BACKGROUND: Lumefantrine (benflumetol) is a fluorene derivative belonging to the aryl amino alcohol class of anti-malarial drugs and is commercially available in fixed combination products with β-artemether. Impurity characterization of such drugs, which are widely consumed in tropical countries for malaria control programmes, is of paramount importance. However, until now, no exhaustive impurity profile of lumefantrine has been established, encompassing process-related and degradation impurities in active pharmaceutical ingredients (APIs) and finished pharmaceutical products (FPPs).

Malaria remains a persistent global health challenge, and emerging resistance to artemisinin-based therapy threatens disease control. Here, we report the evaluation of scaffolds selected from three classes of indole hybrids. A library of 24 synthetically designed indoles was screened in vitro against Plasmodium falciparum sexual and asexual forms. The derivatives displayed a wide range of activity, from inactive to potent, with compound ARN53 exhibiting the strongest inhibition (IC₅₀ = 684.8 ± 289.3 nM). Cytotoxicity assays in HEK293T mammalian cells demonstrated the active compounds to be mor

Lumefantrine (LMF) is a low-solubility antimalarial drug that cures acute, uncomplicated malaria. It exerts its pharmacological effects against erythrocytic stages of Plasmodium spp. and prevents malaria pathogens from producing nucleic acid and protein, thereby eliminating the parasites. Modifying the structure of a drug through the formation of a pharmaceutical cocrystal or salt presents an avenue through which its physicochemical properties can be optimized. In this work, we report the design/synthesis and solid-state characterization of four new salts and cocrystal-salt forms of LMF; an LM

Malaria remains a major global health threat, exacerbated by increasing drug resistance and limited immunological protection. This study investigates the anti-plasmodial and immunomodulatory efficacy of novel synthetic heterocyclic compounds targeting indoleamine 2,3-dioxygenase-1 (IDO-1), a key regulator of immune suppression during malaria. Among the tested compounds, 8-fluorotryptanthrin (C1), exhibited the most promising profile with strong in vitro activity against Plasmodium falciparum (3D7 and Dd2 strains) and P. berghei ANKA with high selectivity indices and minimal cytotoxicity and ha

OBJECTIVE: Malaria parasites invade, grow and multiply inside erythrocytes and obtain nourishment from haemoglobin. Then, the released haem group is oxidized to haematin and inert dimeric haemozoin bio-crystals form, which provides the parasite a unique way to avoid the toxicity associated with the haem group. Therefore, antimalarial drugs are designed to inhibit dimer formation; however, recent electrochemical studies indicate that an inert dimer also promotes a toxic oxidizing environment. Therefore, this work explores drug reactivity in the presence of monomers and dimers to evaluate their

BACKGROUND: Malaria causes significant morbidity in Malawi, with an estimated 5 million cases in 2014. Artemether-lumefantrine (AL) and artesunate-amodiaquine (ASAQ) are the first- and second-line treatments for uncomplicated malaria, respectively, but emerging resistance threatens their efficacy. In order to understand whether AL and ASAQ remain efficacious for the treatment of uncomplicated Plasmodium falciparum malaria in Malawi, a therapeutic efficacy trial was conducted.
METHODS: During March-July 2014, febrile children aged 6-59 months with microscopy-confirmed uncomplicated P. falcipar

The most severe type of malaria, P. falciparum may progress with several complications. We have detected hyponatremia and thrombocytopenia in the cases with falciparum malaria. We have launched coartemether which is used in resistant falciparum and supportive care with our four cases. We found out hyponatremia in the cases because serum concentrations were between 117-134 mEq/L. Clinical conditions of the case were highly heavy, in which blood sodium level was pretty low. While the other three cases responded to the treatment in three days, this case responded after the fifth day. Owing to thi

The involvement of Pfmdr1 (Plasmodium falciparum multidrug resistance 1) polymorphisms in antimalarial drug resistance is still debated. Here, we evaluate the association between polymorphisms in Pfmdr1 (N86Y, Y184F, S1034C, N1042D, and D1246Y) and Pfcrt (K76T) and in vitro responses to chloroquine (CQ), mefloquine (MQ), lumefantrine (LMF), quinine (QN), monodesethylamodiaquine (MDAQ), and dihydroartemisinin (DHA) in 174 Plasmodium falciparum isolates from Dakar, Senegal. The Pfmdr1 86Y mutation was identified in 14.9% of the samples, and the 184F mutation was identified in 71.8% of the isolat

BACKGROUND: The World Health Organization recommends that regular efficacy monitoring should be undertaken by all malaria endemic countries that have deployed artemisinin combination therapy (ACT). Although ACT is still efficacious for treatment of uncomplicated malaria, artemisinin resistance has been reported in South East Asia suggesting that surveillance needs to be intensified by all malaria endemic countries. This study assessed the efficacy and safety of artemether-lumefantrine (AL) for the treatment of uncomplicated falciparum malaria in Muheza district of north-eastern Tanzania, an ar

BACKGROUND: To date no comparative trials have been done, to our knowledge, of fixed-dose artemisinin combination therapies (ACTs) for the treatment of Plasmodium falciparum malaria in pregnancy. Evidence on the safety and efficacy of ACTs in pregnancy is needed as these drugs are being used increasingly throughout the malaria-affected world. The objective of this study was to compare the efficacy, tolerability, and safety of artemether-lumefantrine, the most widely used fixed ACT, with 7 d artesunate monotherapy in the second and third trimesters of pregnancy.

BACKGROUND: Decreased efficacy of artemether-lumefantrine, the globally most used antimalarial, has recently emerged in Africa.
METHODS: An efficacy trial was carried out based on directly observed artemether-lumefantrine therapy at Bengo, Northern Angola. One-hundred Plasmodium falciparum uncomplicated malaria patients (2-10 years old) were enrolled, hospitalized for the treatment period, and followed up for 42 days. Polymerase chain reaction (PCR) correction was performed with pfmsp1/2 plus glurp, with analysis considering 2 or 3 coincident markers. Infections were tested by quantitative PC

Amorphous solid dispersions (ASDs) have been observed to release their active ingredients in an aqueous medium as drug-rich nanoparticles, leading to high apparent solubility and potential benefits for drug absorption. We have prepared the ASDs of lumefantrine with 10 polymers and tested their release of drug-rich nanoparticles in 0.1 % sodium dodecyl sulfate. Poly(acrylic acid) (PAA) significantly outperforms the other polymers in reaching the highest apparent solubility through a 0.2 μm filter, and its outperformance persists as drug loading increases from 25 % to 50 %, without sudden declin

BACKGROUND: Gut microbiota were recently shown to impact malaria disease progression and outcome, and prior studies have shown that Plasmodium infections increase the likelihood of enteric bacteria causing systemic infections. Currently, it is not known whether Plasmodium infection impacts human gut microbiota as a prelude to bacteremia or whether antimalarials affect gut microbiota. Our goal was to determine to what degree Plasmodium infections and antimalarial treatment affect human gut microbiota.
METHODS: One hundred Kenyan infants underwent active surveillance for malaria from birth to 1

BACKGROUND: Blackwater fever is a complication of malaria infection consisting of a syndrome of febrile intra-vascular haemolysis with severe anaemia and intermittent passage of dark-red to black colour urine. Despite numerous reports and studies of this condition, its pathogenesis remains incompletely understood.
CASE PRESENTATION: This report describes a case of classic blackwater fever in a returning traveller, without prior history of malaria infection nor usage of anti-malarial prophylaxis, treated with two courses of oral artemether-lumefantrine combination therapy. Unusual persistence

BACKGROUND: Between 1995 and 2000, KwaZulu-Natal province, South Africa, experienced a marked increase in Plasmodium falciparum malaria, fuelled by pyrethroid and sulfadoxine-pyrimethamine resistance. In response, vector control was strengthened and artemether-lumefantrine (AL) was deployed in the first Ministry of Health artemisinin-based combination treatment policy in Africa. In South Africa, effective vector and parasite control had historically ensured low-intensity malaria transmission. Malaria is diagnosed definitively and treatment is provided free of charge in reasonably accessible pu

INTRODUCTION: Artemether-lumefantrine (AL) is an effective drug combination that is used to treat uncomplicated falciparum malaria worldwide including travellers. Although this treatment is regarded as highly effective, recrudescence of falciparum malaria may occur in the weeks after treatment with AL. The occurrence of recrudescence and its potential (risk) factors in travellers remain poorly investigated.
METHODS: This retrospective cohort study included falciparum malaria cases treated with AL at a tertiary referral hospital in the Netherlands, between January 1, 2010, and July 1, 2024. Th

There are increasing reports of failure of artemisinin-based combination therapy (ACT) in malaria patients returning from endemic areas. ACT has been used globally as first-line treatment for Plasmodium falciparum malaria. However, with the emergence of artemisinin-resistant strains of P.falciparum and their spread across Southeast Asia, there is a risk that these resistant strains could reach areas of high malaria incidence in Africa and elsewhere. In this case report, two cases of malaria imported from Africa, initially treated with six doses of artemether-lumefantrine (AL) were presented. T

Background: Artemisinin-based combination therapies (ACTs) are the global mainstay treatment of uncomplicated Plasmodium falciparum infections. PfMDR1 and PfCRT are two transmembrane transporters, associated with sensitivity to several antimalarials, found in the parasite food vacuole. Herein, we explore if their relatedness extends to overlapping patterns of gene transcriptional activity before and during ACT administration. Methods: In a clinical trial performed in Tanzania, we explored the pfmdr1 and pfcrt transcription levels from 48 patients with uncomplicated P. falciparum malaria infect

BACKGROUND: The choice of malaria treatment for HIV-infected pregnant women receiving efavirenz-based antiretroviral therapy must consider the potential impact of drug interactions on antimalarial exposure and clinical response. The aim of this study was to investigate the effects of efavirenz on artemether-lumefantrine (AL) because no studies have isolated the impact of efavirenz for HIV-infected pregnant women.
METHODS: A prospective clinical pharmacokinetic (PK) study compared HIV-infected, efavirenz-treated pregnant women with HIV-uninfected pregnant women in Tororo, Uganda. All women rec

BACKGROUND: Artemether-lumefantrine is currently the most widely recommended treatment of uncomplicated malaria. Lopinavir-based antiretroviral therapy is the commonly recommended second-line HIV treatment. Artemether and lumefantrine are metabolised by cytochrome P450 isoenzyme CYP3A4, which lopinavir/ritonavir inhibits, potentially causing clinically important drug-drug interactions.
METHODS: An adaptive, parallel-design safety and pharmacokinetic study was conducted in HIV-infected (malaria-negative) patients: antiretroviral-naïve and those stable on lopinavir/ritonavir-based antiretrovira
