Endoscopic surgery of the oesophagus.

Conventional surgical access to the oesophagus often requires a thoracotomy. The application of techniques borrowed from laparoscopic surgery allows transthoracic dissection of oesophagus without thoracotomy. These techniques are useful in treatment of oesophageal leiomyomas, motility disorders and iatrogenic perforations. Oesophageal resection for cancer can be accomplished using minimal access, either thoracoscopically or with the aid of an operating mediastinoscope. Whether these techniques should replace transthoracic or transhiatal oesophagectomy for most patients with cancer remains to b

[Value of noninvasive and intraoperative diagnosis of malignant mediastinal tumors].

Among malignant tumors of the mediastinum lymphomas are no indication for operation. In our experience, however, fine needle puncture, computed tomography (CT), nuclear magnetic resonance, mediastinoscopic biopsy of lymph nodes and intraoperative freeze section histology can only rarely differentiate between lymphomas and other tumors of the mediastinum. Only CT-guided or direct parasternal biopsy including immunhistochemistry can yield a reliable preoperative diagnosis and prevent unnecessary operation of lymphomas.

Technical details of video-assisted transcervical mediastinal dissection for esophageal cancer and its perioperative outcome.

To reduce pulmonary complications after esophagectomy, the transthoracic procedure should be shortened or totally avoided. Transcervical approach assisted by mediastinoscope for the upper mediastinum may be advantageous for this purpose. We carried out video-assisted transcervical mediastinal dissection (VATCMD) as part of totally non-transthoracic radical esophagectomy. A single-port laparoscopy device was adopted to a small cervical incision and the mediastinum was inflated with a positive pressure of 6 to 10 mmHg. Without assistant's retractor, the upper mediastinum and partially the middle

Combined videothoracoscopic and videomediastinoscopic approach improves radicality of minimally invasive mediastinal lymphadenectomy for early stage lung carcinoma.

OBJECTIVES: To assess the feasibility and radicality of a combined thoracoscopic and mediastinoscopic approach to mediastinal lymphadenectomy compared to thoracoscopy only for minimally invasive management of early stage lung carcinoma. METHODS: Prospective observational study of patients undergoing anatomical thoracoscopic lung resection for lung carcinoma in our department in 2007. Mediastinal lymphadenectomy was performed either thoracoscopically (VATS group) or by a combination of video-assisted mediastinoscopic lymphadenectomy (VAMLA) and thoracoscopy (VAMLA+VATS group). Inclusion criter

Single-incision surgery with SILS port for anterior mediastinal lesions: initial experience.

Infrasternal mediastinoscopic single-incision surgery is a minimally invasive surgical option for patients with anterior mediastinal lesions. The main drawback of the procedure is difficult when handling the endoscopic instruments through the small infrasternal incision. Herein, we report our initial experience with use of the procedure in 2 patients with bronchogenic cysts in the anterior mediastinum. We found that use of an SILS Port, now widely available for single-incision laparoscopic surgery, can decrease the difficulty of this approach.

Hyphalike pseudofungus in a lymph node.

Mediastinoscopic biopsy specimens of multiple paratracheal lymph nodes in a 66-year-old man with a history of resected pulmonary carcinoma were submitted for histologic evaluation. Structures present in one of these lymph nodes superficially resembled branching septate fungal hyphae. Perls' stain demonstrated that these structures contained iron, and Gomori's methenamine silver stain was negative. The histopathologic features are similar to those of the first and only previous case report of such structures.

Optimizing the minimally invasive approach to mediastinal parathyroid adenomas.

BACKGROUND: Patients with refractory hyperparathyroidism after neck exploration may have a mediastinal parathyroid gland that has not been identified reliably with a single radiologic study. We report 17 patients who underwent minimally invasive resection for mediastinal parathyroid adenomas after confirmatory multipoint radiologic imaging. METHODS: Fifteen patients underwent thoracoscopic procedures and 2 patients underwent mediastinoscopic procedures for resection of suspected mediastinal parathyroid adenoma. Preoperative localizing studies included sestamibi scan, computed tomography scan

Massive parathyroid hemorrhage in a case of water-clear cell hyperplasia.

A case of water-clear cell hyperplasia of the parathyroid glands is reported. As far as we know this case is unique in respect of the following features. All the parathyroids were located in the superior mediastinum. One abnormal parathyroid gland was removed with the help of a mediastinoscope. The patient survived a massive parathyroid hemorrhage. This case draws attention to the facts that enlarged parathyroid glands can appear as mediastinal tumours and that massive bleeding can occur in abnormal parathyroid glands.

[Closure of bronchopleural fistula by video-assisted mediastinal surgery after pneumonectomy].

Disruption of a mainstream bronchus is a rare but dreaded complication of pneumonectomy. When the bronchial tump measures at least 15 mm, the conventional therapeutic strategy is to drain the pleural cavity followed by closure of the fistula via trans-mediastinal sternotomy. After an experimental study on cadavers to test the technical feasibility of main bronchus closure via a cervical approach using a video-mediastinoscope, we used video-assisted mediastinal surgery successfully in a patient with a left main bronchus fistula.

Lymph node metastases identified with mediastinoscopy in a patient with superficial carcinoma of the esophagus.

Superficial esophageal cancers limited to the lamina propria are not associated with lymph node metastases. Mediastinoscopic transhiatal esophagectomy was planned in a patient with widespread superficial cancer of the midthoracic esophagus. Sampling of the upper mediastinal lymph nodes revealed metastases. The operation was converted to a transthoracic esophagectomy with radical lymphadenectomy. Histopathologic examination of the resection specimen showed three metastatic lymph nodes, despite local invasion limited to the lamina propria. This is the first report of a patient with superficial e

Diagnostic role of videothoracoscopy in mediastinal diseases.

OBJECTIVE: In order to assess the role of videothoracoscopy in the diagnosis of mediastinal diseases, we report a retrospective analysis of 52 cases of mediastinal biopsy performed with this technique. METHODS: Between January 1992 and December 1996 52 patients (39 men and 13 women, mean age 53 +/- 29 years) with mediastinal lesions were referred to our department for videothoracoscopic biopsy. There were eight lesions in the anterior mediastinum, while the remaining 44 were in the middle (25 right and 19 left). The adenopathies were solitary or located in positions not within reach of the me

Utility of the transcervical approach in bilateral synchronous lung cancer.

Bilateral pulmonary nodules represent a challenge in distinguishing between synchronous bronchogenic carcinomas and metastatic disease. In the case of potentially curable synchronous lung cancer, it is recommended to treat each lesion with curative intent if there is no evidence of mediastinal involvement or extrathoracic disease. In this situation, surgical staging of the mediastinum is recommended. This case shows the utility of a transcervical approach to perform precise mediastinal staging and lymphadenectomy, and to access the pleural cavity to resect a pulmonary nodule. Moreover, video-a

A fluorodeoxyglucose avid mediastinal parathyroid adenoma masquerading as metastatic bladder cancer.

A 64-year old male with a prior medical history of bladder transitional cell carcinoma treated with a cystoprostatectomy and adjuvant platinum-based chemotherapy 10 years earlier underwent a surveillance positron emission tomography (PET) scan that revealed a metabolically active 2-cm nodule in the superior mediastinum, anterior to the origin of the innominate artery. The lesion was removed due to concerns of metastatic disease using a cervical mediastinoscope. Final pathology revealed an ectopic mediastinal parathyroid adenoma. The combination of the rare presentation, uncommon surgical appro

Current evidence on transcervical mediastinal lymph nodes dissection.

In the last decade, we witnessed the development of two new, fascinating techniques of mediastinal exploration: the video-assisted mediastinoscopic lymphadenectomy (VAMLA) and the transcervical extended mediastinal lymphadenectomy (TEMLA). During this time, several articles were published, providing a substantial body of evidence regarding different aspects of these techniques. Based on this evidence, we discuss the operative technique, its applications and results as well as questions to be answered and possible future development. The aim of this review is to provide the readers who have no

V/Q mismatch without pulmonary emboli in children with histoplasmosis.

In two children with histoplasmosis pulmonary perfusion and ventilation studies revealed mismatched abnormalities characterized by almost unilaterally absent perfusion but normal ventilation in the right lung. Chest roentgenograms demonstrated right hilar enlargement and pulmonary contrast angiograms revealed narrowing of the right pulmonary arteries by extrinsic fibrotic granulomata but no pulmonary emboli were present. Fibrosing mediastinitis due to histoplasmosis was found by mediastinoscopic examination in one of the patients. Non-embolic causes of V/Q mismatch lung imaging are discussed b

[Special diagnosis of pleural metastasis].

In 58% of cases diagnosis of pleural metastases is confirmed by identification of malignant cells in pleural effusions. Sensitivity of pleural punch biopsies ranges about the same. Combination of both methods increases success rate up to 75%. Thoracoscopy has a sensitivity of about 89%, charged with complications in 1.7%, and over-shadowed by a lethality of 0.08%. Classical thoracoscopy after Jacobäus has to be differentiated from Maassen's method, using the mediastinoscope. Highest success rates are achieved by means of diagnostics thoracotomy, which in fact is restricted to isolated cases, w

Mediastinal fibromatosis presenting with superior vena cava syndrome.

We encountered a fatal case of mediastinal fibromatosis in a 67-year-old female in whom there was aggressive infiltration into the large vessels, nerves and pericardium. She presented with the superior vena cava syndrome, Horner's syndrome, paralysis of bilateral vocal cords and diaphragm and heart failure. Mediastinoscopical examination revealed an extremely firm tumor adhering to the sternum, trachea and brachiocephalic artery. She died of severe heart failure due to the disturbed dilatation of the heart and ventilatory insufficiency. Although mediastinal fibromatosis is very uncommon and so

Technetium-99m-tetrofosmin uptake in sarcoidosis stage I.

The uptake of 99mTc-tetrofosmin in enlarged lymph nodes, of the lung hilus, in the case of sarcoidosis Stage I (histopathologically confirmed by mediastinoscopic biopsy) is demonstrated. On a routine chest radiograph of a 78-yr-old woman, hilar lymphadenopathy was first detected. In the following mammography, disseminated micro calcifications were found in the left breast and a 99mTc-tetrofosmin study was performed for detection of breast cancer. Scintigraphy using 99mTc-tetrofosmin showed clear uptake in the hilar lymph nodes, but not in the left breast. The 99mTc-tetrofosmin uptake in the hi

Primary repair of an iatrogenic bronchial rupture under video mediastinoscopy.

Complications after performing mediastinoscopy are uncommon, but they may occur even for an experienced surgeon. The major complications have the potential to be life-threatening injuries, such as major vascular or airway injury. A 51-year-old man presented to our hospital due to mediastinal node enlargement on follow-up after he had undergone gastric cancer surgery 2 years previously. An iatrogenic bronchial rupture occurred while performing mediastinoscopic biopsy, and this injury was primarily repaired with multiple direct interrupted sutures, along with the aid of a homemade knot pusher un
