Initially Normal MRI, Delayed Splenial Lesion in Marchiafava-Bignami Disease.

Marchiafava-Bignami disease (MBD) may show normal findings on early MRI. In patients with alcohol use disorder or risk of thiamine deficiency, repeat imaging is important because splenial lesions can develop later. Recognizing this pattern is key to avoiding delayed diagnosis.

Marchiafava-Bignami disease with typical imaging findings: A case report.

Marchiafava-Bignami disease is a rare neurological condition characterized by necrosis and demyelination of the corpus callosum, typically associated with chronic alcoholism and/or malnutrition. The clinical manifestations of Marchiafava-Bignami disease are diverse and often nonspecific. Diagnosis of Marchiafava-Bignami disease relies on magnetic resonance imaging findings, which reveal significant and symmetrical involvement of the corpus callosum. We report the case of a 48-year-old man with chronic alcoholism who has been experiencing symptoms of confusion, stupor, difficulties in using and

Marchiafava-Bignami disease with hyperintensity on late diffusion-weighted imaging.

A 69-year-old man with a decades-long history of chronic alcohol consumption was admitted with gait disturbance (short steps and spasticity), deterioration of activity, and stuporous consciousness. Head magnetic resonance imaging (MRI) revealed hyperintensity on fluid-attenuated inversion recovery imaging in the corpus callosum and frontal white matter. The lesion later became more apparent on diffusion-weighted imaging. The clinical diagnosis was Marchiafava-Bignami disease (MBD). As temporary treatment, refraining from alcohol consumption and administration of vitamins were prescribed. The c

Paucisymptomatic Marchiafava-Bignami disease with relevant diffusion-weighted MRI lesions.

Marchiafava-Bignami disease (MBD) is a rare alcohol-associated disorder characterized by demyelization and necrosis of the corpus callosum. Diffusion-weighted (DW) MRI may reveal the lesions in patients showing serious clinical manifestations, high mortality rate and severe cognitive sequelae, though some cases with good outcome have been reported. We describe a case of a man with a history of chronic alcohol abuse associated with malabsorption; the man presented mild clinical signs on the first neurological exam, despite the presence of DW-MRI lesions, bilaterally involving entire corpus call

Acute Marchiafava-Bignami disease with widespread callosal and cortical lesions.

Marchiafava-Bignami disease (MBD) is a rare alcohol-related disorder that results in progressive demyelination and necrosis of the corpus callosum. The process may extend to the optic chiasm and tracts, cerebellar peduncle, subcortical resion, neighboring white matter, and rarely, cortical gray matter. We report a case of MBD in which fluid-attenuated inversion recovery and diffusion magnetic resonance imaging studies revealed symmetrical hyperintense lesions in the cerebral cortex in addition to the callosal lesions.

Marchiafava-Bignami disease: serial changes in corpus callosum on MRI.

Serial MRI findings of changes in corpus callosum lesions in two cases of Marchiafava-Bignami disease are presented. In both, MRI displayed diffuse swelling of the corpus callosum in the acute stage, thought to represent oedema and demyelination. In the chronic stage, in addition to atrophy of the corpus callosum with presumed focal necrosis, previously undescribed focal hypointensity on T2-weighted images, of unknown cause, was observed in the corpus callosum.

Marchiafava-Bignami disease. A correlative computed tomography and morphological study.

The subacute development of Marchiafava-Bignami disease (MBD) is reported in a 60-year-old patient from Southern Germany with a history of chronic alcoholism and severe malnutrition. Computed tomography scans showed progressive white matter destruction over a period of 8 months, ranging from diffuse hypodensities to well-delineated, strongly hypodense areas in the central hemispheric white matter of both frontal and parietal lobes and in the corpus callosum. Autopsy revealed an identical pattern of demyelinated, partially necrotic lesions in addition to small cystic necroses in the genu corpor

Altered Sensorium in a Young Male: Marchiafava Bignami Disease Revisited.

Marchiafava Bignami disease (MBD) is a rare and devastating complication of chronic alcoholism. Degeneration of the corpus callosum is the hallmark feature of MBD. Early diagnosis of MBD by its typical "Sandwich Sign" on magnetic resonance imaging (MRI). Prompt institution of treatment and strict alcohol abstinence can cause regression of changes and hence, clinical improvement. Here we report the case of a young chronic alcoholic male admitted with altered sensorium and his further course in ward.

Diffusion tensor imaging and fiber-tracking in Marchiafava-Bignami disease.

Marchiafava-Bignami disease, a rare disorder most commonly seen in patients with a history of alcohol consumption, involves demyelination and subsequent necrosis of the corpus callosum. Diffusion tensor imaging demonstrates regional abnormalities in the corpus callosum that are not evident by conventional magnetic resonance imaging (MRI). Furthermore, fiber-tracking demonstrates significant disruption of axonal fiber bundles within the corpus callosum, most marked within the body, corresponding to the clinical finding of interhemispheric disconnection, which is characteristic of the disease.

Marchiafava-Bignami disease: longitudinal MR imaging and MR spectroscopy study.

A case of Marchiafava-Bignami disease was serially evaluated with MR imaging and MR spectroscopy at 1, 2, 4, and 11 months after the onset of symptoms. The first MR imaging study showed extensive abnormal signal intensity of the corpus callosum without macroscopic changes; a diagnosis of Marchiafava-Bignami disease was made, and vitamin therapy was initiated. Follow-up studies showed progressive reduction of signal intensity abnormalities and residual callosal atrophy. MR spectroscopy revealed progressive reduction of the N-acetylaspartate:creatine ratio, with partial recovery in the last stud

Marchiafava-Bignami disease with widespread extracallosal lesions and favourable course.

We report a 61-year-old alcoholic man who presented with subacute physical deterioration and severe dysarthria. MRI, suggestive of corpus callosum demyelination with associated white matter involvement in both cerebral hemispheres, indicated the diagnosis of Marchiafava-Bignami disease. During his stay in hospital the patient showed remarkable improvement, and was discharged 22 days after admission. On MRI 2 months later, the extracallosal lesions had disappeared. This case raises questions about some previous ideas on this disease, such as the prognosis of its acute forms and the significance

[Marchiafava-Bignami disease: interhemispheric disconnection, Balint syndrome, spontaneously favourable outcome].

We report a new case of Marchiafava-Bignami disease with favourable outcome. This case is particular on several scores. The interhemispheric disconnection syndrome was associated with Balint's syndrome, and this had apparently never been reported in this context previously. Despite the intensity and diffusion of white matter lesions in both hemispheres, and in the absence of vitamin treatment, the clinical and neuroradiological (CT, MRI) improvement occurred early and rapidly. This course, similar to that of toxic drug withdrawal, suggested that this was not a process of vitamin deficiency. Da

[Marchiafava-Bignami disease: an alcoholic with reduced consciousness and muscle paratonia].

A 69-year-old man presented with decreased consciousness and a puzzling muscle paratonia. MRI showed extensive white matter abnormalities with diffusion restriction in the corpus callosum, consistent with Marchiafava-Bignami disease, a rare complication of chronic alcohol abuse. The phenomenon of paratonia should have raised a clinical suspicion towards endstage neurodegenerative disease.

Marchiafava-Bignami Disease: A Case Report of a Reversible Cause of Dementia.

Marchiafava-Bignami disease (MBD) is a rare neurological disorder predominantly associated with chronic alcohol use, characterized by demyelination and necrosis of the corpus callosum. The condition often presents with cognitive dysfunction, motor deficits, and altered consciousness, which can range from mild confusion to severe stupor. Early recognition and treatment are crucial for improving outcomes. We report a case of MDB that was diagnosed in a 43-year-old woman with a history of chronic alcohol use and a 15-day history of progressive neurological symptoms, including impaired speech, con

Mutism Caused by Severe Demyelination in a Patient With Marchiafava-Bignami Disease.

BACKGROUND: Marchiafava-Bignami (MB) disease is a rare disorder that causes primary degeneration of the corpus callosum. It is associated with chronic alcohol consumption caused by either a toxic or nutritional etiology. CASE REPORT: We report a case of a 54-year-old woman who presented to our emergency department with complete mutism caused by MB disease that completely resolved with intravenous thiamine and dextrose therapy. WHY SHOULD AN EMERGENCY PHYSICIAN BE AWARE OF THIS?: Emergency physicians frequently encounter patients with alcohol abuse and its complications. We report a rare prese
