Conditional Medicaid Expansion and Mental Health Outcomes in Georgia.

IMPORTANCE: The continuous Medicaid enrollment provision adopted during the COVID-19 pandemic ended in March 2023, after which approximately 24 million individuals were disenrolled. This unwinding was associated with worsening mental and behavioral health, an urgent public health concern in the US. On July 1, 2023, Georgia implemented Pathways to Coverage, a limited Medicaid expansion program that conditions eligibility on meeting monthly work or community engagement requirements. OBJECTIVE: To evaluate changes in mental health outcomes following implementation of Georgia's Pathways to Covera

Naloxone receipt following nonfatal opioid poisoning among Medicaid beneficiaries.

BACKGROUND: Despite naloxone's proven effectiveness in reversing opioid poisoning, receipt rates among the high-risk Medicaid population remain insufficient and the patient- and system-level characteristics associated with this gap are poorly understood. OBJECTIVES: To describe the patient-and system-level characteristics associated with a naloxone prescription fill within 7 days of a nonfatal opioid poisoning among Medicaid beneficiaries. METHODS: Using the Merative™ MarketScan Multi-State Medicaid Database (2018-2022), we identified adult (≥18 years) beneficiaries with a nonfatal opioid po

Medicaid Primary Care Utilization and Area-Level Social Vulnerability.

IMPORTANCE: The concentration of poverty and multidimensional disadvantage has been shown to limit access to health care in these communities. There is a growing interest in using area-level socioeconomic indexes to address the unequal geographic distribution of health care resources. However, the association of area-level socioeconomic indexes with access to primary care-a key area in health policy-has not been determined. OBJECTIVE: To investigate the association of Medicaid primary care utilization with the concentration of poverty and multidimensional disadvantage at the zip code level.

Entrenched Opportunity: Medicaid, Health Systems, and Solutions to Homelessness.

CONTEXT: As inequality grows, politically powerful health care institutions-namely Medicaid and health systems-are increasingly assuming social policy roles, particularly related to solutions to homelessness. Medicaid and health systems regularly interact with persons experiencing homelessness who are high users of emergency health services and who experience frequent loss of or inability to access Medicaid services because of homelessness. This research examines Medicaid and health system responses to homelessness, why they may work to address homelessness, and the mechanisms by which this oc

Treatment Access among Younger Medicaid Beneficiaries with Multiple Myeloma.

PURPOSE: Continuous Medicaid coverage prior to a cancer diagnosis has been associated with earlier detection and better outcomes, for patients with solid tumors. In this study, we aimed to determine if this was observed among patients with multiple myeloma, a hematologic cancer where there are no routine screening tests and most are diagnosed through acute medical events. MATERIALS AND METHODS: This is an analysis of the Merative MarketScan Multistate Medicaid Database, a claims-based dataset. In total, 1105 patients < 65 years old were included in the analyses. Among them, 66% had continuous

Thematic Analysis of State Medicaid Buprenorphine Prior Authorization Requirements.

IMPORTANCE: Prior authorization (PA) requirements for buprenorphine are associated with lower provision of the medication for the treatment of opioid use disorder (OUD). While Medicare plans have eliminated PA requirements for buprenorphine, many Medicaid plans continue to require them. OBJECTIVE: To describe and classify buprenorphine coverage requirements based on thematic analysis of state Medicaid PA forms. DESIGN, SETTING, AND PARTICIPANTS: This qualitative study used a thematic analysis of 50 states' Medicaid PA forms for buprenorphine between November 2020 and March 2021. Forms were o

Specialist access and leg amputations among Texas Medicaid patients.

Medicaid coverage among patients with peripheral artery disease (PAD) has been associated with higher rates of primary amputations. We sought to determine the relative contributions of clinical, demographic, and hospital factors to leg amputations among Texas Medicaid patients. Patient-level data were used to identify patients who underwent treatment for PAD-related foot complications in Texas. Patients were categorized into groups by insurance provider (Medicaid, Medicare, dual-enrollee, commercial, and provider network). Individual- and area-level multivariate analyses were used to find asso

Appalachian disparities in tobacco cessation treatment utilization in Medicaid.

BACKGROUND: Kentucky Medicaid enrollees, particularly those in the rural Appalachian region, face disproportionate smoking rates and tobacco-related disease burden relative to the rest of the United States (US). The Affordable Care Act (ACA) mandated tobacco cessation treatment coverage by the US public health insurance program Medicaid. Medicaid coverage was also expanded in Kentucky, in 2013, with laxer income eligibility requirements. This short report describes tobacco use incidence and tobacco cessation treatment utilization, comparing by Appalachian status before and after ACA-mandated c

Prescription Opioid Quality Measures Applied Among Pennsylvania Medicaid Enrollees.

BACKGROUND: The Pharmacy Quality Alliance (PQA) recently developed 3 quality measures for prescribing opioids: high dosages, multiple providers and pharmacies, and concurrent use of opioids and benzodiazepines. OBJECTIVE: To examine the prevalence of the PQA measures and identify the patient demographic and health characteristics associated with the measures. METHODS: We conducted a cross-sectional analysis using Pennsylvania Medicaid data (2013-2015). We limited our analyses to noncancer patients who were aged 18-64 years and not dual-eligible for Medicare/Medicaid. Per PQA specifications,

HIV Testing among Outpatients with Medicaid and Commercial Insurance.

OBJECTIVE: To assess HIV testing and factors associated with receipt of testing among persons with Medicaid and commercial insurance during 2012. METHODS: Outpatient and laboratory claims were analyzed from two databases: all Medicaid claims from six states and all claims from Medicaid health plans from four other states and a large national convenience sample of patients with commercial insurance in the United States. We excluded those aged <13 years and >64 years, enrolled <9 of the 12 months, pregnant females, and previously diagnosed with HIV. We identified patients with new HIV diagnoses

Psychotropic medication use among Kansas Medicaid youths with disabilities.

Knowledge of current patterns of psychotropic medication use by disabled youths is an important starting point for targeting future efforts to improve prescribing practices. This study provides a retrospective, descriptive analysis of the prevalence and patterns of psychotropic medication among 1022 Kansas Medicaid youths in 2001 who were enrolled with either Supplemental Security Income (SSI) or Medically Needy benefits and who were having clearly defined mental and neurological disabilities. Rates of psychotropic medication use were generally higher for the Medically Needy than for the SSI y

The Association of the Affordable Care Act With Medicaid Enrollment Status and Costs of Care at a Level I Trauma Center in a Medicaid Non-expansion State.

INTRODUCTION: The intended purpose of the Patient Protection and Affordable Care Act (ACA) was to expand access to health care insurance for all Americans. In our study, we examine the association of Medicaid enrollment status, health care outcomes, and financial outcomes for trauma patients at a level I urban trauma center in a state that did not expand Medicaid coverage under the ACA. METHODS: We retrospectively reviewed trauma admissions from 2011 to 2016, via the trauma registry (n = 36,250). Chronic Hepatitis B Prevalence and Care Among Medicaid Beneficiaries.

BACKGROUND: Monitoring and antiviral treatment are essential to reduce chronic hepatitis B (CHB)-related morbidity and mortality. METHODS: We analyzed Medicaid claims data from January 1, 2016, through December 31, 2019, to estimate claims-recorded CHB prevalence and to evaluate CHB-directed monitoring, antiviral use, and qualifying liver imaging. RESULTS: Among 72.4 million adult Medicaid beneficiaries, 292,267 had claims-recorded CHB diagnosis (0.40%). After excluding beneficiaries with HIV or hepatitis C, HCC, or liver transplantation, 208,933 remained in the care-analysis cohort. Non-His

Work Requirements and Medicaid Enrollments: Will States Stanch Erosion?

The enactment of the One Big Beautiful Bill Act in July 2025 has introduced rigorous Medicaid work requirements, sparking projections of significant coverage losses for millions of low-income Americans. This essay examines the administrative and political dynamics that will determine whether states can mitigate this enrollment erosion. The Congressional Budget Office estimates that eligibility provisions, particularly work mandates for the Affordable Care Act's expansion population, will account for the majority of projected federal savings. We identify five primary coping strategies states ma

State Medicaid Programs Face Increased Spending On Medicare Premiums.

Medicaid covers Medicare premiums and out-of-pocket expenses for low-income older adults, who make up a growing segment of the US population. Using administrative data, we characterized the change in the number of beneficiaries dually enrolled in Medicare and Medicaid and corresponding changes in Medicaid spending on Medicare Part A and Part B premiums during the period 2013-24. The number of dually enrolled beneficiaries increased by 27 percent, while Medicaid spending on Medicare premiums nearly doubled, primarily as a result of growing Part B premium spending. We estimated that at least hal

Preventive service use among Medicaid expansion adults, 2014-2019.

INTRODUCTION: In 2014, the Affordable Care Act (ACA) mandated that newly eligible Medicaid expansion beneficiaries receive recommended preventive services at no cost. Yet, evidence regarding the Medicaid expansion population's use of preventive services is limited. METHODS: Medicaid claims data from 2011 to 2019. We focused on 12 selected preventive services that received new, revised, or upheld A or B USPSTF ratings since 2010. RESULTS: We find that receipt of preventive services among the Medicaid Expansion population increased from 22% in 2014 to 26% in 2019, with 41% of beneficiaries rec
