Acceptability of a Mobile-Health Living Kidney Donor Advocacy Program for Black Wait-Listed Patients.

Marked racial disparities exist in rates of living donor kidney transplantation (LDKT). The Living Organ Video Educated Donors (LOVED) program is a distance-based, mobile health program designed to help Black kidney transplant wait-list patients advocate for a living donor. This study reported on the acceptability outcomes to aid in future refinements. Participants were randomized to LOVED (n = 24, mean age = 50.9 SD (9.2) years), male = 50%) and usual care groups (n = 24 (mean age 47.9 SD (10.0), male 50%). Four LOVED groups completed an eight-week intervention that consisted of six online vi

Should Deceased Donation be Morally Preferred in Uterine Transplantation Trials?

In recent years much research has been undertaken regarding the feasibility of the human uterine transplant (UTx) as a treatment for absolute uterine factor infertility (AUFI). Should it reach clinical application this procedure would allow such individuals what is often a much-desired opportunity to become not only social mothers (via adoption or traditional surrogacy arrangements), or genetic and social mothers (through gestational surrogacy) but mothers in a social, genetic and gestational sense. Like many experimental transplantation procedures such as face, hand, corneal and larynx transp

Critical Illness Versus End-of-Life Conversations:A Novel Curriculum for Enhancing Resident Physician Communication Skills.

CONTEXT: End-of-life (EOL) discussions are of utmost importance to emergency medicine physicians, and residency programs vary in their training on this subject, despite being an Accreditation Council for Graduate Medical Education requirement. OBJECTIVES: The primary objective of this work was to improve resident familiarity by delivering bad news to patients and/or families in time of critical illness and/or EOL discussions. The secondary objective was exposure to interpreting living wills and physician orders for life-sustaining treatment, which previous data from our department indicated t

Emergency adult to adult living donor liver transplantation for fulminant hepatic failure.

BACKGROUND: The high mortality rate associated with fulminant hepatic failure combined with the limited availability of cadaveric organs requires consideration of alternatives to conventional cadaveric transplantation. Use of the donor right lobe in adult-to-adult living donor transplantation holds promise in a variety of circumstances, including high-acuity situations. METHODS: A 28-year-old male with fulminant hepatic failure secondary to hepatitis B was referred to our institution. He rapidly progressed to grade IV encephalopathy, and laboratory values were indicative of a poor prognosis w

Preferences of heart failure patients in daily clinical practice: quality of life or longevity?

AIMS: Knowledge of patient preferences is vital for delivering optimal healthcare. This study uses utility measurement to assess the preferences of heart failure (HF) patients regarding quality of life or longevity. The utility approach represents the perspective of a patient; facilitates the combination of mortality, morbidity, and treatment regimen into a single score; and makes it possible to compare the effects of different interventions in healthcare. METHODS AND RESULTS: Patient preferences of 100 patients with HF were assessed in interviews using the time trade-off (TTO) approach. Heal

Are Thai MSM willing to take PrEP for HIV prevention? An analysis of attitudes, preferences and acceptance.

OBJECTIVE: We aimed to understand the attitudes, preferences and acceptance of oral and parenteral PrEP among men who have sex with men (MSM) in Thailand. BACKGROUND: Pre-exposure prophylaxis (PrEP), the use of antiretrovirals to prevent HIV acquisition, has shown promising results in recent trials. To assess the potential impact of this new HIV prevention method, in addition to efficacy data, we need to understand which psychosocial factors are likely to determine its uptake among members of potential user groups. METHODS AND FINDINGS: Surveys of willingness to use PrEP products were admini

Supporting Patients with Psychosis in the Community To Stand Up and Move More: Perspectives of Community Mental Health Staff.

People with psychosis typically show high levels of sedentary behaviour and low levels of physical activity. Effective interventions are needed, and staff will play a crucial role in implementation. Aims: To understand staff perspectives on reducing sedentary behaviour and increasing physical activity. Eighteen staff from NHS mental health trust community teams were interviewed, with data analysed using reflexive thematic analysis. Four themes were developed: (1) Choosing to target movement: staff recognise the need to address movement but struggle to prioritise it; (2) Encouraging but not ste

Strategies for the Prevention of Sudden Cardiac Death in Children and Adolescents.

Sudden cardiac arrest (SCA) or sudden cardiac death (SCD) in children and adolescents is a devastating event. Although the true incidence is unclear, the etiologies are not. Strategies for prevention include both primary as well as secondary prevention strategies, and these strategies are not mutually exclusive. From a primary prevention standpoint, many of the episodes of SCA or SCD may occur without any antecedent warning signs and in the absence of an important family history. There are a subset of children and adolescents who may have antecedent warning signs and symptoms such as syncope w

Biological measures of the standard of living.

When economists investigate long-term trends and socioeconomic differences in the standard of living or quality of life, they have traditionally focused on monetary measures such as gross domestic product--which has occupied center stage for over 50 years. In recent decades, however, scholars have increasingly recognized the limitations of monetary measures while seeking useful alternatives. This essay examines the unique and valuable contributions of four biological measures--life expectancy, morbidity, stature, and certain features of skeletal remains--to understand levels and changes in hum

'My dad was like "it's your brain, what are you doing?"': Participant experiences of repetitive transcranial magnetic stimulation treatment in severe enduring anorexia nervosa.

OBJECTIVE: Repetitive transcranial magnetic stimulation (rTMS) is a promising emerging treatment for anorexia nervosa (AN). However, to date, patients' views and experiences of this treatment have not been fully explored. To assess these, we integrated a qualitative study into a feasibility randomised controlled trial of rTMS in individuals with severe enduring AN. METHOD: Twenty-nine (of 34) trial participants contributed to this study. Semi-structured interviews were conducted 3-months following the completion of rTMS treatment (4-months post-randomisation), prior to unblinding. Transcripts

How to address smoking cessation in HIV patients.

Tobacco consumption is the modifiable risk factor contributing most to the development of non-AIDS-defining events among persons living with HIV/AIDS (PLWHA). Clinicians' awareness of this problem is critical and not yet adequate. Practical information issued by public health authorities or contained in experts' clinical guidelines regarding how to address smoking cessation in PLWHA is scarce. The aim of this review is to provide physicians with comprehensive and practical information regarding how to identify HIV-positive patients willing to stop smoking and those more likely to succeed, how

Epilepsy and neurocysticercosis in a village in Huaraz, Peru.

Sixteen individuals presenting with seizures in a rural village of Peru were screened for antibodies to Taenia solium, and those willing to attend were offered a complete neurological work-up including EEG and cerebral CT scan in a reference center. Seroprevalence using immunoblot was 35% (5/16). Eight individuals came for examination. CT scans were abnormal in all four seropositive cases (an enhancing lesion in one case, multiple live cysts and calcifications in one case, and multiple calcifications in two cases), and normal in the four seronegative individuals. Generation Y and surgical residency - Passing the baton or the end of the world as we know it? Results from a survey among medical students in Germany.

INTRODUCTION: The current student generation have their own expectations toward professional life and pay particular attention to their work-life balance. Less interest in work-intensive specialties leads to a shortage of skilled candidates especially in surgery. In order to motivate students into a surgical residency, new priorities become important. A deeper understanding of the underlying arguments and students' expectations towards a surgical training are necessary to counteract a future shortage of specialized surgeons. METHODS: We conducted an internet-based survey among medical student

Living donor kidney transplantation in a global environment.

Live donor kidney transplantation has become a widely sought treatment by patients with end-stage renal failure. As the outcome for the genetically and emotionally related live donor transplants is the same, this review considers live kidney transplantation from the broad scope of current international practice. Unrelated live donor transplantation can now be performed for incompatible donor recipient pairs via a simultaneous paired kidney donation. However, acceptance of the scientific data that an unrelated live donor transplant can now be performed successfully should not be misconstrued as
