Challenges in the management of hip dislocation disease in Madagascar.

BACKGROUND: Developmental dysplasia of the hip (DDH) remains a significant public health challenge, particularly in developing countries where cultural factors and limited access to appropriate medical equipment complicate optimal management. AIM: To evaluate the difficulties encountered in the management of DDH in our healthcare setting. METHODS: A retrospective, single-center study was conducted over nine years (2015-2023), including 20 patients (26 hips) with idiopathic DDH. Patients with post-traumatic or post-infectious hip dislocations were excluded. Data collected included age at diag

Opportunities and challenges for plague vector control in Madagascar.

Plague is a rodent-borne disease transmitted to humans by the bite of fleas infected with the bacterium Yersinia pestis. Flea control is a key part of the overall plague management strategy. Insecticide-based strategies are designed to reduce or eliminate fleas from the environment in order to stop the disease transmission cycle during outbreaks. Many efforts have been directed toward monitoring flea susceptibility to insecticides using standardized bioassay methods recommended by the World Health Organization (WHO). Several studies have reported the development of insecticide resistance in ve

Review of genotyping methods for Yersinia pestis in Madagascar.

BACKGROUND: Plague, a zoonotic disease caused by Yersinia pestis, was responsible for 3 historical human pandemics that killed millions of people. It remains endemic in rodent populations in Africa, Asia, North America, and South America but human plague is rare in most of these locations. However, human plague is still highly prevalent in Madagascar, which typically records a significant part of all annual global cases. This has afforded an opportunity to study contemporary human plague in detail using various typing methods for Y. pestis. AIM: This review aims to summarize the methods that

Co-Radiation of Leptospira and Tenrecidae (Afrotheria) on Madagascar.

Leptospirosis is a bacterial zoonosis caused by pathogenic Leptospira that are maintained in the kidney lumen of infected animals acting as reservoirs and contaminating the environment via infected urine. The investigation of leptospirosis through a One Health framework has been stimulated by notable genetic diversity of pathogenic Leptospira combined with a high infection prevalence in certain animal reservoirs. Studies of Madagascar's native mammal fauna have revealed a diversity of Leptospira with high levels of host-specificity. Native rodents, tenrecids, and bats shelter several distinct

Factors influencing the re-emergence of plague in Madagascar.

Plague is an infectious disease found worldwide and has been responsible for pandemics throughout history. Yersinia pestis, the causative bacterium, survives in rodent hosts with flea vectors that also transmit it to humans. It has been endemic in Madagascar for a century but the 1990s saw major outbreaks and in 2006 the WHO described the plague as re-emerging in Madagascar and the world. This review highlights the variety of factors leading to plague re-emergence in Madagascar, including climate events, insecticide resistance, and host and human behaviour. It also addresses areas of concern f

Overview of accessibility and quality of antiepileptic drugs in Madagascar.

PURPOSE: To determine the accessibility of treatment and the quality of antiepileptic drugs (AEDs) in the Haute Matsiatra district of Madagascar. METHODS: Cross-sectional descriptive study and interviews. Samples of 10 units of each available AED were collected, and the active ingredient was quantified by reversed-phase high-performance liquid chromatography (RP-HPLC) with photodiode-array UV detection. The quality of an AED was considered satisfactory if the quantity of active ingredient in each tablet was in the range ±15% of the average value according to the European Pharmacopeia (6th edi

Coevolution of Cyanogenic Bamboos and Bamboo Lemurs on Madagascar.

Feeding strategies of specialist herbivores often originate from the coevolutionary arms race of plant defenses and counter-adaptations of herbivores. The interaction between bamboo lemurs and cyanogenic bamboos on Madagascar represents a unique system to study diffuse coevolutionary processes between mammalian herbivores and plant defenses. Bamboo lemurs have different degrees of dietary specialization while bamboos show different levels of chemical defense. In this study, we found variation in cyanogenic potential (HCNp) and nutritive characteristics among five sympatric bamboo species in th

Phylogeography and molecular epidemiology of Yersinia pestis in Madagascar.

BACKGROUND: Plague was introduced to Madagascar in 1898 and continues to be a significant human health problem. It exists mainly in the central highlands, but in the 1990s was reintroduced to the port city of Mahajanga, where it caused extensive human outbreaks. Despite its prevalence, the phylogeography and molecular epidemiology of Y. pestis in Madagascar has been difficult to study due to the great genetic similarity among isolates. We examine island-wide geographic-genetic patterns based upon whole-genome discovery of SNPs, SNP genotyping and hypervariable variable-number tandem repeat (VN

[Isolated prostate tuberculosis. Report of a case in Madagascar].

Isolated prostate tuberculosis is uncommon. Authors report herein the case of 64-year-old man who presented obstructive syndrome of low urinary tract. After clinical and biological examination, and imagery, prostate cancer was highly suspected. Transrectal biopsy was performed and histological examination showed tuberculosis lesions. We suggest that prostate tuberculosis should be evoked in old patient presenting similar signs and living in tuberculosis endemic zone.

[A case of Mycobacterium shimoïdei lung infection in Madagascar].

In 1980, a 32 years-old Madagascan female developed a pulmonary tuberculosis, bacteriologically confirmed. She cured with right apical cavitary sequellae. In 1989, she presented haemoptysis again. Antituberculous treatment was adopted without bacteriological confirmation and did not improve clinical symptoms. In 1991 and 1992 cultures from sputa and bronchi aspiration yielded acid-fast bacilli identified as Mycobacterium shimoïdei. M. tuberculosis could not be detected. The patient died during treatment. This case is the fourth one in the literature. Whereas previous cases have been reported i

[Hepatic hydatidosis in Madagascar: a case of autochthonous origin].

Hydatid disease, due to the larval stage of Echinococcus granulosus, is described for the second time at liver level, in Madagascar. Even with a low level of endemic animal disease and an uncontrolled population of stray dogs, the human parasitosis is very rare in Madagascar. It is interesting to note that, since a hundred of years, the epidemiological status of this Zoonosis has not really changed in the great island.

Is Anopheles mascarensis a new malaria vector in Madagascar?

Anopheles mascarensis De Meillon, 1947, a mosquito that is native to Madagascar, is reported for the first time to act as a vector of Plasmodium falciparum malaria. From September 1989 to March 1990, 2, 499 An. mascarensis specimens from different regions of Madagascar were tested by an enzyme-linked immunosorbent assay (ELISA), using monoclonal antibodies against the circumsporozoite (CS) protein of the four human species of Plasmodium. The salivary glands of 237 specimens were also dissected. Fourteen of 1,864 specimens obtained from Sainte Marie island on the Malagasy east coast were found

Zoonotic Semiotics: Plague Narratives and Vanishing Signs in Madagascar.

Zoonosis calls for a multispecies approach to medical semiotics, a method involving the decipherment of outward symptoms and the construction of narrative. In Madagascar, early detection of bubonic plague outbreaks relies on sightings of sick and dead rats. However, people most vulnerable to plague often do not perceive warning signs, and plague symptoms do not always present in rat and human bodies. In August 2015, a plague outbreak killed 10 residents of a rural hamlet in the central highlands. To reconstruct the transmission chain, scientists elicited survivors' memories of dead rats in the
