Rare type of Bellini Duct Carcinoma in a Patient With Cacchi-Ricci Disease: A Case Report and Mini-Review.

Medullary sponge kidney (MSK) is an uncommon kidney malformation, characterized by cystic dilatation of the precalyceal papillary collecting ducts. Urography and computed tomography scan represent the gold standard to detect this congenital disorder. A clear diagnosis is not always feasible, especially in the presence of a concomitant renal mass, which in turn can be difficult to detect in MSK patients. When conventional imaging is inconclusive, a renal biopsy can be considered in doubtful cases. Here, we report a unique case of a Bellini duct carcinoma in a patient with MSK and we review the

[Description of a case of Cacchi Ricci disease associated with hyperparathyroidism in the setting of multiple endocrine disease].

The authors present a case of a 36 years old woman applied to them for a follow up of sponge kidney disease and in whom they discovered a primary hyperparathyroidism. Further investigations discovered a medullary thyroid carcinoma. So this patient present multiple endocrine neoplasia (MEN) type 2A confirmed by microscopic and genetic analysis. This observation enlarged the discussion upon the nature of the relationship between kidney disease and primary hyperparathyroidism. Elsewhere it recommended to check systematically genetic markers of MEN type 2A if there is an association with sponge ki

[Hereditary tubulo-interstitial nephropathy].

Most of the hereditary tubulointerstitial nephropathy see the cyst formation in the kidney. Among them, juvenile nephronophthisis and medullary sponge kidney, which primarily involve the tubule structures of the renal medulla, are associated with variable enlargement of the distal tubules and collecting ducts and with interstitial fibrosis and inflammation of a variable extent. Juvenile nephronophthisis features sodium wasting, anemia, and renal failure. Eighty % of juvenile nephronophthisis is an autosomal recessive disorder, and it also has a variant form which is an autosomal dominant disea

Weeping sponge kidney: an unusual phenomenon that should be considered in cases of severe renal haemorrhage.

AIM: To describe the clinical presentation, imaging evaluation, endovascular management, and outcomes of multifocal renal capsular haemorrhage, "weeping sponge kidney", and to identify associated risk factors and the pathophysiological mechanism behind this condition. MATERIALS AND METHODS: This is a case series in which clinical information for each of the cases was collected retrospectively from electronic patient notes as well as the radiology information and picture archiving and communication systems. RESULTS: Four consecutive cases were included in the series. All of the cases were tre

Urinary proteome in inherited nephrolithiasis.

In the last decades, proteomics has been largely applied to the Nephrology field, with the double aim to (1) elucidate the biological processes underlying renal diseases; (2) identify disease-specific biomarkers, predictor factors of therapeutic efficacy and prognostic factors of disease progression. Kidney stone disease, and in particular, inherited nephrolithiasis (INL) are not an exception. Given the multifactorial origin of these disorders, the combination of genomics and proteomics studies may complement each other, with the final objective to give a global and comprehensive mechanistic v

[Nephronophthisis and medullary cystic kidney disease complex].

BACKGROUND: Nephronophthisis and medullary cystic kidney disease complex refers to the genetic heterogeneous group of inherited tubulointerstital nephritis. Nephronophthisis comprises at last 3 clinical manifestations, has the autosomal recessive pattern of inheritance, appears early in life and is the most frequent inherited kidney disease that causes terminal renal failure in childhood, while medullary cystic kidney disease has the autosomal dominant pattern of inheritance, is less frequent, and terminal renal failure appears later in life. These two forms have similar clinical and morpholog

[A woman with urinary tract infections and flank pain].

A 52-year-old woman presented with recurrent urinary tract infections and flank pain. Both an abdominal CT-scan and a plain abdominal X-ray showed bilateral nephrocalcinosis and a kidney stone in the left ureter. These findings are suggestive of medullary sponge kidneys.

Exploring the relationship of supernumerary recurrent renal calculi formation and tick-borne infections: a case report.

A 51-year-old male with a history of Cacchi-Ricci disease and long-standing infection with various species of Borrelia, Babesia, and Bartonella presented with recurrent symptoms of right-sided flank pain. Numerous renal calculi were identified on imaging. The etiology of the calculi had not been previously elucidated. Symptoms intermittently date back to 2002 when uric acid stones were identified. Subsequent calculi analysis revealed calcium oxalate stones. Despite the commonality of nephrolithiasis in patients with Cacchi-Ricci disease, the extreme number of calculi and recurrent presentation

Hyperechoic medulla of the kidneys.

Eighteen patients were identified in whom ultrasound (US) of the kidney demonstrated a hyperechoic medulla. Diagnoses in the patients included gout in seven; Sjögren syndrome in two; medullary sponge kidney in two; primary aldosteronism in two; and Lesch-Nyhan syndrome, hyperparathyroidism, glycogen storage disease type XI, Wilson disease, and pseudo-Bartter syndrome in one each. The pathogenesis of the echogenicity was evaluated by comparing the findings from computed tomography and conventional radiography. It appears that a hyperechoic medulla is caused by hyperuricemia, medullary nephrocal

Chronic Low Back Pain Masking Medullary Nephrocalcinosis Diagnosed by Point-of-Care Ultrasound.

Low back pain (LBP) is a major cause of disability worldwide and is usually attributed to musculoskeletal etiologies. However, non-musculoskeletal causes should be considered in patients with persistent or atypical symptoms. Renal causes, such as renal calcific disorders, are less frequently considered, especially in the absence of urinary symptoms. Medullary nephrocalcinosis is a rare renal calcific disorder characterized by calcium deposition in the renal medullary parenchyma and tubules. We report the case of a 46-year-old woman with a three-year history of chronic LBP without urinary tract

Non-papillary prone percutaneous nephrolithotomy for renal abnormalities: single-institution experience.

PURPOSE: To evaluate the effectiveness and safety of non-papillary prone PCNL for the treatment of patients with renal abnormalities. METHODS: An observational retrospective cohort study including PCNL cases of patients with renal abnormalities was performed. The following inclusion criteria were applied: renal stones > 1.5 cm with maximal diameter, anatomical malformations of affected kidney (malrotated kidneys, horseshoe kidneys and kidneys with complete duplicated systems, medullary sponge kidney), patients treated with standard (30Fr) PCNL or mini-PCNL (22Fr). The lithotripsy was performe

Urological implications of the beckwith-wiedemann syndrome.

A significant number of children with the Beckwith-Wiedemann syndrome are prone to the development of nephroblastomas and adrenal carcinomas. Renomegaly, a prolonged tubular blush on excretory urography similar to that of medullary sponge kidney, and previously undescribed caliceal diverticula may be observed. Diligent followup is mandatory to detect early neoplasia. Suggestions are detailed based on reported case management.

Lithiasis in cystic kidney disease and malformations of the urinary tract.

The prevalence of renal stones in renal cystic and malformative conditions exceeds the prevalence of renal stones in the general population, suggesting that the above-mentioned cystic and malformative disorders favor stone formation. Urinary stasis is generally assumed to play a major part in the pathogenesis of the nephrolithiasis associated with distorted renal anatomy due to a delayed washout of crystals and risk of urinary infections. However metabolic factors are also important in the pathogenesis of stones in these conditions. Indeed, metabolic abnormalities have been observed in the maj

Medullary cystic kidney disease type 1: mutational analysis in 37 genes based on haplotype sharing.

Medullary cystic kidney disease type 1 (MCKD1) is an autosomal dominant, tubulo-interstitial nephropathy that causes renal salt wasting and end-stage renal failure in the fourth to seventh decade of life. MCKD1 was localized to chromosome 1q21. We demonstrated haplotype sharing and confirmed the telomeric border by a recombination of D1S2624 in a Belgian kindred. Since the causative gene has been elusive, high resolution haplotype analysis was performed in 16 kindreds. Clinical data and blood samples of 257 individuals (including 75 affected individuals) from 26 different kindreds were collect
