The prevalence of treatment-emergent central sleep apnea with mandibular advancement device therapy.

STUDY OBJECTIVES: Treatment-emergent central sleep apnea (TECSA) describes the appearance or persistence of central sleep apnea while undergoing treatment for obstructive sleep apnea. TECSA is well studied in continuous positive airway pressure therapy with an estimated prevalence of 8%. Based on a few case reports, mandibular advancement devices (MAD) may also provoke TECSA. This study aims to gain insight into the prevalence of TECSA with MAD therapy. METHODS: This retrospective study includes a total of 129 patients with moderate to severe obstructive sleep apnea who were treated with a cu

Resolution of fibromyalgia by controlling obstructive sleep apnea with a mandibular advancement device.

Fibromyalgia (FM) is a chronic, often disabling disorder characterized by multisite pain along with sleep problems and fatigue. Pain and sleep exhibit a reciprocal relationship. When FM and obstructive sleep apnea/hypopnea (OSA) co-exist, treatment options include continuous positive airway pressure or mandibular advancement device. We present a patient experiencing fibromyalgia and OSA whose symptoms vanished wearing a Mandibular Advancement Device (MAD) during sleep. To our knowledge, this is the first documented case of FM symptom resolution by MAD treatment.

Designing a mandibular advancement device with topology optimization for a partially edentulous patient.

STATEMENT OF PROBLEM: Patients with partial tooth loss treated with implant-supported fixed partial dentures (FPDs) have difficulty using conventional mandibular advancement devices (MADs) because of the risk of side effects. Also, which design factors affect biomechanical stability when designing MADs with better stability is unclear. PURPOSE: The purpose of this finite element (FE) analysis study was to analyze the effect of the MAD design on biomechanical behavior and to propose a new design process for improving the stability of MADs. MATERIAL AND METHODS: Each 3D model consisted of the

Mandibular Advancement Device-Emergent Central Sleep Apnea Can Resolve Spontaneously: A Case Report.

The development of treatment emergent central sleep apnea (CSA) has been described after almost all obstructive sleep apnea (OSA) therapies. While the course of positive airway pressure (PAP) emergent CSA, is better established; little is known about the natural course of mandibular advancement device (MAD) emergent CSA. Previous reports failed to comment on its natural course or report treatment with advanced ventilator modes such as adaptive servoventilation. We describe spontaneous resolution of MAD emergent CSA in a patient with moderate OSA who refused PAP. We also highlight the need for

Comparative evaluation of the breaking strength of a simple mobile mandibular advancement splint.

Mandibular advancement splints (MASs) are used to advance the mandible forward in patients with sleep-disordered breathing. The conventional rigid MAS restricts the movement of the mandible, and this immobility sometimes produces discomfort, including temporomandibular disorders. A simple method for fabricating a mobile MAS was devised, using a connector made from a polyethylene toothed belt, with the intention of making the MAS more comfortable. The experimental connector is easily constructed, inexpensive, and small enough for use as an intraoral MAS. To evaluate durability, the axial and di

Mandibular advancement devices: rate of contraindications in 100 consecutive obstructive sleep apnea patients.

Mandibular advancement devices (MAD) should be contraindicated (Clark GT, Sleep Med Rev 1998;2:163-174) if there are: (1) insufficient teeth to support the device, (2) periodontal problems inducing tooth mobility, (3) active temporomandibular joint (TMJ) disorder, and (4) limited maximum protrusive distance (< 6 mm). The aim of the present study was to evaluate the proportion of the obstructive sleep apnea population that exhibits any contraindication (CI) to MAD. For this study there were 100 unselected adult patients consecutively diagnosed by polysomnography in a tertiary sleep laboratory.
