The impact of health insurance status on stage at diagnosis of oropharyngeal cancer.

BACKGROUND: Although patients who have early-stage oropharyngeal cancer can be treated with little impairment of function, the treatment of advanced disease can result in decreased quality of life and mortality. Patients without insurance and with other barriers to access to care may delay seeking medical attention for early symptoms, resulting in more advanced disease at presentation. In this study, the authors examined whether patients who had no insurance or who were covered by Medicaid insurance were more likely to present with advanced oropharyngeal cancer. METHODS: In this retrospective

Full subsidy and direct delivery of essential chronic disease medications for low-income populations: A pilot randomized controlled trial.

INTRODUCTION: Care transition interventions focused on medication therapy management have not proven effective among low-income populations. This pilot study assesses a novel intervention approach that directly addresses social determinants of health (SDoH) reported as major barriers to medication adherence post-hospitalization by low-income patients. METHODS: This pilot pragmatic randomized controlled trial enrolled hospitalized adult Medicaid and uninsured patients with ambulatory care sensitive conditions using ≥2 chronic disease medications from a large safety-net hospital in the Mid-Sout

Cardiovascular Risk in Middle-Aged and Older Immigrants: Exploring Residency Period and Health Insurance Coverage.

PURPOSE: It is reported that while immigrants are, initially, healthier than the native-born upon resettlement, this advantage erodes over time. In the United States, uninsured aging immigrants are increasingly experiencing severe complications of cardiovascular disease (CVD). The purpose of this study was to compare overall CVD risk and explore the importance of health insurance coverage on CVD risk relative to other health access barriers, from 2007 to 2012, in recent and long-term immigrants >50 years of age. METHODS: This study was based on secondary cross-sectional analysis of the Nation

The upper extremity trauma patient.

Trauma patients are predominantly young men. Urban trauma patients frequently are uninsured, and often are victims of intentionally inflicted penetrating injury, usually caused by firearms. Substance use is a common underlying factor in both urban and rural trauma. This article introduces urban and rural patients. It discusses their past medical and social histories, which are required to help the decision-making process when treating these patients.

Access to preventive health care for cancer survivors.

BACKGROUND: Access to health care, particularly effective primary and secondary preventive care, is critical for cancer survivors, in order to minimize the adverse sequelae of cancer and its treatment. PURPOSE: The goal of the study was to evaluate the association between cancer survivorship and access to primary and preventive health care. METHODS: Cancer survivors (n=4960) and individuals without a cancer history (n=64,431) aged ≥ 18 years, from the 2008-2010 Medical Expenditure Panel Survey (MEPS), were evaluated. Multiple measures of access and preventive services use were compared. The

Association of insurance types and outcomes in acute promyelocytic leukemia.

Understanding the association between insurance status and survival in an evolving US healthcare system remains a challenge but is essential to address healthcare disparities. We utilized National Cancer Database to evaluate the effects of insurance type on one-month mortality and overall survival (OS) in patients with acute promyelocytic leukemia. Among patients <65 years, one-month mortality was worse for uninsured patients and patients with Medicare compared to patients with private insurance. OS was similar between patients with private insurance and uninsured patients but worse for pat

Financial Instability of Federal Navigator Program Challenges Organizations to Help Uninsured Enroll in Health Insurance Coverage.

Under the Affordable Care Act, the federal Health Insurance Navigator Program aims to reduce the rate of uninsured in the United States. Under this program, navigators help people obtain insurance coverage through federally facilitated Marketplaces. However, the program's financial instability and substantial budget cuts created a severe shortage of navigator assistance for the uninsured and underserved. The COVID-19 pandemic added further pressure to the already-strained program. Our study examined how unstable and unpredictable federal funding and the COVID-19 pandemic affected organizations

Charges for medical care at different hospitals.

BACKGROUND: The United States has a high proportion of people without health insurance (15%) and a low proportion of people without employment (5%), resulting in millions who lack insurance but have some ability to pay. We tested whether hospitals charge similar prices for well-specified elective services to individuals paying out-of-pocket for medical care. METHODS: We surveyed the 2 largest general hospitals from every large city (population >500 000) in the United States and Canada. At each hospital we evaluated 5 diagnostic, 7 therapeutic, and 3 nonclinical services to determine the total

Access to Healthcare Services among Thai Immigrants in Japan: A Study of the Areas Surrounding Tokyo.

Numerous undocumented and uninsured foreigners living in Japan have faced barriers when trying to obtain appropriate healthcare services, which have occasionally led to issues with unpaid medical bills to medical institutions. Although information on health and socioeconomic status is essential to tackle such issues, relevant data has been unavailable due to difficulties in contacting this population. This study involved a cross-sectional survey using questionnaires concerning the general demographic characteristics, socioeconomic status, health profiles, information access, and knowledge/atti

Race and Prevalence of Large Bowel Polyps Among the Low-Income and Uninsured in South Carolina.

BACKGROUND: Compared to whites, blacks have higher colorectal cancer incidence and mortality rates and are at greater risk for early-onset disease. The reasons for this racial disparity are poorly understood, but one contributing factor could be differences in access to high-quality screening and medical care. AIMS: The present study was carried out to assess whether a racial difference in prevalence of large bowel polyps persists within a poor and uninsured population (n = 233, 124 blacks, 91 whites, 18 other) undergoing screening colonoscopy. METHODS: Eligible patients were uninsured, asym

Self-Pay Prices and Financial Assistance Programs for Urology Appointments.

IMPORTANCE: For uninsured or out-of-network patients, uncertainty regarding the expected prices for urology appointments presents a meaningful barrier to timely evaluation. Although price variation for hospital-based services is well documented, self-pay price information in the ambulatory urology setting remains uncharacterized. OBJECTIVE: To quantify self-pay appointment prices and characterize the availability and structure of financial assistance programs (discounts and payment plans) in urology clinics nationwide. DESIGN, SETTING, AND PARTICIPANTS: This national, cross-sectional, audit

Outpatient Visits among Older Adults Living Alone in China: Does Health Insurance and City of Residence Matter?

This study aimed to examine the association between health insurance, city of residence, and outpatient visits among older adults living alone in China. A sample of 3173 individuals was derived from "Survey on Older Adults Aged 70 and Above Living Alone in Urban China" in five different cities. Logistic regression models indicated that older adults living alone who had urban employee basic medical insurance, urban resident basic medical insurance, and public medical insurance were more likely to have outpatient visits than those without any health insurance. After controlling the number of chr
