After the first description of meniscus repairs in the literature it took nearly a century until they were routinely performed in orthopaedic surgery. The scientific basis for meniscus reconstruction needed to be elaborated, including the study of the anatomy, and for instance the vascularisation and the function of the meniscus. After the first experiences with open repair, several arthroscopic techniques have been developed. It has been shown that several factors could influence the healing rate of meniscus lesions. One of the major factors is knee stability. Meniscus repairs performed in co

Posterior medial meniscal root tears and meniscal extrusion have come under increasing focus in recent years, with management that prevents or delays advancing degenerative changes in the medial compartment being the ultimate goal. Although meniscal root repair can restore meniscal function and slow degeneration, it does not eliminate osteoarthritis progression, with conversion to total knee arthroplasty reported as high as 33%. Augmenting posterior medial meniscal root repair with centralization techniques appears to reduce short-term postoperative extrusion and may mitigate early radiographi

BACKGROUND: The knee meniscus has limited intrinsic repair and regenerative capacity. Safe and effective biotherapeutic products remain lacking, and enhancing cell migration and angiogenesis remains a major challenge. This study aimed to develop a novel self-assembled meniscus-specific extracellular matrix membrane (saECM-M) and evaluate its performance in scaffold-free meniscus repair and scaffold-guided whole-meniscus reconstruction, in comparison with chemically processed decellularized ECM (cpd-ECM).
METHODS: Meniscal fibrochondrocytes (MFCs) were cultured under membrane-inducing conditio

Medial meniscus extrusion (MME) is the displacement of the meniscus beyond the edge of the medial tibial plateau. Proposed etiologies for MME include biologic degeneration of meniscal tissue, the result of a medial meniscus posterior root tear (MMPRT), and the failure of the meniscotibial ligament. Standard MMPRT repairs do not address this MME, as it often persists or worsens postoperatively, even with successful root healing. New techniques, such as centralization sutures, have been developed to supplement the standard MMPRT repair technique. Systematic reviews and randomized controlled tria

Meniscal injuries, particularly in the avascular zone, have a low propensity for healing and are associated with the development of osteoarthritis. Current meniscal repair techniques are limited to specific tear types and have significant risk for failure. In previous work, we demonstrated the ability of meniscus-derived matrix (MDM) scaffolds to augment the integration and repair of an in vitro meniscus defect. The objective of this study was to determine the effects of percent composition and dehydrothermal (DHT) or genipin cross-linking of MDM bioscaffolds on primary meniscus cellular respo

INTRODUCTION: The ring-shaped lateral meniscus is very rare. Although it is essentially known as a congenital anomaly, a central tear in an incomplete discoid meniscus or an old bucket-handle tear in a meniscus may be easily mistaken for a ring-shaped meniscus. We experienced a ring-shaped lateral meniscus that regenerated after partial resection of a discoid meniscus together with anterior cruciate ligament (ACL) reconstruction.
PRESENTATION OF CASE: A 37-year-old female patient still experienced unrelenting knee pain 6 months after ACL reconstruction and partial meniscectomy of a discoid la

Meniscal root tears remain a common problem, with devastating biomechanical and clinical consequences. Thankfully, numerous techniques have been developed to repair the symptomatic meniscal root tear. However, rates of conversion to arthroplasty are reported to be 21% to 33% at 10 years, and persistent extrusion of the meniscus at follow-up is a known limitation of current root repair techniques. There is also growing evidence that some medial meniscal root tears may be an effect of meniscal extrusion, rather than the cause of it. In that vein, failure to correct extrusion may be a key mechani

The proper function and longevity of the knee joint are ensured by the knee menisci. Their susceptibility to damage and injury is one of the main risk factors for rapid cartilage loss and the development of osteoarthritis. The vascularization pattern and nutritional status of a torn meniscus determine its potential for healing and the success of meniscus surgery. Blood supply is a crucial factor in assessing healing potential. Knee cartilage volume loss and its modification often result from meniscal damage or excision, leading to osteoarthritis. Modern methods for preserving meniscal tissue a

The distribution of differential extracellular matrix (ECM) in the lateral and medial menisci can contribute to knee instability, and changes in the meniscus tissue can lead to joint disease. Thus, deep proteomic identification of the lateral and medial meniscus cartilage is expected to provide important information for treatment and diagnosis of various knee joint diseases. We investigated the proteomic profiles of 12 lateral/medial meniscus pairs obtained from excess tissue of osteoarthritis patients who underwent knee arthroscopy surgery using mass spectrometry-based techniques and measured

BACKGROUND: Aging is the most prominent risk factor for the development of osteoarthritis (OA), which affects knees and causes major health burdens. Meniscal dysfunction mostly based on degeneration contributes to the development and progression of knee OA. Meniscal degeneration is caused by various extrinsic factors, such as repetitive trauma or leg malalignment, while meniscal aging is considered as internal changes, such as molecular or cellular changes. Little is known about age-related changes in the meniscus. Therefore, this review aimed to summarize and clarify the understanding of the

PURPOSE: To summarize and discuss the current knowledge on posterior lateral meniscus root tears.
METHODS: A comprehensive review of the MEDLINE database was carried out to identify relevant articles using different keywords (e.g. "meniscus root", "root tear", "meniscus avulsion", "radial tear" and "lateral meniscus"). The reference lists of the reviewed articles were searched for additional relevant articles.
RESULTS: Posterior lateral meniscus root tears are found in 7-12% of patients with a tear of the anterior cruciate ligament (ACL). Biomechanical studies have found an increase in later

Tears of the posterior root of the medial meniscus are becoming increasingly recognized. Early identification and treatment of these tears help halt the progression of cartilage degeneration and osteoarthritis of the knee. Repair of these tears is essential for recreating the hoop stress of the medial meniscus. In this note, we describe a successful arthroscopic technique to repair this lesion. A posteromedial portal is established by which two 2-0 PDS sutures are placed through the meniscus root and pulled down through a trans-tibial tunnel and fixed using an EndoButton distally along the ant

This review details current efforts to tissue engineer the knee meniscus successfully. The meniscus is a fibrocartilaginous tissue found within the knee joint that is responsible for shock absorption, load transmission, and stability within the knee joint. If this tissue is damaged, either through tears or degenerative processes, then deterioration of the articular cartilage can occur. Unfortunately, there is a dearth in the amount of work done to tissue engineer the meniscus when compared to other musculoskeletal tissues, such as bone. This review gives a brief overview of meniscal anatomy, b

We report 2 cases of an abnormal band of the lateral meniscus of the knee joint. A 23-year-old man complained of mechanical symptoms in both knees and an arthroscopic procedure was performed. We found meniscal abnormalities in the both knees. A 20-year-old man sustained a posterior cruciate ligament injury. In the course of an arthroscopy for his torn posterior cruciate ligament, an abnormal band of the lateral meniscus was seen. It was characteristic that the upper abnormal band was loose and serpentine and connected at the posterior horn and the middle segment of the lower normal lateral men

Implants of polyurethane-coated polyester (Dacron), polyurethane-coated PTFE (Teflon), or uncoated PTFE were used to substitute the medial rabbit meniscus, and were observed over a 3-month period. Joint mechanics were similar for sham-operation, meniscectomy, or substitution with a coated implant. Joints with an uncoated PTFE prosthesis tended to be less compliant and to show greater load dissipation during relaxation. Cartilage indentation showed softening with all alternatives compared to sham-operation. Ingrowth into the prosthesis periphery was best with the PTFE implants. Both types of co

A prospective study of arthroscopically repaired peripheral meniscal tears in 24 patients (19 men and five women) was initiated in 1983. Two patients were lost to follow-up. Seventeen medial and five lateral tears were followed an average of 29 months (15-42 months) with 17 having clinically apparent healing (77%). Sixteen had ACL tears, 10 of which were stabilized. Thirteen of 16 stable knees healed their menisci (81%), whereas only 4 of 6 unstable knees had healed menisci (67%). Fifteen were acute tears repaired within 2 weeks of injury, and 7 were chronic tears. Four acutely repaired menisc

The meniscus is a fibrocartilaginous structure crucial for load distribution, shock absorption and knee stability. With advancing age, its biomechanical and biochemical integrity declines through progressive cellular senescence, accumulation of advanced glycation end-products, reduced vascularity and altered collagen cross-linking. These changes predispose to structural damage most often seen as horizontal cleavage lesions-the hallmark of meniscal ageing-best understood as 'wrinkles' of the meniscus rather than true pathology. Such lesions are often asymptomatic and incidental on MRI. However,
