Marginal Mandibular Nerve Mapping Prior to Nonablative Radiofrequency Skin Tightening.

BACKGROUND: Aging skin and increased skin laxity is a prevalent concern of patients. Nonsurgical treatments, such as radiofrequency, are increasing in popularity due to decreased pain, downtime, and scarring. ThermiRF (Thermi, Irving, TX) is a subdermal radiofrequency treatment for tightening skin. When applying radiofrequency treatments to the neck, it is important to avoid ablating the marginal mandibular nerve and causing nerve trauma. OBJECTIVES: The purpose of this study was to locate and record the position of the marginal mandibular nerve in 72 patients undergoing subdermal radiofreque

Surgical management of the facial nerve in craniofacial trauma and long-standing facial paralysis: cadaver study and clinical presentations.

BACKGROUND AND OBJECTIVES: Examination of the extratemporal branches of the facial nerve reveals several branching patterns of the facial nerve, indicating the variability in the course of the nerve. Due to such variance, injury to this nerve often accompanies facial trauma and surgical dissection for the repair of facial bone injuries, and it may result in high morbidity. METHODS AND MATERIALS: A study of 12 fresh cadavers was performed to 1) review the variability in location of the extratemporal branches of the facial nerve, 2) identify the soft tissue injuries in which the facial nerve is

Topographical Landmarks for the Identification of Branches of Mandibular Nerve and Its Surgical Implications: A Cadaveric Study.

Introduction Basic knowledge of anatomy is crucial in providing predictable, safe, and efficacious mandibular anesthesia as the mandibular nerve is vulnerable to injury during dental procedures and other surgical manoeuvers. The lack of availability of the appropriate topographical bony landmarks for the location of the branches of this nerve often accounts for iatrogenic injuries and the failure to obtain adequate local anesthaesia. Hence we aimed to describe the topographical landmarks of the branches of the mandibular nerve and their variations in the infratemporal fossa. Methodology In 16

Facial Nerve Preservation During Giant Mandibular Tumor Surgery.

Marginal mandibular branch of facial nerve is easy to be ignored and injured in patients undergoing giant mandibular tumor resections. In this article, a 30-year-old patient with a giant tumor in the left mandible, an operation of segmental mandibulectomy and reconstruction was given, and the surgery was assisted by intraoperative facial nerve monitoring. During surgery, the marginal mandibular branch of the facial nerve was detected and well preserved. The patient showed no facial function injury postoperation. In patients of giant mandibular tumor resection, the anatomic location of the faci

Unusual facial pain secondary to inferior alveolar nerve compression caused by impacted mandibular second molar.

Symptoms of inferior alveolar nerve (IAN) compression are reported during endodontic procedures, placement of implants, third molar surgeries, inferior alveolar nerve block injections, trauma, orthognathic injuries, ablative surgeries or use of medicaments. Presented is a rare case of a 15-year-old girl who reported severe pain in relation to an impacted permanent mandibular left second molar, the roots of which had entrapped the mandibular canal causing compression of IAN. Timely surgical intervention and sectional removal of the impacted molar is indicated to relieve the symptoms and avoid p

A prospective study of the incidence of injury to the cranial nerves during carotid endarterectomy.

In order to determine the incidence of cranial nerve injury during carotid endarterectomy, preoperative and postoperative otolaryngologic examinations were performed in a prospective series of 240 operations. Thirty patients experienced a total of 38 cranial nerve injuries, involving the recurrent laryngeal nerve in 14, the hypoglossal nerve in 13, the marginal mandibular nerve in six and the superior laryngeal nerve in five. Postoperative dysfunction affected one cranial nerve in 25 patients, two nerves in four and three nerves in one patient. Most cranial nerve injuries were asymptomatic or

Influence of the tooth section technique in alveolar nerve damage after surgery of impacted lower third molars.

This study is a randomized control split-mouth non-blinded prospective trial, the aim of which was to evaluate clinically the frequency and type of injury to the inferior alveolar nerve (IAN) following mandibular third molar surgery using or not using the tooth section technique. The sample consisted of 50 lower third molars in 25 patients, in the control group the tooth section technique was not used, and it was used in the experimental group. The outcomes have shown a frequency of 8% of IAN injury for both groups (in both genders), and there were no statistically significant associations bet

Post-extraction inferior alveolar nerve neurosensory disturbances--a guide to their evaluation and practical management.

Inferior alveolar nerve injuries are a recognised complication of mandibular third molar extractions. This paper describes the different types of nerve injuries that may occur. A differential of possible causes is provided and an approach to the immediate and follow-up management is outlined. The prognosis of such injuries is reviewed so that patients can be informed of the possible postoperative outcome. The algorithm shows the timeline for monitoring/referring and the included tables outline the advantages and disadvantages of surgery versus watchful waiting.

[Exposure of distal internal carotid artery through mandibular vertical ramus osteotomy].

Exposure of the distal internal carotid artery at the level of the second cervical vertebra required manoeuvers such as division of digastric muscle or mandibular subluxation. These increase the exposure but may not provide adequate access and are associated with significant cranial nerves or temporal mandibular joint complications. Vertical Ramus Osteotomy (VRO) provided access of the internal carotid artery (ICA) up to the base of the skull, with low incidence of cranial nerve injury temporo-mandibular joint (TMJ) pain and no preincision preparation. We report two cases in which vertical div

Simultaneous bilateral facial palsy as a result of isolated mandibular fractures.

Post-traumatic bilateral facial nerve palsy is usually associated with fracture of the temporal bones in patients who have sustained a significant head injury. Facial nerve injury can also very rarely occur because of isolated mandibular fractures, without associated temporal bone fractures. Here is reported a case of bilateral facial nerve palsy in a patient with mandibular symphysis and bilateral condylar fractures of the mandible without evidence of temporal bone fracture or significant head injury.

Inferior alveolar nerve injury caused by thermoplastic gutta-percha overextension.

Injuries to the inferior alveolar nerve following trauma resulting in a mandibular fracture are well documented and are a well-known risk when surgical procedures are planned for the mandible in the region of the inferior alveolar canal. Such injuries are relatively rare following endodontic therapy. This article reports a case of combined thermal and pressure injury to the inferior alveolar nerve, reviews the pathogenesis of such an injury and makes suggestions for its management.

Modified Mandibular Sagittal Split Osteotomy.

INTRODUCTION: Mandibular osteotomies aim to displace the dental arch to the necessary position, ideally without limitation, while preserving inferior alveolar nerve (IAN) function. Supraforaminal osteotomies offer nerve safety but limit the extent of advancement, whereas Epker and Obwegeser-Dal Pont osteotomies enable unchallenged mandibular advancement but are associated with an inferior border notch. Here, we describe a new technique to avoid such disadvantages. TECHNICAL NOTE: The beginning of the procedure was similar to Epker's technique, with sectioning of the lingual cortex up to the l

Facial nerve paralysis and frey syndrome in an infant following removal of an internal mandibular distraction device.

Mandibular distraction using an implantable device has become a widely accepted and utilized procedure for the treatment of retrognathia. Although excellent results have been reported and observed with distraction osteogenesis, complications such as facial nerve injury have been previously reported. Often, this injury is usually temporary and corrects over the course of time. Frey syndrome has been classically described as an injury or severance of the auricotemporal branch of the trigeminal nerve. It is commonly seen as a complication of parotid surgery and has never been reported in associat

Discerning a smile - The intricacies of analysis of post-neck dissection asymmetry.

INTRODUCTION: Iatrogenic facial nerve palsy is distressing to the patient and clinician. The deformity is aesthetically displeasing, and can be functionality problematic for oral competence, dental lip trauma and speech. Furthermore such injuries have litigation implications. Marginal mandibular nerve (MMN) palsy causes an obvious asymmetrical smile. MMN is at particular risk during procedures such as rhytidoplasties, mandibular fracture, tumour resection and neck dissections. Cited causes for the high incidence are large anatomical variations, unreliable landmarks, an exposed neural course an

Complete recovery from chronic implant-induced paresthesia after implant removal: a case report.

Inferior alveolar nerve (IAN) injury is one of the most significant adverse outcomes of dental implantation. IAN injury can occur because of direct invasion of the mandibular canal, compression from implant placement, or postoperative hematoma formation. These injuries may result in prolonged neurosensory disturbances, and the prognosis largely depends on how quickly treatment is initiated. A 71-year-old male presented with persistent hypoesthesia, pain, and tinnitus in the left mandibular region of 6 years duration after implant placement. Clinical and radiographic examinations revealed peri-

Bilateral microneurosurgical reconstruction of inferior alveolar nerves via autogenous sural nerve transplantation.

Microneurosurgical operative techniques permit satisfactory restoration of sensation in many lesions of the inferior alveolar nerve. Therefore, restoration of the sensory deficit is becoming increasingly more important in the total functional rehabilitation of individuals with mandibular continuity defects involving transection of or permanent damage to the inferior alveolar nerve. This article reviews the case history of a young man who underwent bilateral osseous mandibular reconstruction and microneurosurgical reconstruction of his inferior alveolar nerves following severe maxillofacial tra

Unerupted teeth associated with dentigerous cysts and treated with coronectomy: mini case series.

There is a resurgence of interest in coronectomy, but its appropriate application in the management of third molar teeth has yet to be defined. Dentigerous cysts associated with unerupted teeth are most commonly associated with mandibular third molars. In this case series we evaluate outcome after coronectomy of teeth with associated dentigerous cysts in cases where the inferior dental nerve was thought to be at risk, or there was an increased risk of mandibular fracture. We retrospectively studied 21 consecutive patients treated by coronectomy for dentigerous cysts at the Oral Surgery Departm
