[Standards for the interpretation of constitutional copy number gain: Recommendation from the American College of Medical Genetics and Genomics (ACMG) and Clinical Genome Resource (ClinGen)].

Copy number variants (CNVs) are common causes of human genetic diseases. CNVs detection has become a routine component of genetic testing, especially for pediatric neurodevelopmental disorders, multiple congenital abnormalities, prenatal evaluation of fetuses with structural anomalies detected by ultrasound. Although the technologies for CNVs detection are continuously improving, the interpretation is still challenging, with significant discordance across different laboratories. In 2020, the American College of Medical Genetics and Genomics (ACMG) and the Clinical Genome Resource (ClinGen) dev

Hershey Medical Center Technical Workshop Report: optimizing the design and interpretation of epidemiologic studies for assessing neurodevelopmental effects from in utero chemical exposure.

Neurodevelopmental disabilities affect 3-8% of the 4 million babies born each year in the U.S. alone, with known etiology for less than 25% of those disabilities. Numerous investigations have sought to determine the role of environmental exposures in the etiology of a variety of human neurodevelopmental disorders (e.g., learning disabilities, attention deficit-hyperactivity disorder, intellectual disabilities) that are manifested in childhood, adolescence, and young adulthood. A comprehensive critical examination and discussion of the various methodologies commonly used in investigations is ne

The experiences of three women who underwent termination of pregnancy for medical reasons: a comparative methodological study using grounded theory analysis and interpretative phenomenological analysis.

INTRODUCTION: Termination of pregnancy [ToP] for medical reasons, especially for a wanted pregnancy, is an underexplored area in public health. Women face significant emotional, psychological, and social challenges, often compounded by inconsistent policies, healthcare inequities, and societal stigma. Additionally, the decision-making process can be influenced by financial and systemic factors, with women reporting long-lasting emotional impacts and inadequate mental health support following a ToP. METHODS: Semi-structured interviews were conducted with three women who underwent ToP for medic

Development and validation of an interpretable medical deep foundation model for predicting occult lymph node metastasis in early-stage small cell lung cancer: a multicenter retrospective diagnostic study.

BACKGROUND: Occult lymph node metastasis (OLM) is common in patients with clinical stage T1-2N0M0 small cell lung cancer (SCLC) and is strongly associated with inferior survival. Accurate preoperative identification of OLM remains challenging using conventional imaging, yet is critical for nodal staging, surgical planning, and individualized treatment decision-making. This study aimed to develop and externally validate an interpretable deep foundation model-based approach using preoperative contrast-enhanced computed tomography (CECT) to predict OLM in early-stage SCLC. METHODS: In this multi

Feasibility, Acceptability, and Preliminary Performance of Check In for Exchange of Clinical and Key Information: A Communication Guide to Facilitate Pre-Encounter Huddles with Medical Interpreters Prior to Conversations Around Serious Illness.

BACKGROUND: Patients with non-English Language Preference are at risk of adverse health outcomes, particularly at end of life and during serious illness. Medical interpreters often feel unprepared to interpret conversations about serious illness. Best practice guidelines recommend a pre-encounter huddle between clinician and interpreter to better prepare both providers. The CHECK-IN (Check in for Exchange of Clinical and Key Information) guide is a simple communication tool designed to facilitate this pre-encounter huddle. We describe the results of a pilot feasibility study of the CHECK-IN gu

Tailoring the American College of Medical Genetics and Genomics and the Association for Molecular Pathology Guidelines for the Interpretation of Sequenced Variants in the FBN1 Gene for Marfan Syndrome: Proposal for a Disease- and Gene-Specific Guideline.

BACKGROUND: The introduction of next-generation sequencing techniques has substantially increased the identification of new genetic variants and hence the necessity of accurate variant interpretation. In 2015, the American College of Medical Genetics and Genomics and the Association for Molecular Pathology proposed new variant interpretation guidelines. Gene-specific characteristics were, however, not considered, sometimes leading to inconsistent variant interpretation. METHODS: To allow a more uniform interpretation of variants in the FBN1 (fibrillin-1) gene, causing Marfan syndrome, we tail

Interpretable machine learning for 28-day all-cause in-hospital mortality prediction in critically ill patients with heart failure combined with hypertension: A retrospective cohort study based on medical information mart for intensive care database-IV and eICU databases.

BACKGROUND: Heart failure (HF) combined with hypertension is an extremely important cause of in-hospital mortality, especially for the intensive care unit (ICU) patients. However, under intense working pressure, the medical staff are easily overwhelmed by the large number of clinical signals generated in the ICU, which may lead to treatment delay, sub-optimal care, or even wrong clinical decisions. Individual risk stratification is an essential strategy for managing ICU patients with HF combined with hypertension. Artificial intelligence, especially machine learning (ML), can develop superior
