Sagittal split osteotomy of the mandibular ramus is performed in close proximity to the inferior alveolar nerve and may result in postoperative neurosensory disturbances. Intraoperative strain on the nerve and other complications in 25 patients undergoing bilateral sagittal split osteotomy were recorded. Neurosensory testing was carried out before and after surgery. The patients reported sensory disturbances in 54% of sites 4 days postoperatively and 42% and 34% of sites at 9 weeks and 6 months, respectively. Objective assessments showed an incidence of 34% at 4 days, 20% at 9 weeks, and 8% at

A cephalometric analysis of the positional changes of the mandible and the upper and lower incisors following vertical subcondylar ramus osteotomy was performed on 80 patients. The patients were prognathic without laterognathism or open bite and all the patients had preoperative orthodontic treatment. 40 patients were operated with an intraoral (IVSO) and 40 with an extraoral approach (EVSO). Both groups showed postoperatively posterior rotation of the mandible, shortening of the posterior facial height, increase in anterior facial height and extrusion of the incisors in both jaws. A significa

Patients whose facial esthetics are not severely compromised but who need surgical intervention for the correction of a dentoskeletal problem can be treated by repositioning of the whole mandibular alveolar segment in the direction needed to allow for such correction. The challenge to achieve efficient and relatively stable results with the use of combined orthodontic and surgical methods has been met by the use of various surgical techniques. The lower total alveolar osteotomy is another viable surgical technique that could be considered when treatment is being planned in an orthognathic surg

Mandibular osteotomies are known to widen the mandibular angles by increasing the bigonial distance (BGD). Previous studies have highlighted the relationship between the extent of mandibular movement and the increase in BGD, both for cutaneous bigonial distance (CBGD) and osseous bigonial distance (OBGD). However, the relationship between changes in OBGD, CBGD and mandibular advancement remains to be fully established. The aim of this study was to compare changes in OBGD and CBGD using adapted three-dimensional examination techniques in patients who underwent mandibular osteotomies. Preoperati

Background/Objectives: Variation in the mandibular canal and surrounding bone may affect surgical risk during sagittal split ramus osteotomy (SSRO). This study examined whether canal position and bone morphology at SSRO-relevant sites differ according to mandibular sagittal position. Methods: This retrospective cone-beam computed tomography study included 72 adults classified as retrognathic, normal, or prognathic according to the SNB angle. Bilateral measurements from 144 mandibular canals were obtained at the mandibular foramen entrance, ramus-corpus transition, and mesial aspect of the seco

PURPOSE: The anatomical position of the lingula is clinically very important to prevent injuries during sagittal split ramus osteotomy. Our study aims to evaluate the localisation of the lingula by cone beam computed tomography (CBCT) and to compare the localisation of the lingula between malocclusion, gender, and lingula types.
METHODS: A retrospective study was conducted to evaluate the shape and location of the lingula using CBCT. A total of 250 CBCT images were included in this study. The lingula was classified as nodular, assimilated, truncated, or triangular type. Six defined distances

Bimaxillary transverse width discrepancies are commonly encountered among patients with dentofacial deformities. Skeletal discrepancies should be diagnosed and managed appropriately with possible surgical corrections. Transverse width deficiencies can present in varieties of combinations involving the maxilla and mandible. We observed that in a significant proportion of cases, the maxilla is normal, and the mandible showed deficiency in the transverse dimension after pre-surgical orthodontics. We designed novel osteotomy techniques to enhance mandibular transverse width correction, as well as

PURPOSE: Class II dento-facial deformity due to inferior alveolar retrusion may be difficult to treat using conventional surgical procedures. The aim of this study was to report on total mandibular subapical osteotomy (TMSO) to correct inferior alveolar retrusion, to describe its indications, the surgical technique and to analyse the aesthetic, occlusal changes and stability.
MATERIALS AND METHODS: Patients treated with TMSO from January 2004 to December 2014 were included and analysed retrospectively. Delaire's cephalometric analysis was performed before surgery and one year after surgery. S

INTRODUCTION: Patients sometimes spontaneously report a modification of the width of their lower face after an advancement bilateral sagittal split osteotomy (ABSSO). The main goal of our study was to assess the variation of the bigonial distance (BGD) before and after ABSSO in a group of patients. The second goal was to look for a possible relation between the variation of BGD and the amount of mandibular advancement.
MATERIALS AND METHODS: We conducted a retrospective radiological study on patients who underwent an isolated ABSSO (Obwegeser-Dal Pont II type osteotomy) for a class II maloccl

We successfully treated a 32-year-old woman who had facial asymmetry and unilateral mandibular condylar osteochondroma using ipsilateral mandibular condylectomy and contralateral ramus osteotomy. Mirror image analysis with a noncontact 3-dimensional image scanner showed that the soft tissue on the deviated side was protruded more than 5.50 mm compared with the nondeviated side. The patient was diagnosed as having facial asymmetry with a skeletal Class III jaw-base relationship caused by unilateral mandibular condylar osteochondroma. After 18 months of preoperative orthodontic treatment, an ips

Aneurysmal bone cyst is a benign pseudocystic osseous lesion characterized by a fibrous connective tissue stroma with cellular fibrous tissue, multinucleated giant cells, and large blood-filled spaces with no endothelial lining. The entity is uncommon in facial bones, and it rarely involves the mandibular condyle. Resection of the lesion is the most accepted treatment. The present case is the 11th reported case of aneurysmal bone cyst of the mandibular condyle in the existing literature and the first where, rather than using conventional extra oral approach, sagittal split ramus osteotomy was

Temporomandibular joint (TMJ) ankylosis (TMJA) is an anatomic and functional alteration of the TMJ surfaces, caused by the fusion of these surfaces by either bone or fibrous tissue. Several techniques are used for the treatment of TMJA. The following case report describes a 5-year-old boy who was diagnosed with TMJA. The treatment of choice is reconstruction of the condyle by sliding the posterior border of the mandibular ramus. Temporomandibular joint ankylosis treatment with vertical ramus osteotomy and mandibular posterior border repositioning offers minimizing the reduction in height or sh

In order to evaluate the risk of nerve injury and to prevent iatrogenic damage at different stages of bilateral sagittal-split osteotomy (BSSO) of the mandible, we monitored the function of the inferior alveolar nerve (IAN) continuously on both sides in 13 orthognathic patients undergoing BSSO. The IAN was stimulated at the mental foramen with two monopolar needle electrodes fixed to the dental splint, and the orthodromic sensory nerve action potentials (SNAP) of the IAN were recorded with a silver-wire electrode inserted near the oval foramen on each side. The latencies, amplitudes, and senso

Arthroscopic surgery is an effective method for treating painful hypomobility of the temporomandibular joint. Decreased range of motion after sagittal ramus osteotomies of the mandible has been reported. Causes may include intra-articular factors. These cases may be effectively treated with arthroscopic lysis and lavage after failure of nonsurgical modalities. A series of 15 patients is presented.

OBJECTIVE: The aim of this study was to elucidate the physiological position of the proximal segment for postoperative jaw movement in patients with mandibular prognathism.
METHODS: Twenty-two patients with mandibular prognathism were treated by orthognathic surgery using bilateral mandibular sagittal split ramus osteotomies (SSRO) with a physiological positioning strategy. The skeletal stability was assessed, and the movement of the proximal segment was evaluated by cephalography and computed tomography performed preoperatively, immediately postoperatively, and one year postoperatively.

Recent advances in orthognatic surgery have been remarkable. Among the surgical procedures, sagital ramus osteotomy has been often used for mandibular protrusion. Causes of dentofacial deformities include not only congenital but also acquired ones, which are caused by trauma or resection of tumor in the maxillofacial region. In all cases, functional and psychological disorders would often occur in the patient with these dentofacial deformities. We report a favorable result of anterior alveolar osteotomy of the mandible, which was applied to a patient complaining of malocculusion and facial def
