Maternal age and parity as predictors of human twinning.

A case-control study was conducted to evaluate the association between maternal age and parity and dizygotic (DZ) and monozygotic (MZ) twinning, respectively. Mothers of all twins born alive in Denmark in 1984 or 1985 were included into the study as cases, and a random sample of mothers of singletons were controls. Data on maternal age at delivery and number of previous births were collected from the Danish Medical Birth Register. By means of logistic regression analysis, a significant and positive association was found between maternal age and DZ twinning, and a significant and negative assoc

Maternal serum alpha-fetoprotein, age, and Down syndrome risk.

Before the discovery that an association existed between low maternal serum alpha-fetoprotein levels and Down syndrome, the ability to detect Down syndrome pregnancies was limited to women aged greater than or equal to 35, whose individual risks were sufficiently high (greater than or equal to 1:270 in the second trimester) to justify offering amniocentesis. If such women were to opt for that procedure, about 20% of all cases of Down syndrome could be detected. It is now possible to identify an additional 20% of all Down syndrome pregnancies in women under age 35, with the use of a screening p

Maternal age, obstetric complications, and the outcome of pregnancy.

Data from 44,386 pregnancies were analyzed to determine if advancing maternal age influences frequency or outcome of antenatal disorders. The perinatal mortality rate progressively increased from 25/1000 at age 17 ro 19 years to 69/1000 after age 39. Stillbirths accounted for 92% of this increase. Fourteen percent of the increase was due to congenital malformations and 50% to disorders associated with uteroplacental underperfusion, ie, abruptio placentae, large placental infarcts, and placental growth retardation. Sclerotic lesions in the myometrial arteries are a possible cause of underperfus

Maternal hyperuricemia superimposed on maternal hypertension aggravates the risk of small-for-gestational-age fetus.

AIMS: Small-for-gestational-age (SGA) fetus is an important public health issue because of its high mortality and long-term effects on health. Maternal hyperuricemia is associated with diverse adverse pregnant outcomes and neonatal disturbance. We aimed to evaluate whether maternal hyper-uric acid (HUA) is associated with the risk of SGA fetus and to explore whether it can modify the association between maternal hyper-blood pressure (HBP) and SGA fetus. MATERIALS AND METHODS: We performed a population-based cross-section retrospective study, a total of 6715 pregnant females were recruited. Mu

Advanced maternal age and infertility: The role of biological aging, lifestyle, and biomarkers.

Infertility in women of advanced maternal age is often attributed to chronological aging, yet the contribution of biological aging and immunometabolic dysregulation remains unclear. This cross-sectional study analyzed nationally representative data from the 2021-2023 National Health and Nutrition Examination Survey, including 895 women aged 35-49 years with complete reproductive, lifestyle, and biomarker data. Self-reported infertility was defined as a history of attempting pregnancy for at least one year without success. Biological age was estimated using standardized metabolic and inflammato

Impact of aging on obstetric outcomes: defining advanced maternal age in Barcelona.

BACKGROUND: Women of advanced maternal age (AMA) are a growing population, with higher obstetric risks. The Mediterranean population has specific characteristics different from other areas. Thus, the objective of this study was to establish a cut-off to define AMA in a selected mediterranean population coming from a tertiary referral private/mutual health hospital in Barcelona. METHODS: Retrospective cohort of euploid singleton pregnancies delivered from January 2007 to June 2017. Main maternal outcomes were: gestational diabetes, preeclampsia, placenta previa, c-section and prolonged hospita

The impact of maternal age on pregnancy and its outcome.

There were 28,600 deliveries of 500 g or more to women at the Rotunda Hospital between January 1st 1985 and December 1st 1989. Of these, 595 were to women aged 40 years and over. Thirty-five variables of clinical significance were analyzed, comparing those of 40 years of age and more with those under 40. The older group had significant increases in gestational diabetes, ante-partum hemorrhage, fetal distress, prematurity, low birth weight and perinatal mortality. Chromosome congenital abnormalities were significantly higher, particularly Down syndrome. There were significantly increased rates

Impact of advanced maternal age on the outcome of pregnancy.

We assessed the impact of advanced maternal age on the outcome of pregnancy by studying all 1,328 women who were primarily cared for and delivered at our institution between 14 September 1984 and 12 February 1985. Important peripartum maternal complications were no more frequent in women aged 35 years or more than in women 20 to 34 years old, although operative delivery was significantly more common. Similarly, adverse outcomes of infants were no more frequent. Perinatal mortality tended to be lower. In addition, we noted a trend for fewer infants with congenital anomalies to be born among old

Association Between Obesity Severity and Peripartum Maternal and Neonatal Outcomes Among Advanced Maternal Age Pregnant Women.

Background Obesity and advanced maternal age are both associated with adverse maternal and neonatal outcomes. However, the effect of increasing obesity severity within an advanced maternal age population remains insufficiently characterized, particularly in regional cohorts. Objective The objective of this study is to investigate the association between obesity severity and peripartum maternal and neonatal outcomes among pregnant women of advanced maternal age. Methods This retrospective cohort study was conducted at the Specialized Hospital for Women's Health, Ministry of National Guard Healt

Is age at menarche a predictor of maternal birth trauma?

OBJECTIVE: Age at menarche and the interval between menarche and age at first vaginal birth have been shown to be associated with reproductive performance; however, their association with maternal birth trauma has not been studied. We aimed to determine whether age at menarche, age at first vaginal birth and their interval are associated independently with levator ani muscle (LAM) avulsion and external anal sphincter (EAS) defect. METHODS: This was a retrospective analysis of the ultrasound volume datasets of 466 vaginally parous patients attending one of two tertiary urogynecological units i

Questioning the construction of maternal age as a fertility problem.

The belief that fertility problems derive from maternal age, increasing markedly at 35, reflects social constructions of biology in developed nations. These constructions perpetuate a negative view of female aging. However, research since 1985 can be interpreted to suggest that there is no, or minimal, association between maternal age and problems associated with fertility. Differences in problems between pre- and postmaternal age 35 fertility can be explained by social conditions occurring with fertility, notably, parenting decisions, physical problems with the male and the potential child, a

Rates of chromosome abnormalities at different maternal ages.

Estimates are made of the rates of all clinically significant cytogenetic abnormalities in live births by 1-year maternal age intervals ranging from 15 to 49. Down syndrome results from the most prevalent clinically significant cytogenetic abnormality, yet the rates of disorders associated with other cytogenetic abnormalities also increase with maternal age and significantly contribute to the overall rate at the older ages. The estimated rate of all clinically significant cytogenetic abnormalities rises from about 2 per 1000 (1 per 500) at the youngest maternal ages to about 2.6 per 1000 (1 pe

Pregnancy and advanced maternal age-The associations between regular exercise and maternal and newborn health variables.

INTRODUCTION: Despite the associations between delayed childbearing and poorer maternal and perinatal outcomes, little is known about these issues in regular exercisers and in women with healthy lifestyles. The aims of the present study were to: (a) compare lifestyle variables and exercise, pregnancy and birth outcomes in women ≥35 years and women <35 years of age, and (b) investigate the associations between regular exercise and maternal health and newborn variables in women of advanced maternal age. MATERIAL AND METHODS: Healthy pregnant women (≥35 years, n = 104 and <35 years, n = 362) wer

Maternal serum alpha-fetoprotein screening and fetal chromosome anomalies: is lowering maternal age for amniocentesis preferable?

We have compared the cytogenetic abnormalities diagnosed prenatally in 1,098 patients referred for amniocentesis because of low maternal serum alpha-fetoprotein (MSAFP) to those of 445 patients whose indication was elevated MSAFP and those of 361 patients who had amniocentesis for "maternal anxiety." Autosomal trisomies, sex chromosome aberrations, and various structural rearrangements were detected in all 3 groups and actually exceeded the age-related incidence estimates. The frequency of chromosome anomalies in cases studied because of "maternal anxiety" with no prior screening was similar t

Should PGT-A be routinely utilized for advanced maternal age?

Preimplantation genetic testing for aneuploidy (PGT-A) is often presented to women of advanced maternal age as a logical safeguard against miscarriage and chromosomal abnormality. Yet intuitive rationale and biological plausibility do not establish net clinical benefit. PGT-A does not improve oocyte or embryo quality; it ranks embryos that already exist. The appropriate objective endpoint is therefore cumulative live birth from an initiated treatment strategy, not implantation after transfer of a selected euploid embryo. Randomized clinical trials, contemporary systematic reviews, meta-analyse

Longitudinal maternal glycemia during pregnancy and placental epigenetic age acceleration.

BACKGROUND: Dysregulation of maternal glucose homeostasis has been related to an increased risk of morbidity and mortality in mothers and fetuses, yet the mechanism remains unclear. This study investigated the association between maternal glycemic levels and placental epigenetic age acceleration (PAA) in a multiethnic cohort. METHODS: In a sample of 301 pregnant women (102 Hispanic, 77 White, 72 Black, and 50 Asian/Pacific Islander), the association of glycemic markers cumulative exposure with PAA was tested using linear regression adjusting for fetal sex, maternal age, educational status, an

Maternal Urinary Fluoride and Child Neurobehavior at Age 36 Months.

IMPORTANCE: Recent studies in Canadian and Mexican populations suggest an association of higher prenatal fluoride exposure with poorer neurobehavioral development, but whether this association holds for US-based populations is unknown. OBJECTIVE: To examine associations of third trimester maternal urinary fluoride (MUF) with child neurobehavior at age 3 years in the US. DESIGN, SETTING, AND PARTICIPANTS: This prospective cohort study utilized urine samples archived from 2017 to 2020 and neurobehavioral data assessed from 2020 to 2023 from the Maternal and Developmental Risks from Environment

Unplanned cesarean sections in advanced maternal age: A predictive model.

INTRODUCTION: As maternal age during pregnancy is rising all over the world, there is a growing need for prognostic factors that determine maternal and perinatal outcomes in older women. MATERIAL AND METHODS: This study is a retrospective cohort study of women aged 40 years or older at the time of delivery in four Santeon hospitals across the Netherlands between January 2016 and December 2019. Outcomes were compared between women of 40-44 years (advanced maternal age) and 45 years and older (very advanced maternal age). Primary outcome was unplanned cesarean section, secondary outcomes includ
