Guideline-based audit of the hospital management of heart failure with reduced ejection fraction.

BACKGROUND: Heart failure is a major burden in Australia in terms of morbidity, mortality and healthcare expenditure. Multiple evidence-based therapies are recommended for heart failure with reduced ejection fraction (HFrEF), but data on physician adherence to therapy guidelines are limited. AIM: To compare use of HFrEF therapies against current evidence-based guidelines in an Australian hospital inpatient population. METHODS: A retrospective review of patients admitted with a principal diagnosis of HFrEF across six metropolitan hospitals in Sydney, Australia, between January 2015 and June 2

Using transanal irrigation in the management of low anterior resection syndrome: a service audit.

Low anterior resection syndrome (LARS) is a collection of symptoms that can occur as a result of a low anterior resection for bowel cancer. Transanal irrigation (TAI) can be used to manage these symptoms. This article describes a retrospective audit of 15 patients who were using TAI to manage symptoms of LARS. The aim of the audit was to ascertain whether the use of TAI improved outcomes for these patients. The data suggest that TAI has reduced both the frequency of bowel movements and episodes of faecal incontinence. Those patients using higher volumes of water seem to have experienced more b

Clinical Practice Audit on the Management of Antineutrophil Cytoplasmic Antibody-Associated Vasculitis in the Netherlands.

INTRODUCTION: Managing complex and rare systemic autoimmune diseases such as antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV) can be challenging and is often accompanied by undesirable variations in clinical practice. Adequate understanding of clinical practice can help identify essential issues to improve the care for AAV patients. Therefore, we studied the real-life management and outcomes of AAV patients in the Netherlands. METHODS: In this cohort study, we investigated clinical practice in university and nonuniversity teaching hospitals with respect to patients with

A multi-centre audit of epistaxis management in England: is there a case for a national review of practice?

OBJECTIVE: The ENT-UK Clinical Audit and Practice Advisory Group initiated a pilot audit to investigate variance in epistaxis management between six units nationwide. METHOD: All patients with a diagnosis of epistaxis who were admitted for in-patient care at six ENT departments between November 2011 and February 2012 were prospectively enrolled. RESULTS: A total of 166 patients were included in the study. Variance was demonstrated between the six units in a number of the key outcome areas. Twenty-eight per cent of patients were identified as eligible for operative intervention for epistaxis

Are we following an algorithm for managing chronic anal fissure? A completed audit cycle.

BACKGROUND: Anal fissure is one of the commonest proctological diseases with considerable national variation in sequential treatment. We aimed to audit our compliance of chronic anal fissure (CAF) management with national guidance provided by the Association of Coloproctology of Great Britain and Ireland (ACPGBI). METHODS: We retrospectively audited patients presenting to outpatient clinics with CAF over a 6-month period. Using electronic patient records, notes and clinic letters, we compared their management with ACPGBI algorithm. A prospective re-audit was then performed. RESULTS: Forty-on

An audit of anesthetic management and complications of tracheo-esophageal fistula and esophageal atresia repair.

BACKGROUND: Many different anesthetic techniques have been suggested for the management of tracheo-oesophageal fistula/oesophageal atresia (TOF/OA) although the incidence of ventilation difficulty is not well known and it is unclear which technique is best in managing this. The aim of our audit was to determine the incidence of ventilation difficulty during repair of TOF/OA. We also recorded the current practice for anesthesia and analgesia in these children as well as the incidence of comorbidities and surgical complications. METHODS: We retrospectively audited cases of TOF/OA repair over a

Retrospective audit of the acute management of stroke in two district general hospitals in the uk.

BACKGROUND: There is some evidence to suggest that the standard of acute medical care provided to patients with cerebrovascular disease is a major determinant of the eventual outcome. Consequently, the Royal College of Physicians (RCP) of London issues periodic guidelines to assist healthcare providers in the management of patients presenting with stroke. OBJECTIVE: An audit of the acute management of stroke in two hospitals belonging to the same health care trust in the UK. METHOD: Retrospective review of 98 randomly selected case-notes of patients managed for cerebrovascular disease in two

Adult tetanus in Accra, why the high mortality? An audit of clinical management of tetanus.

BACKGROUND: Tetanus is a life threatening infection relatively uncommon in the developed countries but occurs frequently in developing countries with case fatality rates of 40-60 %. Recent review of adult tetanus at the Korle-bu Teaching Hospital showed a high case fatality of 50%. In order to determine the factors underlying this high case fatality we conducted a retrospective clinical audit of the clinical management of adult tetanus admitted in 1994 to 2001 to the Korle-bu Teaching Hospital. METHODS: Data extracted from the admission and discharge books of the Fevers' Unit of Korle-bu Teac

Audit of a conservative management policy of the axilla in elderly patients with operable breast cancer.

Between April 1982 and February 1994, 344 women aged > 70 years with cancers < 5 cm in diameter were treated at the City Hospital Breast Unit. The majority were enrolled in two successive randomized trials. One hundred and sixty patients had primary therapy with tamoxifen alone and were subsequently treated with mastectomy if the primary cancer progressed. Fifty-three women with a high oestrogen receptor (ER) status in the tumour received mastectomy and post-operative tamoxifen. One hundred and thirty-one patients underwent primary surgery (104, mastectomy; 27, wide local excision) and did not

Low-Risk Febrile Neutropenia Management: An Audit of Practice in a UK Cancer Center.

BACKGROUND: Febrile neutropenia (FN) is a known complication of chemotherapy. Guidelines from American Society of Clinical Oncology and National Institute for Health and Care Excellence support outpatient management for low-risk FN patients; however, implementation varies across United Kingdom (UK) cancer centers. While some centers have adopted outpatient pathways, others continue to rely on inpatient care, leading to inequities in FN management. Standardized use of risk stratification tools, such as the Multinational Association of Supportive Care in Cancer (MASCC) and Clinical Index of Stab
