Factors Associated with Excessive Daytime Sleepiness in Medical Students of a Higher Education Institution of Bucaramanga.

INTRODUCTION: Excessive daytime sleepiness (EDS) can interfere with academic and professional performance, as affected individuals tend to fall asleep in situations that demand a high level of alertness. Medical students are often a population at risk of suffering from EDS due to the demanding number of study hours, the significant number of credits per subject in the academic curriculum, practical teaching sessions and hospital night shifts, which can lead to sleep deprivation or sleep debt. It is for these reasons that it is important to estimate the prevalence of EDS and its associated fact

Determinants of the generalist career intentions of 1995 graduating medical students.

Using national databases of the Association of American Medical College, the authors employed logistic regression analysis to show the relative predictive influences of selected demographic, structural, attitudinal, and educational variables on the specialty careers choices of 1995 U.S. medical school graduates. Plans to pursue certification in family practice or an unspecified generalist career could be predicted with moderate success, while choices of general internal medicine and general pediatrics could not. The intentions of the 1995 graduates to pursue generalist specialty, were signific

The ghostwriter and the test-tube baby: a medical breakthrough story.

Ghostwriting autobiographies has gained so high a profile that novels and films focus on the ghost. To deepen understanding of such collaborations in science and medicine, this article reconstructs the making of A Matter of Life (1980), 'the sensational story of the world's first test-tube baby'. Although critiqued by feminist scholars, revised through research and embellished in fiction, this double autobiography of Robert Edwards and Patrick Steptoe is still the standard history of the British team's work to achieve in vitro fertilisation (IVF). It is thus high time to investigate the debt a

Resurgence of seasonal influenza driven by A/H3N2 and B/Victoria in succession during the 2023-2024 season in Beijing showing increased population susceptibility.

During the COVID-19 pandemic, non-pharmaceutical interventions were introduced to reduce exposure to respiratory viruses. However, these measures may have led to an "immunity debt" that could make the population more vulnerable. The goal of this study was to examine the transmission dynamics of seasonal influenza in the years 2023-2024. Respiratory samples from patients with influenza-like illness were collected and tested for influenza A and B viruses. The electronic medical records of index cases from October 2023 to March 2024 were analyzed to determine their clinical and epidemiological ch

Distinct Features of Vascular Diseases in COVID-19.

The Coronavirus Disease 2019 (COVID-19) pandemic was declared in early 2020 after several unexplained pneumonia cases were first reported in Wuhan, China, and subsequently in other parts of the world. Commonly, the disease comprises several clinical features, including high temperature, dry cough, shortness of breath, and hypoxia, associated with findings of interstitial pneumonia on chest X-ray and computer tomography. Nevertheless, severe forms of acute respiratory syndrome-related coronavirus 2 (SARS-CoV-2) are not limited to the respiratory tract but also may be extended to other systems,

Predictors of outcomes of psychological treatments for disordered gambling: A systematic review.

This systematic review aimed to synthesise the evidence relating to pre-treatment predictors of gambling outcomes following psychological treatment for disordered gambling across multiple time-points (i.e., post-treatment, short-term, medium-term, and long-term). A systematic search from 1990 to 2016 identified 50 articles, from which 11 socio-demographic, 16 gambling-related, 21 psychological/psychosocial, 12 treatment, and no therapist-related variables, were identified. Male gender and low depression levels were the most consistent predictors of successful treatment outcomes across multiple

Cancer treatment-related financial toxicity in Japan: a scoping review.

Financial toxicity during cancer survival has been studied mainly in the United States; 47-49% of cancer survivors reported financial hardships and 12-63% reported debt owing to treatment costs. Financial toxicity is influenced by each country's economic status and healthcare system. We aimed to review the evidence on financial toxicity in Japan. A systematic search was performed using PubMed and Ichushi databases. We included English or Japanese peer-reviewed articles that (1) explored the experiences of cancer patients facing financial toxicity due to cancer diagnosis and treatment, (2) were

Risk factors, behaviors, and adverse health outcomes of financial toxicity in adult chronic myeloid leukemia survivors: a theory-based structural equation model.

PURPOSE: Patients diagnosed with chronic myeloid leukemia (CML) are at risk of developing financial toxicity (FT) due to the high cost of tyrosine kinase inhibitor (TKI) treatment and other cancer-related costs. This theory-based study evaluated relationships between risk factors, patients' behaviors, FT experience, and health outcomes among survivors of CML. METHODS: We collected cross-sectional survey data from 129 adults with CML prescribed a TKI from 10/2019 to 01/2024. Instruments included measures of FT, patients' behaviors related to FT (financial sacrifices, non-adherence, and debt or

Use of Financial Hardship as a Metric for Assessing Financial Toxicity in Surgical Trauma Patients.

BACKGROUND: Financial toxicity is the detrimental impact of health care costs that must be mitigated to achieve universal health coverage. Catastrophic health expenditure (CHE) is widely used to measure financial toxicity but does not capture patient perspectives of unaffordable health care costs. Financial hardship (FH), a patient-reported outcome measure, is currently underutilized but may be an important adjunct metric. The authors compare CHE to FH as metrics evaluating financial toxicity. METHODS: A prospective, multicenter cohort study was conducted across 3 public and private tertiary-

Can a Single Night of Sleep Deprivation Unmask Human Sleep and Neurophysiobehavioral Impairments During the Post-Acute Phase of Mild Traumatic Brain Injury?

The purpose of this study was twofold: (a) to determine whether, and the extent to which, the challenge of a single night of total sleep deprivation (TSD) unmasks lingering brain health-related deficits in individuals who within the past 3 to 12 months had been diagnosed (yet medically cleared) with a mild traumatic brain injury (mTBI+), and (b) to determine whether mTBI+ results in any neurophysiobehavioral deficits in the ability to recover from TSD. Seven previously concussed (mTBI+) adults (24.5 ± 5.3 years old) and six non-concussed control (mTBI-) adults underwent 24 h TSD preceded by 8

The effect of associated injuries, blood loss, and oxygen debt on death and disability in blunt traumatic brain injury: the need for early physiologic predictors of severity.

Studies of 4590 patients with blunt trauma injuries admitted to a Level I Trauma Center, have shown that 37% had a blunt traumatic brain injury (BTBI). Of these brain injured patients 60% has an associated other injury. Examination of mortality has shown that those with an isolated brain injury had an 11% mortality compared with 21.8% in those with an associated systemic injury. Further investigation demonstrated that the cause of the increased mortality was related to the blood loss associated with the injuries and that when hypovolemic shock resulted, mortality rose from 12.8 to 62%. The sev

Biological Connection of Psychological Stress and Polytrauma under Intensive Care: The Role of Oxytocin and Hydrogen Sulfide.

This paper explored the potential mediating role of hydrogen sulfide (H2S) and the oxytocin (OT) systems in hemorrhagic shock (HS) and/or traumatic brain injury (TBI). Morbidity and mortality after trauma mainly depend on the presence of HS and/or TBI. Rapid "repayment of the O2 debt" and prevention of brain tissue hypoxia are cornerstones of the management of both HS and TBI. Restoring tissue perfusion, however, generates an ischemia/reperfusion (I/R) injury due to the formation of reactive oxygen (ROS) and nitrogen (RNS) species. Moreover, pre-existing-medical-conditions (PEMC's) can aggrava
