The medicine use pathway is a process which is constantly evolving in order to comply with intangible rules. As in other therapeutic fields, the drug regimen in paediatrics must tolerate no error and must be able to detect all warning signs, however minor, in order to optimise this approach.

Nowadays, patients usually take more than three drugs for diseases such as hypertension, diabetes, and dyslipidemia. Hence, nuclear medicine physicians should be very careful about the medication history of each patient and ensure that their medication will not cause false positive or false negative imaging results, because either condition will interfere with adequate treatment of the patient and result in a wrong diagnosis. The aim of the present paper is to develop an ontology-based medication search and alert system for scintiphotography of Chang Gung Memorial hospital at Kaohsiung. Compos

STRUCTURE: The hospital emergency plan consists of a basic plan and an appendix. The basic plan deals with the general aspects of emergency operation and the special aspects of external and internal emergencies. The appendix contains special instructions, e.g., emergency action orders, staff alert lists, material lists, and situation plans.
CONTENT AND IMPLEMENTATION: External emergencies (e.g., mass casualty incidents) and internal emergencies (e.g., fire and other environmental threats) should be regarded. Once a defined experienced physician decides to activate the emergency plan, the hosp

BACKGROUND: People with Parkinson's (PwP) experience disproportionately higher rates of hospitalisation, where they encounter many medication-related challenges. Although interventions have been developed to address these issues, medication-related problems persist, and the patient's voice is largely absent from their design.
OBJECTIVES: To explore medication-related priorities during hospitalisation from the perspective of PwP to inform the development of future patient-led interventions.
METHODS: A systematic review using meta-ethnography was conducted and registered with PROSPERO. A SPICE

BACKGROUND: High-alert medications are frequently responsible for adverse drug events and present significant hazards to inpatients, despite technical improvements in the way they are ordered, dispensed, and administered.
METHODS: A real-time surveillance application was designed and implemented to enable pharmacy review of high-alert medication orders to complement existing computerized provider order entry and integrated clinical decision support systems in a tertiary care hospital. The surveillance tool integrated real-time data from multiple clinical systems and applied logical criteria t

BACKGROUND: Parenteral nutrition and concentrated electrolytes are associated with medication errors in pediatrics; however, a systems-based understanding of these errors is lacking. We aimed to describe the errors, contributing factors, and proposed defenses related to these medications in pediatrics.
METHODS: The study analyzed self-reported medication safety incidents at a pediatric university hospital between 2018 and 2020, focusing on parenteral nutrition and concentrated electrolytes. The reports were investigated through qualitative content analysis, and the results were quantified in

BACKGROUND: Laboratory testing is frequently unnecessary, particularly repetitive testing. Among the interventions proposed to reduce unnecessary testing, Computerized Decision Support Systems (CDSS) have been shown to be effective, but their impact depends on their technical characteristics. The objective of the study was to evaluate the impact of a Serology-CDSS providing point of care reminders of previous existing serology results, embedded in a Computerized Physician Order Entry at a university teaching hospital in Paris, France.
METHODS: A CDSS was implemented in the Cardiovascular Surg

BACKGROUND AND AIMS: Health information technologies are widely used in hospitals to reduce the risk of medication errors. However, evidence on their impact on medication management and patient safety in pediatric settings remains limited. The aim of this study was to assess safety culture in medication management and medication error reporting in pediatric wards and to identify areas for improvement.
METHOD: A survey was developed and presented on the annual meeting of Medicines for Children Network, Norway, attended by 80 physicians, pharmacists, and nurses. Following adjustments, the quest

High-consequence pathogens create a unique problem. To provide effective treatment for infected patients while providing safety for the community, a series of 10 high-level isolation units have been created across the country; they are known as Regional Ebola and Special Pathogen Treatment Centers (RESPTCs). The activation of a high-level isolation unit is a highly resource-intensive activity, with effects that ripple across the healthcare system. The incident command system (ICS), a standard tool for command, control, and coordination in domestic emergencies, is a command structure that may b

BACKGROUND: Hospital readmissions due to medication-related problems occur frequently, burdening patients and caregivers emotionally and straining health care systems economically. In times of limited health care resources, interventions to mitigate the risk of medication-related readmissions should be prioritized to patients most likely to benefit. Focusing on general internal medicine patients, this scoping review aims to identify risk factors associated with drug-related 30-day hospital readmissions.
METHODS: We began by searching the Medline, Embase, and CINAHL databases from their incept

Medication safety is a critical component of patient care, yet fragmented regulations, under-reporting of adverse events, and resource constraints continue to pose risks in India and Southeast Asia. Addressing these challenges requires system-level interventions supported by robust policies and technology-driven solutions. This review analyzes medication safety frameworks in India and World Health Organization South-East Asia Region countries using a Strengths, Weaknesses, Opportunities, and Threats (SWOT) approach, identifying internal strengths and weaknesses alongside external opportunities

INTRODUCTION: Prescribing errors (PEs) in hospital care lead to patient harm, prolonged hospital stays, readmissions, and mortality. Despite interventions that successfully target 'high risk' populations in trials, PE rates remain largely unchanged in real-world settings. Existing studies often focus narrowly on specific populations, overlooking the wider complexities of hospital-wide prescribing. This scoping review evaluates interventions for adult inpatients to identify knowledge gaps in how to reduce in-hospital PEs.
METHODS: A systematic search of PubMed, EMBASE.com, and the Cochrane Lib
