Mesenteric Vessel-Preserving Laparoscopic Surgery for Pediatric Unicentric Castleman Disease in the Transverse Mesocolon.

INTRODUCTION: Castleman's disease is a rare lymphoproliferative disorder. Unicentric Castleman disease (UCD) is usually benign and can be treated by complete resection. We herein report a pediatric case of UCD that occurred in the transverse mesocolon and was successfully treated with a laparoscopic approach. CASE PRESENTATION: The patient was a 10-year-old girl at the time of the surgery. She was referred to our hospital with a persistent fever of unknown origin and high C-reactive protein levels. Abdominal enhanced MRI showed a 38 × 25-mm enhanced mass in the left transverse mesocolon, and

Laparoscopic Sigmoidectomy for Sigmoid Cancer With Persistent Descending Mesocolon: Anatomical Characteristics and Technical Tips.

Although rare, persistent descending mesocolon (PDM) is an anatomical anomaly that carries potential risks for laparoscopic colorectal surgery. Impaired blood circulation of the reconstructed colon is especially risky during surgery. We report a case of sigmoid cancer with PDM, in which the patient underwent laparoscopic sigmoidectomy. A 52-year-old man diagnosed with sigmoid cancer was referred to our hospital. PDM was identified with preoperative enhanced-contrast computed tomography, which revealed the sigmoid colon located in the right lower quadrant and a bear-claw inferior mesenteric art

Acute abdominal pain due to sigmoid volvulus with persistent descending mesocolon: a case report.

BACKGROUND: Persistent descending mesocolon, an anomaly of fixation of the mesentery of the descending colon, can sometimes cause complications such as intestinal obstruction and intussusception. We present the first reported case of sigmoid volvulus with persistent descending mesocolon. CASE PRESENTATION: An 82-year-old Japanese man had intermittent lower abdominal pain. Abdominal computed tomography showed dilation and a shift to the right side of the sigmoid colon, but no findings of volvulus. The next day, he presented continuous lower abdominal pain with bloody stool. A second abdominal

Giant Cystic Lymphangioma of the Transverse Mesocolon in a 7-Year-Old Child: A Case Report.

BACKGROUND: Cystic lymphangiomas are rare benign lymphatic malformations that are usually diagnosed in early childhood. Intra-abdominal lesions are uncommon, and involvement of the transverse mesocolon is particularly rare. CASE PRESENTATION: A 7-year-old girl presented with a 3-4 week history of vomiting, abdominal pain, progressive abdominal distension, poor appetite, abdominal fullness, generalized weakness, and irregular bowel movements. Examination revealed a large, well-defined, irregular, non-tender abdominal mass measuring approximately 12 × 10 x 12 cm and extending from the right fla

A Rare Case of Asymptomatic Insulinoma with Mesocolonic Lymph Node Metastases and Long-Term Stability.

INTRODUCTION: Malignant insulinomas are rare, and lymph node metastases are particularly uncommon in small, low-grade tumors. We report an asymptomatic insulinoma of the pancreatic body with multiple lymph node metastases confined to the transverse mesocolon, which remained radiologically stable for at least 3 years prior to the diagnosis. This case highlights the potential for metastatic disease, even in indolent insulinomas. CASE PRESENTATION: A 75-year-old man was referred after repeated findings of low fasting glucose levels during annual health screenings over 3 years. Despite persistent

Hydatid cysts in the mesocolon mimicking peritoneal pseudomyxoma in a post-appendectomy patient: A case report.

We aimed to present a case of two mesocolonic hydatid cysts that mimicked the presentation of peritoneal pseudomyxoma. Hydatidosis is a zoonotic parasitic infection caused by the cestode Echinococcus spp., whose larval stage affects various organs. The present case describes a 40-year-old male patient who presented with severe lower abdominal pain and was diagnosed with acute appendicitis. The patient underwent an appendectomy and was later referred to an oncology surgery clinic because of imaging findings suggestive of peritoneal pseudomyxoma or carcinomatosis. A video-assisted laparoscopic p

Laceration of the transverse mesocolon in an old man with a habit of abdominal massage for constipation: a case report.

BACKGROUND: Abdominal massage for the resolution of constipation has been reported to be safe and recommended in some studies. It is conventionally performed for the elderly suffering from intractable constipation. Meantime, isolated mesenteric injury after blunt abdominal trauma is uncommon. Here, we report a case of isolated mesenteric injury following self-abdominal massage for constipation. CASE PRESENTATION: A 68-year-old man consulted a local hospital due to a sudden abdominal pain. He had a history of prostate cancer treated with radiation therapy 3 years ago, and he had been suffering

A colovesical fistula with a persistent descending mesocolon due to partial situs inversus: A case report.

INTRODUCTION: Situs inversus viscerum, a congenital condition in which the visceral organs are a mirror image of their normal physiological positions, could be total or partial. Persistent descending mesocolon (PDM) is a congenital anomaly that is asymptomatic because of its short length. PDM causing intestinal obstruction is a known clinical complication. PRESENTATION OF CASE: A 74-year-old woman presented with pneumaturia and enteruria for two months, and recurrent cystitis for a month. An enhanced computed tomography (CT) showed air in the bladder along with sigmoid colonic diverticula adh

Vascular anatomical study of persistent descending mesocolon in patients undergoing laparoscopic surgery for colorectal cancer.

INTRODUCTION: Persistent descending mesocolon (PDM) is a rare congenital atypia of fixation of the descending colon, and currently, very few detailed studies exist on its vascular anatomy. This study was conducted to evaluate the features of the vascular anatomy of PDM to help avoid intraoperative lethal injury and subsequent postoperative complications in laparoscopic colorectal surgery. METHODS: We retrospectively analyzed the data of 534 patients who underwent laparoscopic left-sided colorectal surgery. PDM was diagnosed using preoperative axial computed tomography (CT) view. The vascular

Extended Versus Standard Complete Mesocolon Excision in Sigmoid Colon Cancer: A Multicenter Randomized Controlled Trial.

OBJECTIVE: The aim of this study was to evaluate whether extended complete mesocolic excision (e-CME) for sigmoid colon cancer improves oncological outcomes without compromising morbidity or functional results. BACKGROUND: In surgery for cancer of the sigmoid colon and upper rectum, s-CME removes the lymphofatty tissue surrounding the inferior mesenteric artery (IMA), but not the lymphofatty tissue surrounding the portion of the inferior mesenteric vein that does not run parallel to the IMA. Evidence about the safety and efficacy of extending CME to include this tissue is lacking. METHODS: T

Involvement of transverse mesocolon is associated with development of colonic fistula in patients with acute necrotizing pancreatitis.

BACKGROUND: Involvement of transverse mesocolon (TM) during acute necrotizing pancreatitis(ANP) indicates that inflammation has spread from retroperitoneal space to peritoneum. Nevertheless, the impact of TM involvement, as confirmed by contrast-enhanced computed tomography (CECT), on local complications and clinical outcomes was poorly investigated. PURPOSE: This study aimed to explore the association between CECT-diagnosed TM involvement and the development of colonic fistula in a cohort of ANP patients. METHODS: This is a single-center, retrospective cohort study involving ANP patients ad

Intra-abdominal cystic lymphangioma of the mesocolon sigmoids: a rare entity in adult patient woman.

The lymphangioma is a benign neoplasm, mostly connate, it occurs more often in children. It is more frequently located in the head-neck region, as well as in the armpit, and less frequently in the abdomen. The diagnosis, as well as their appearance is variant, they can be asymptomatic depending on the size. The signs on the physical exam are usually nonspecific and can look like an acute abdomen. Imaging tests used are the ultrasound and tomography, which determine the therapeutic behavior. The differential diagnosis should be made with other intra-abdominal injuries, such as cavernous hemangi

Four cases of laparoscopic colectomy for sigmoid colon and rectal cancer with persistent descending mesocolon.

BACKGROUND: Persistent descending mesocolon (PDM) is a congenital anomaly associated with the failure of fixation of the descending colon to the lateral abdominal wall. In the laparoscopic colectomy for colorectal cancer, it has been noticed that there are extensive adhesions and a distinctive anatomy of colonic vessels in cases with PDM. Therefore, it is necessary to have sufficient knowledge about PDM so that it can be appropriately treated during surgery. CASE PRESENTATION: Case 1-a 79-year-old man underwent laparoscopic intersphincteric resection for rectal cancer. Preoperative barium ene

Surgical techniques for advanced transverse colon cancer using the pincer approach of the transverse mesocolon.

BACKGROUND: Laparoscopic surgery for colorectal cancer, not only early cancer but also advanced cancer, has become standardized by some randomized controlled studies. However, cases involving advanced transverse colon cancer were excluded from these studies due to the technical difficulty of the surgery. Hence, laparoscopic surgery for advanced transverse colon cancer is still a theme that we need to overcome. To solve these issues, it is necessary to establish a standardized approach and surgical technique. SURGICAL TECHNIQUES: The advantage of our method, which approaches from both sides of

A rare case of solitary fibrous tumour of the sigmoid mesocolon: imaging features and review of literature.

Solitary fibrous tumours are rare spindle cell tumours that generally arise from the pleura but on rare occasions arise in other locations such as the head and neck region, stomach and retroperitoneum. Very few reports exist on the imaging findings of these tumours in extra pleural sites such as the mesentery. We report the computed tomography (CT) imaging findings of a rare case of solitary fibrous tumour in a 68-year-old man who presented with a slowly enlarging painless pelvic mass. CT scan showed a well-defined, multilobulated, highly vascular pelvic mass with dense calcifications and cent

Internal hernia through an idiopathic transverse mesocolon defect treated by laparoscopic surgery: a case report.

INTRODUCTION: Internal hernia is a rare cause of intestinal obstruction, and transverse mesocolon hernia (TMH) is an extremely rare subtype of internal hernia. PRESENTATION OF CASE: We report the successful treatment of a TMH through an idiopathic transverse mesocolon defect (iTMD). A 50-year-old man was rushed to hospital with sudden onset of abdominal pain. Computed tomographic imaging showed an internal hernia with small bowel obstruction in the right upper abdomen. He underwent emergency surgery for suspected strangulated intestinal obstruction caused by internal hernia. Laparoscopic surg

Internal hernia through a transverse mesocolon defect after laparoscopic distal pancreatectomy: Report of a case.

We report a case of an internal hernia through a transverse mesocolon defect after laparoscopic distal pancreatectomy. The patient was a 58-year-old man with an intraductal papillary mucinous neoplasm of the pancreatic body who underwent laparoscopic distal pancreatectomy. During surgery, an approximately 5-cm defect in the transverse mesocolon was inadvertently made. The defect was not closed as it was thought to be large enough to preclude incarceration. However, the patient developed a bowel obstruction 2 months postoperatively. Laparotomy revealed that a loop of the proximal jejunum hernia
