Intussusception associated with bacterial meningitis.

Despite its common association with viral illnesses, intussusception has only rarely been found in the presence of bacterial infections. Two infants are described, both of whom were admitted to hospital with bilious vomiting, drowsiness, and dehydration. Both infants required urgent intravenous volume expansion. Intussusception was confirmed, and reduction was achieved by enema in both cases. Recovery was slow, and one infant developed a seizure. Evidence of meningococcal meningitis was found in both, with septicaemia in one. Neurological outcome is normal to date, and there has been no recurr

Cognitive outcome after bacterial meningitis.

OBJECTIVE: To evaluate long-term cognitive deficits in unselected patients with previously diagnosed meningitis and to compare these deficits to neurologic and psychopathologic impairment. PATIENTS AND METHODS: Twenty-two unselected patients (mean age 52.5 +/- 17.1 years) were examined neurologically, psychiatrically, and psychometrically 30 +/- 11 months after the acute stage of bacterial meningitis. Results of psychometric tests were compared with clinical long-term deficits. Psychometric tests were additionally applied on 17 healthy controls (mean age 49.2 +/- 14.2 years). RESULTS: Neurol

Bacterial meningitis and meningococcal infection.

Now that invasive disease caused by Haemophilus influenzae type b has been largely controlled in the developed world, the principal bacterial pathogen causing meningitis in most countries is the meningococcus. Serogroup B and C strains predominate in most industrialised countries in contrast to the situation in sub-Saharan Africa, where serogroup A meningococcal disease is periodically a problem of massive epidemic proportions. This review focuses on developments in our understanding of the diagnosis and treatment of meningococcal disease, of host factors important in susceptibility to, and se

Hearing loss during bacterial meningitis.

OBJECTIVE: To determine the natural history and pathogenesis of hearing loss in children with acute bacterial meningitis. DESIGN: Multicentre prospective study. SETTING: 21 hospitals in the south and west of England and South Wales. SUBJECTS: 124 children between the ages of 4 weeks and 16 years with newly diagnosed bacterial meningitis. METHODS: Children underwent repeated audiological assessment with the first tests being performed within six hours of diagnosis. By using a combination of otoacoustic emissions, auditory brainstem responses, and tympanometry the differences between cochlea

Bacterial meningitis after MMR immunisation.

Two children developed bacterial meningitis within five days of measles-mumps-rubella (MMR) immunisation. Diagnosis was delayed because symptoms were attributed to the vaccine, although both had a raised C-reactive protein. Fever or rash within five days of MMR vaccination are unlikely to be due to the vaccine and a raised C-reactive protein suggests bacterial infection.

Steroid therapy for bacterial meningitis.

Routine dexamethasone therapy for bacterial meningitis in pediatric patients is controversial. Two experts debated this topic at the 1993 meeting of the Infectious Diseases Society of America. Both experts agreed that for management of Haemophilus influenzae meningitis, dexamethasone significantly reduced sensorineural hearing loss and probably reduced other long-term sequelae. Because relatively few patients with pneumococcal and meningococcal meningitis have been studied, no conclusions could be reached regarding the effectiveness of dexamethasone. Dr. Urs Schaad emphasized the impressive an

[Epidemiology of primary bacterial meningitis].

In industrialized countries, the incidence of community-acquired bacterial meningitis is between 5 and 10 cases/10(5) population/year. The highest age-specific attack rates are recorded between 0 and 2 years of age and can reach 100 cases/10(5) population/year or more in some countries. In this range of age, Streptococcus agalactiae, Listeria monocytogenes and Escherichia coli are responsible for meningitis in neonates whereas Haemophilus influenzae, Neisseria meningitidis and Streptococcus pneumoniae cause meningitis in children older than 1 month. H. influenzae was the leading cause of bacte

Bacterial meningitis in Saudi children.

During the four years period from 1988 to 1991, 50 pediatric patients were diagnosed to have bacterial meningitis, out of a total number of 9057 pediatric admissions at Qatif Central Hospital, Qatif, Saudi Arabia, and 82% were less than two years of age. The causative organisms were isolated in 27 (54%) patients. The bacteria grown included Haemophilus influenzae type B in 8 patients (29.6%), Neisseria meningitidis in 8 patients (29.6%), Streptococcus pneumonia in 6 (22%) patients, and other bacteria in 5 patients (18.5%). Cerebro spinal fluid cultures from twenty three patients (46%) showed n

Movement disorders in bacterial meningitis.

Movement disorders developed in five children, ages 6 to 21 months, during the course of bacterial meningitis caused by Hemophilus influenzae (one), Streptococcus pneumoniae (one), Neisseria meningitidis (one), or Mycobacterium tuberculosis (two). Athetosis, choreoathetosis, and hemiballismus occurred, ranging in duration from hours to months. Cranial computed tomography, performed in four cases, showed no lesion of the basal ganglia. The movements were of such abrupt onset and severity that in four cases they were initially misinterpreted as seizures, and anticonvulsant therapy was contemplat

Functional epidemiology of bacterial meningitis.

The epidemiological behaviour of bacterial meningitis is examined to find clues as to the determinants of the disease. Such an examination has shown that a very limited number of bacteria are able to cause the disease and has suggested some of the virulence characteristics thereof, most notably the presence of certain types of polysaccharide capsules, the predilection of the disease to the first years of life is partially explained by the gradual development of immunity, especially serum antibodies to the capsular polysaccharides. The varying proportion of asymptomatic carriers to those fallin

Cerebral arteritis and bacterial meningitis.

Twelve cases of large- and medium-sized cerebral artery stenosis and/or occlusion associated with bacterial meningitis occurred. Neurological complication due to arterial involvement developed in seven patients on the third and fourth days of illness; in one patient, it developed on the fifth day, and in another it developed on the 14th day. In three cases, this could not be determined. Arterial stenosis is considered primarily to result from arterial spasm due to humoral factors that may be elaborated within the CSF or arterial wall, as in the cases of ruptured aneurysm; and secondarily, from

Persistent pleocytosis in bacterial meningitis.

Persistent pleocytosis of greater than 60 white blood cells/mm3 was commonly seen in children adequately treated for bacterial meningitis. It occurred in 13 of 21 (62%) children with Hemophilus influenzae meningitis and in 2 of 9 (22%) with pneumococcal meningitis. Pleocytosis alone cannot be used as an indication of prolonging therapy; significance of persistent pleocytosis is not known.

Bacterial meningitis in the United States, 1978 through 1981. The National Bacterial Meningitis Surveillance Study.

From 1977 to 1981, 18,642 cases of bacterial meningitis were reported to the Centers for Disease Control. We analyzed data from 27 states with full participation from 1978 through 1981. Hemophilus influenzae was the most frequent cause of bacterial meningitis (48.3%), followed by Neisseria meningitidis (19.6%) and Streptococcus pneumoniae (13.3%). Overall attack rates for males were greater than for females (3.3 v 2.6 cases per 10(5) population per year). Attack rates were highest in children under 1 year of age (76.7 per 10(5) population per year). Case-fatality ratios were highest for gram-n

Serum procalcitonin in bacterial & non-bacterial meningitis in children.

BACKGROUND: Bacterial meningitis is a paediatric emergency with high mortality and morbidity requiring prompt diagnosis and treatment. Clinically, it is often difficult to differentiate between bacterial and non-bacterial meningitis. Several studies have demonstrated the raised values of serum procalcitonin (PCT) in bacterial infections including meningitis but without definite cut-off guidelines. Hence, this study was done to evaluate serum PCT as a marker to differentiate bacterial and non-bacterial meningitis in children and assess its efficacy. METHODS: It was a cross-sectional study done
