BACKGROUND: Accuracy and transportability of the recorded outpatient medication list are important in the continuum of patient care. Classifying discrepancies between the electronic medical record (EMR) and actual drug use by the root cause of discrepancy (either system generated or patient generated) would guide quality improvement initiatives.
OBJECTIVES: To quantify and categorize the number and type of medication discrepancies that exist between the medication lists recorded in EMRs and the comprehensive medication histories obtained through telephone interviews conducted by a team of nur

BACKGROUND: Hospitalized older adults are commonly discharged with changes to antihypertensive and glucose-lowering (cardiometabolic) medications. Though adverse drug events remain a leading cause of readmissions, there is little contemporary data on how medication discharge planning is communicated and how often medication errors occur post-discharge.
OBJECTIVE: To assess older adults' post-hospital medication use and ambulatory follow-up after receiving cardiometabolic medication changes during hospitalization.
DESIGN: Prospective cohort study from 11/2022 to 01/2024.
PARTICIPANTS: Adults

INTRODUCTION: Medication management is central to endoscopic safety because comorbidities, polypharmacy, and variable drug responses can substantially influence procedural risk. This review examines how a precision medicine approach can improve peri-endoscopic decision-making and patient outcomes.
AREAS COVERED: A structured literature review of PubMed/MEDLINE, Embase, and the Cochrane Library (2000-2025) evaluated evidence on medications and supplements affecting endoscopic safety and efficacy. The review examined drug classes influencing bleeding, sedation, bowel preparation, aspiration, me

A 66-year-old male patient presented with fever, headache, mental status changes, and nuchal rigidity with a lumbar puncture revealing neutropenic pleocytosis, and a presumptive diagnosis of bacterial meningitis was made. A careful history revealed that symptoms started within hours of starting oral trimethoprim-sulfamethoxazole. Additional history uncovered a nearly identical episode 1 year earlier after 1 dose of trimethoprim-sulfamethoxazole. All microbiologic diagnostic testing for meningitis was negative and all antimicrobials were discontinued. The patient had resolution of symptoms by 9

INTRODUCTION: Medication incident reporting systems enable healthcare professionals to report incidents and their analysis may help prevent reoccurrences. While previous research has primarily focused on medication errors in general, incidents involving unintentional medication discrepancies remain unexplored.
AIM: Therefore, this study aimed to identify the causes of voluntarily reported medication incidents describing unintentional medication discrepancies occurring in care transitions. The secondary objective was to characterise the reported medication incidents regarding type of incident,

Acute pancreatitis (AP) is one of the most common gastrointestinal diagnoses for inpatient admissions, with medications being a rare inciting agent. Our case highlights a 55-year-old male patient who was admitted for recurring epigastric pain and elevated lipase levels, with imaging showing AP with necrosis. After common etiologies for AP, including gallstones and alcohol, were ruled out, medication reconciliation showed that lisinopril was prescribed concurrently with the onset of AP episodes. Once lisinopril was discontinued, the patient's symptoms resolved. This case aims to make healthcare

Direct oral anticoagulants have become increasingly used for atrial fibrillation and venothromboembolic disease. Thus far, there have been a few published cases of pericardial effusion associated with rivaroxban. However, there has been little published regarding the effects of concurrent medications and their effect on the cytochrome enzyme systems involved in rivaroxaban metabolism. We present a case of a 76-year-old female who develops a spontaneous haemopericardium after initiating rivaroxaban. After thorough medical reconciliation, we offer pharmacokinetic mechanisms that may have contrib

BACKGROUND: The number of Chinese Americans is growing and nearly 20% of older Chinese-American home care patients have at least one potentially inappropriate medications (PIMs) prescribed at hospital discharge. Further examination of PIMs during care transition is warranted, especially among older Chinese-Americans.
OBJECTIVES: To describe, during care transitions from hospital to home care: (i) the subtypes of PIMs, (ii) the difference of PIM prevalence prescribed at hospital discharge as compared to home care admission, (iii) the relationship between subtypes of PIMs, and (iv) issues affec

Medication exposure is dependent upon many factors, the single most important being if the patient took the prescribed medication as indicated. To assess medication exposure for psychotropic and other medication classes, we enrolled 115 highly adherent psychiatry patients prescribed five or more medications. In these patients, we measured 21 psychotropic and 38 nonpsychotropic medications comprising a 59 medication multiplex assay panel. Strict enrollment criteria and reconciliation of the electronic health record medication list prior to study initiation produced a patient cohort that was adh

Delirium is common in older emergency department (ED) patients. Although associated with significant morbidity and mortality, it often goes unrecognized. A consistent approach to evaluation of mental status, including use of validated tools, is key to diagnosing delirium. Identification of the precipitating event requires thorough evaluation, including detailed history, medication reconciliation, physical examination, and medical work-up, for causes of delirium. Management is aimed at identifying and treating the underlying cause. Meaningful improvements in delirium care can be achieved when p

WHAT IS KNOWN AND OBJECTIVE: Neuroleptic malignant syndrome (NMS) is a rare but severe adverse effect of antipsychotic drugs.
CASE DESCRIPTION: We report two cases of NMS highlighted by clinical pharmacists in an emergency unit during summer. One of them was fatal. Medication reconciliation processes performed at admission identified treatment with loxapine for one of them and with loxapine and clozapine for the other. Interview of the patients highlighted clinical symptoms suggesting NMS, allowing the pharmacists to alert the medical team.
WHAT IS NEW AND CONCLUSION: Adverse drug events may

BACKGROUND: Proper medication reconciliation (= comparing the accuracy of patient-reported medication use with pharmacy records) could prevent potentially dangerous situations such as drug-drug interactions and hospitalization. This is particularly important when patients rely on multiple medications, such as in neurodegenerative disorders like Huntington's Disease (HD). Currently, it is unknown how often medication discrepancies occur in HD patients and which factors contribute to the discrepancies.
OBJECTIVE: Identify prognostic factors of medication discrepancies in HD, using patient-repor

BACKGROUND: Hospital readmissions due to medication-related problems occur frequently, burdening patients and caregivers emotionally and straining health care systems economically. In times of limited health care resources, interventions to mitigate the risk of medication-related readmissions should be prioritized to patients most likely to benefit. Focusing on general internal medicine patients, this scoping review aims to identify risk factors associated with drug-related 30-day hospital readmissions.
METHODS: We began by searching the Medline, Embase, and CINAHL databases from their incept

A 7-year-old boy presented with new-onset type one diabetes mellitus (T1DM) and moderate diabetic ketoacidosis (DKA). The patient presented with polyuria, polydipsia, and weight loss along with reduced activity and excessive sleepiness. Initial blood tests revealed elevated blood glucose levels, high anion-gap metabolic acidosis, and ketonuria. The patient was treated with fluid resuscitation, insulin infusion, and electrolyte replacement, which resolved within three days. During medication reconciliation, the patient was administered empagliflozin, a sodium-glucose co-transporter 2 (SGLT2) in

Methaemoglobinaemia is a rare disease that is typically caused by a medication or other exogenous agent, with dapsone being the most common. It occurs when the concentration of methaemoglobin rises via ferrous haeme irons becoming oxidised to the ferric state, which shifts the oxygen dissociation curve to the left. The net result of an elevated methaemoglobin concentration is functional anaemia and impaired oxygen delivery to tissues. At lower blood levels, this can cause symptoms such as cyanosis, lethargy, headache and fatigue, whereas at higher levels it can be fatal. Here we discuss a subt

Sjogren's syndrome is an autoimmune disease associated with xerostomia and xerophthalmia. The association of Sjogren's with hyponatremia has rarely been reported and has been attributed to syndrome of inappropriate antidiuretic hormone secretion. Here, we report a case of polydipsia secondary to xerostomia as a cause of chronic hyponatremia in the setting of Sjogren's syndrome. Analysis of the patient's medical record, including medication reconciliation and dietary habits, revealed several underlying causes of her recurrent hyponatremia. A thorough review of the patient's clinical history and

Retroperitoneal haemorrhage (RH) is not uncommon in patients with provoking events like trauma. However, spontaneous RH (SRH) is a rare and life-threatening complication described as the development of bleeding into the retroperitoneal cavity, appearing spontaneously and without a preceding history of trauma or other predisposing illness. We are reporting a case of an elderly patient with recurrent deep vein thrombosis who had developed SRH secondary to concurrent use of multiple anticoagulation agents, resulting from poor healthcare follow-up and lack of sufficient medication reconciliation.

Medication errors and adverse drug reactions (ADRs) remain major patient safety concerns in primary healthcare settings worldwide, contributing substantially to preventable morbidity, mortality, and healthcare costs. This systematic review synthesized evidence regarding the prevalence, causes, prevention strategies, and impacts of medication errors in primary care. A comprehensive search of PubMed, EMBASE, CINAHL, Cochrane Library, and Web of Science was conducted for studies published between 2013 and 2024. Eligible studies included randomized controlled trials, observational studies, qualita
