess Following Complicated Appendicitis.

Pyogenic liver abscesses (PLAs) are a rare condition in North America and Europe and, rarer still, the cause of septic shock. This case report will describe the rare occurrence of a PLA producing septic shock in a 36-year-old male residing in the United Kingdom following a case of complicated appendicitis. The patient presented to the emergency department (ED) with a three-week history of intermittent loose stools, cramping abdominal pain, recurrent fevers, a heart rate of 111 beats per minute, a blood pressure of 94/58 mmHg, and a fever of 40.1 degrees Celsius. Recent upper gastrointestinal panendoscopy increases the risk of pyogenic liver abscess.

AIM: To investigate the association between a recent gastrointestinal (GI) endoscopy and the subsequent risk of pyogenic liver abscess (PLA). METHODS: We designed a nested case control study. Using the Taiwan National Health Insurance Research Database, 2135 patients with a first diagnosis of PLA were identified from 1998 to 2011. Another 10675 patients without PLA matched by age and sex were selected as reference controls. We identified and compared the possible risk factors for PLA and GI endoscopies performed before the index date (when PLA was diagnosed) between the two cohorts. Multivari
