Epimutations in migraine and in medication overuse headache: a longitudinal study after acute medication withdrawal.

BACKGROUND: Headache disorders are among the leading causes of neurological disability worldwide, affecting over half of the global population. Recent data indicate that headache prevalence among young people increased by 23.7% between 1990 and 2021. Migraine, a highly disabling primary headache, significantly impacts daily life, yet timely and accurate diagnosis remains challenging. Many patients self-manage with analgesics, contributing to medication-overuse headache (MOH), which affects 1–2% of the general population. Emerging evidence suggests that epigenetic mechanisms, particularly DNA m

Treatment of medication-overuse headache in children and adolescents: A systematic review.

OBJECTIVE: To examine the effectiveness of treatment strategies for medication-overuse headache (MOH) in children and adolescents. BACKGROUND: There are no guidelines for the treatment of adult or pediatric patients with MOH. However, research pertaining to MOH in adults is far outpacing research in children and adolescents. METHODS: A systematic review of evidence on treatment for MOH in children and adolescents was performed via PubMed, Embase, Web of Science, PsycINFO, and Cochrane from database inception to October 2024 for articles with the subject headings and keywords for MOH and pedi

Pain Coping in Patients With Chronic Migraine and Medication Overuse Headache.

BACKGROUND: Chronic migraine (CM) is a leading cause of disability and often linked to medication overuse headache (MOH). Psychological factors, such as pain coping, may contribute to chronification and medication overuse. While behavioral therapy can help, identifying patients who will benefit remains challenging. This study compared pain coping in individuals with CM and MOH to those with episodic migraine (EM) and controls without headache. It also assessed whether baseline pain coping in CM patients could predict withdrawal treatment success. METHODS: In patients that received behavioral

Reduction of neck pain severity in patients with medication-overuse headache.

BACKGROUND: Neck pain and primary headache disorders are highly prevalent in populations and clinical cohorts. Medication-overuse headache (MOH) is a treatable secondary headache, mainly developing in migraine sufferers, that accounts for the majority of patients presenting to headache clinics. Nevertheless, the association between neck pain and MOH has not been reported. This study evaluated the prevalence and clinical course of neck pain in patients with MOH before and after MOH treatment. METHODS: We analyzed 635 MOH patients enrolled in a nationwide, prospective, multicenter MOH registry.

Medication Overuse Headaches among Children-The Contribution of Migraine and TTH.

Medication overuse headaches are a frequent phenomenon observed in individuals suffering from chronic headaches. It arises due to the excessive consumption of pain-relief medications, resulting in the escalation and continuous persistence of headache symptoms. Nevertheless, the prevalence and distinctive characteristics of medication overuse headaches in the pediatric population have not been comprehensively explored. The primary objective of this research is to delineate the features of medication overuse headaches in children, particularly emphasizing the investigation of its epidemiology an

The Evolution of Medication Overuse Headache: History, Pathophysiology and Clinical Update.

Medication overuse headache (MOH), the development or worsening of chronic headache resulting from frequent and excessive intake of medications used for acute treatment of headache, is a common secondary headache disorder and is associated with significant personal and societal burdens. The plausible physiologic mechanism is that chronic exposure to acute care migraine treatment leads to suppression of endogenous antinociceptive systems, consequently facilitating the trigeminal nociceptive process via up-regulation of the calcitonin gene-related peptide (CGRP) system. Recognizing and preventin

European Academy of Neurology guideline on the management of medication-overuse headache.

BACKGROUND: The frequent use of medication to treat migraine attacks can lead to an increase in migraine frequency and is called medication-overuse headache (MOH). METHODS: Based on the available literature in this guideline, the first step in patient management is education and counselling. RESULTS: Patients with MOH should be managed by a multidisciplinary team of neurologists or pain specialists and behavioral psychologists. Patients in whom education is not effective should be withdrawn from overused drugs and should receive preventive treatment with drugs of proven efficacy. Patients wi

Pathophysiology of medication-overuse headache: implications from animal studies.

Recent animal experiments have shown that chronic medication exposure profoundly affects the function of several areas in the nervous system related to headache pathogenesis. These changes include upregulation of calcitonin gene-related peptide, substance P, and nitric oxide synthase in trigeminal ganglia; expansion of receptive field and decreased nociceptive threshold of central trigeminal neurons; decrease in diffuse noxious inhibitory control; and increased susceptibility to develop cortical spreading depression (CSD). These changes indicate an increase in excitability of cortical and trig

Medication overuse as a cause of chronic headache in shunted hydrocephalus patients.

PURPOSE: To highlight the group of hydrocephalus patients known to have a long history of shunt revisions and refractory chronic headache. When a shunt in perfect working order has no effect on headache, other causes of headache should be investigated. In this paper, patients with medication overuse headache are identified and the positive effect of medication withdrawal are described. MATERIALS AND METHOD: Patients with hydrocephalus and shunt referred from the neurosurgical department to the Danish Headache Centre were identified. In all cases, over- and underdrainage was ruled out prior to

What is the role of dependence-related behavior in medication-overuse headache?

Medication-overuse headache (MOH) can be viewed as an interaction between the worsening of the primary headache course and individual predispositions for dependence. We present here a review of the clinical and biological data raising the role of dependence-related behavior in MOH. Indeed, several clinical studies show that acute headache medications containing psychoactive components (barbiturates, opiates) are associated with an increased risk of MOH. Diagnostic and Statistical Manual of Mental Disorders, 4th edition substance dependence criteria were identified in a sub-group of MOH patient

Tension-type headache with medication overuse: pathophysiology and clinical implications.

Tension-type headache (TTH) is the most prevalent primary headache disorder. An important factor in the long-term prognosis of TTH is the overuse of acute medications used to treat headache. There are many reasons why patients with TTH overuse acute medications, including biobehavioral influences, dependency, and a lack of patient education. Chronic daily headache occurs in 4.1% of the general population, and chronic tension-type headache and medication overuse headache (MOH) occur in approximately 2.2% and 1.5%, respectively. A proper diagnosis is essential for the treatment of these patients

Screening and behavioral management: medication overuse headache--the complex case.

The new appendix criteria for a broader concept of chronic migraine from the International Headache Society no longer require headache resolution or return to the previous headache pattern to confirm the diagnosis of medication overuse headache (MOH). MOH can be subdivided into simple (Type I) and complex (Type II). Complex cases may involve long-term use of daily opioids or combination analgesics, multisourcing, multiple psychiatric comorbidities, and/or a history of relapse. Daily use of opioids for other medical conditions, psychiatric comorbidity including borderline personality disorder,
