Australian and Canadian clinicians' views and application of 'carbon health literacy': a qualitative study.

BACKGROUND: Clinical care contributes to at least 50% of the greenhouse gas (GHG) emissions of healthcare. This includes the 40% of healthcare that is harmful or low value, adding avoidable emissions without improving health or quality of care. Clinicians are well-placed to mitigate emissions associated with the provision of clinical care. This study aimed to explore clinicians' views on a new construct we have termed 'carbon health literacy' to understand how knowledge, skills and capacities related to the emissions of clinical care has application in clinical practice. METHODS: Qualitative

Low-Value Proton Pump Inhibitor Prescriptions Among Older Adults at a Large Academic Health System.

BACKGROUND: Older adults are particularly vulnerable to complications from proton pump inhibitor (PPI) drugs. We sought to characterize the prevalence of potentially low-value PPI prescriptions among older adults to inform a quality improvement (QI) intervention. METHODS: We created a cohort of patients, aged 65 years or older, receiving primary care at a large academic health system in 2018. We identified patients currently prescribed any PPI using the electronic health record (EHR) medication list (current defined as September 1, 2018). A geriatrician, a gastroenterologist, a QI expert, and

Health care utilization patterns of patients enrolled in an interdisciplinary program for back pain.

BACKGROUND: Chronic low back pain is a costly condition and the leading cause of disability worldwide. A recent call to action identified the overuse of low-value health care as ineffective and potentially harmful in the context of back pain. Interdisciplinary pain programs (IPPs) incorporate physical therapy and behavioral pain treatments with the goal of empowering patients to actively use skills to self-manage pain, decreasing stress, disability, and reliance on low-value care. While IPPs are deemed "cost-effective," few studies elaborate upon the patterns of healthcare utilizations among p

Decreasing Admissions to the NICU: An Official Transition Bed for Neonates.

BACKGROUND: Evidence supports the need to decrease healthcare costs. One approach may be minimizing use of low-value care by reducing the number of unnecessary neonatal intensive care unit (NICU) admissions through the use of official neonatal transition beds. PURPOSE: To evaluate whether transition beds decrease unnecessary NICU admissions and estimate the cost savings of this practice change. METHODS: This retrospective chart review examined the records of all neonates of 350/7 weeks' gestational age and greater with birth weights of 2000 g and more admitted to a neonatal transition bed fr

A value-based care program in dentistry centered on caries risk assessment: Preliminary results.

BACKGROUND: The Benefits and Rewards for Utilization, Services and Healthy Outcomes program (known as BRUSH) (Liberty), a value-based oral health care program, was implemented in 4 US states from 2021 through 2022 using a caries risk assessment (CRA) strategy. This value-based care program provides oral health care for Medicaid beneficiaries up to the age of 21 years. METHODS: Deidentified data from patient insurance claims that included CRA levels with 2-year follow-up for 2,706 patients in Florida were available for analysis. Providers were incentivized to manage caries on the basis of an i

[Reducing inappropriate vitamin D testing: an overview].

25-(OH)-vitamin D testing is an example of low-value care whose prevalence has increased dramatically in recent years. In Switzerland, testing rates doubled in less than a decade, resulting in annual costs of over 100 million Swiss francs, without any demonstrated clinical benefit. This article provides an overview of various national and international approaches aimed at reducing low-value care and overtesting. The analysis compares the effectiveness of informative approaches (smarter medicine - Choosing Wisely), decision support approaches (clinical alerts), restrictive approaches (reimburse

The Joy Is in the Gray-The Problems Family Medicine Leaders Should Be Talking About.

Family medicine care in the United States has been suppressed through structural impediments in the healthcare system, yet could provide more care and better health outcomes with appropriate changes. The role of general practitioners in other developed countries provide insights on the value of primary care that could be better incorporated into US healthcare. The fundamental value of family physicians' care emerges from generalist thinking, particularly the use of probabilistic reasoning that supports care variation when patient preferences, situations, capabilities, as well as potential outc

[Comparative analysis of regional patient safety strategies in Spain: Structure, content, and alignment with the national strategy].

INTRODUCTION: Patient safety is a strategic priority for healthcare systems. In Spain, decentralisation of the National Health System (NHS) facilitates the adaptation of policies to regional characteristics, but poses challenges in terms of coordination, standardisation, and evaluation. OBJECTIVES: To compare regional patient safety strategies, assess their concordance with the 2015-2020 and 2025-2035 NHS strategies, and identify the changes and advances introduced in the new national strategy compared with its predecessor. METHODS: A retrospective descriptive study was conducted using docum

Does Value Vary by Center Surgical Volume for Neonates With Truncus Arteriosus? A Multicenter Study.

BACKGROUND: Truncus arteriosus is a congenital heart defect with high resource use, cost, and mortality. Value assessment (outcome relative to cost) can improve quality of care and decrease cost. This study hypothesized that truncus arteriosus repair at a high-volume center would result in better outcomes at lower cost (higher value) compared with a low-volume center. METHODS: This study retrospectively analyzed a multicenter cohort of neonates undergoing truncus arteriosus repair (2004 to 2015) by using the Pediatric Health Information Systems database. Multivariate quantile, logistic, and n

Drug use and spending under a formulary informed by cost-effectiveness.

BACKGROUND: The National Academy of Medicine has called for value-based drug formularies to address health plan prescription drug spending while maintaining access to high-value medicines. Thirty employer-sponsored plans implemented a "Value-Based Formulary-essentials" (VBF-e) program that uses cost-effectiveness evidence to inform cost-sharing and coverage exclusion. OBJECTIVE: To evaluate if the VBF-e was associated with changes in medication use and patient out-of-pocket spending and health plan spending on prescription drugs and other health care. METHODS: This was a cohort study using a d

Obstetric care provider's knowledge about the use of low dose aspirin for preeclampsia prevention in low and middle income countries: a systematic review and meta-analysis.

INTRODUCTION: Preeclampsia can elevate the likelihood of unfavorable consequences for a mother, such as severe morbidity and mortality. World Health Organization recommends low dose acetylsalicylic acid (aspirin, 75 mg per day) for the prevention of preeclampsia in women at moderate or high risk of developing the condition. The use of low dose aspirin is dependent on the knowledge of health care providers working in the antenatal care units. We found inconsistent figures regarding the knowledge level of health care providers on low dose aspirin for preeclampsia prevention around different low

Association between measures of spinal mobility and low back pain. An analysis of new attenders in primary care.

STUDY DESIGN: Comparison of spinal movements in new attenders to primary care with an episode of low back pain with a sample with no recalled history of ever having had low back pain. OBJECTIVES: To examine the association between restriction of spinal movement and the presence of low back pain in primary care. SUMMARY OF BACKGROUND DATA: The presence of restriction in spinal mobility has frequently been investigated by specialists in the clinical setting. What is not known, however, is whether an association exists between pain and restriction in those newly presenting to primary care and w

Integrating a Parenting Assessment into Practice: Pediatric Providers' Time and Perspectives.

PURPOSE: To integrate a parenting assessment into primary care and assess pediatric providers' time needed to review it and their perceptions of the process. DESCRIPTION: The Quick Parenting Assessment (QPA) is a validated, 13 item parent support tool that assesses for healthy and unhealthy parenting practices. Higher QPAs indicate more unhealthy parenting being used. In a clinic serving low-income parents, the QPA was integrated into the 15 month, 30 month, 5 year, and 8 year well child visits. After each well child visit in which the QPA was administered, providers were invited to complete

Evolving determinants of implementing low-risk childbirth quality improvement in Japan: a longitudinal qualitative study.

BACKGROUND: International evidence suggests that reducing user costs increases maternity care utilization but not necessarily care quality, underscoring the need for transparent, comparable measurement. In Japan, evidence-based maternity care for low-risk childbirth has not been systematically evaluated, and limited claims-data capture hinders assessment, benchmarking, and sustained quality improvement. We aimed to explore how determinants (facilitators and barriers) of implementing a multifaceted quality improvement intervention for low-risk childbirth emerge and evolve over time. METHODS: T

Interplay between psychosocial and heart failure related factors may partially explain limitations in self-efficacy in patients with heart failure: Insights from a real-world cohort of 1,123 patients.

BACKGROUND AND AIMS: Worse self-care is associated with a higher risk of readmission and mortality in patients with heart failure (HF). Little is known about how the interplay between clinical and psycho-social factors may modulate self-care behaviours in these patients. The aim of our study was to identify clinical, and particularly psycho-social factors associated with worse self-care and assess their interaction inpatients with heart failure. METHODS AND RESULTS: We conducted an observational, prospective, cohort study of 1,123 consecutive patients with chronic heart failure. Self-care was

Promoting Reflection on Medical Maximizing-Minimizing Preferences May Create Undesired Effects on Decisions About Low-Benefit and High-Benefit Care.

BACKGROUND: Medical maximizing-minimizing (MM) preferences predict a variety of medical decisions. We tested whether informing people about their MM preferences and asking them to reflect on the pros and cons of that preference would improve medical decisions when clear clinical recommendations exist. METHODS: We surveyed 1219 US adults age 40+ that were sampled to ensure a 50%/50% distribution of medical maximizers versus minimizers. Participants either received no MM feedback (Control) or received feedback about their MM type and instructions to reflect on how that MM type can be helpful in
