Pancreatic resections for advanced M1-pancreatic carcinoma: the value of synchronous metastasectomy.

BACKGROUND: For M1 pancreatic adenocarcinomas pancreatic resection is usually not indicated. However, in highly selected patients synchronous metastasectomy may be appropriate together with pancreatic resection when operative morbidity is low. MATERIALS AND METHODS: From January 1, 2004 to December, 2007 a total of 20 patients with pancreatic malignancies were retrospectively evaluated who underwent pancreatic surgery with synchronous resection of hepatic, adjacent organ, or peritoneal metastases for proven UICC stage IV periampullary cancer of the pancreas. Perioperative as well as clinicopa

11-year survival of a renal cell cancer patient following multiple metastasectomy.

CASE REPORT: A renal cell cancer patient with late onset of multiorgan metastases showed an unusually long survival following surgical resection. Femoral metastasis appeared 11 years, and contra lateral kidney and adrenal gland metastasis 19 years after the primary nephrectomy, respectively. Following the resection of the femur and implantation of endoprosthesis and removal of adrenal gland and partial nephrectomy, the patient was disease-free 20 years after the first diagnosis of cancer. CONCLUSION: The long survival and successful treatment underline the importance and efficiency of radical

Prognostic factors and long-term results of pulmonary metastasectomy for pediatric histologies.

OBJECTIVE: Pulmonary resection in metastatic pediatric solid tumors is an accepted method of treatment. The purpose of this study was to determine the clinical course, outcome and prognostic factors after surgery. METHODS: A retrospective analysis from 1985 to 2006 of 52 patients less than 17 years old at the time of tumor diagnosis and submitted to thoracotomy for pulmonary metastatic disease was performed. Three had nodules excised which proved to be benign at histology and were excluded from further analysis. There were 28 males and 21 females with median age of 13.2 years [2-36] at the ti

Impact of Metastasectomy on Patient-Reported Outcomes and Health-Related Quality of Life.

This article explores the impact of pulmonary metastasectomy on patient health-related quality of life, emphasizing the role of patient-reported outcome measures (PROMs) in evaluating surgical outcomes. It provides a detailed analysis of studies reporting PROMs in lung metastasectomy, compares different surgical techniques, and discusses the broader implementation of PROMs in this setting. Limitations and future directions for improving PROMs integration in clinical practice are also addressed, highlighting the importance of these measures in enhancing patient-centered care and shared decision

Time to Incurable Recurrence for Patients Treated With Pulmonary Metastasectomy for Colorectal Cancer.

BACKGROUND: Probability of cure is important for patients with lung metastasis who must decide whether to undergo metastasectomy. Although progression-free survival (PFS) is thought to reflect this, it does not include curative effects by repeat metastasectomy. Thus, the authors developed a new indicator, time to incurable recurrence (TTIR), in which only incurable recurrence was set as an event that included death, with incurable recurrence defined as recurrence not treated by definitive local therapy (DLT), recurrence treated by DLT but with PFS maintained less than 2 years, or recurrence fo

Role of pulmonary metastasectomy in colorectal cancer in the era of modern multidisciplinary therapy.

PURPOSE: To clarify the role of pulmonary metastasectomy in colorectal cancer in the era of modern multidisciplinary therapy. METHODS: The characteristics and outcomes of the patients who underwent pulmonary metastasectomy for colorectal cancer through 2002 (n = 26) and from 2003 (n = 68) were compared. RESULTS: The patients treated from 2003 had a smaller tumor size and more frequently had a history of extra-pulmonary relapses than did those treated through 2002. There was a significant improvement in the 5-year overall survival (42.0% vs. 73.1%, p = 0.03) but not the 5-year relapse-free su

Surgical management of ovarian carcinoma metastatic to the breast and axilla: A role for metastasectomy?

As patients with ovarian cancer are living longer, surgeons will be faced with the management of metastatic lesions amenable to resection more frequently. We present two patients with ovarian cancer metastatic to the breast who underwent resection for their metastases, but their outcomes differed significantly. After reviewing the literature, we propose that there can be definite benefit to resection in the appropriately selected patients and discuss factors to consider prior to proceeding with surgery.

Metastatic adrenocortical carcinoma: results of 56 pulmonary metastasectomies in 24 patients.

BACKGROUND: Surgical resection is an important form of treatment for metastatic disease in patients with adrenocortical carcinoma (ACC). However, data about the results of this treatment are sparse. We reviewed our experience with the resection of pulmonary lesions metastatic from ACC as a means of evaluating such results. METHODS: A retrospective review of the database at a German national registry for ACC identified 24 patients (9 men and 15 women; median age, 41 years) who underwent pulmonary metastasectomy for primary ACC during the study period of 1989 through 2009. Only patients who met

Effects of mediastinal lymph node dissection in colorectal cancer-related pulmonary metastasectomy.

BACKGROUND: The benefits of mediastinal lymph node dissection (MLND) in colorectal cancer-related pulmonary metastasectomy (PM) have been poorly reported. This study aimed to determine whether MLND affects survival in patients undergoing PM and to identify the prognostic factors for survival. METHODS: We retrospectively reviewed 275 patients who had undergone colorectal cancer-related PM from January 2010 to December 2016. MLND was defined as the resection of at least six mediastinal lymph node stations according to the International Association for the Study of Lung Cancer criteria (N1, ≥3 s

Complete metastasectomy for renal cell carcinoma: Comparison of five solid organ sites.

BACKGROUND AND OBJECTIVES: Patients with metastatic RCC can undergo metastasectomy to improve survival time. Our goal was to provide and compare characteristics and oncological outcomes of RCC patients who underwent complete metastasectomy at a single organ site. METHODS: A total of 138 RCC patients were identified as undergoing complete metastasectomy at a single organ site including adrenal, lung, liver, pancreas, or thyroid. Competing risk regression analysis was used to assess RFS and CSS adjusting for several covariates. RESULTS: In this highly selected cohort, RFS and CSS was 27% and 8

Non-occlusive mesenteric ischemia after splenic metastasectomy for small-cell lung cancer.

A 68-year-old man presented with severe abdominal pain. Seven months earlier, he had received systemic chemotherapy for small-cell lung cancer with solitary metastasis to the spleen, followed by splenectomy. Abdominal computed tomography and abdominal arterial angiography showed diffuse ischemia of the mesenteric artery without apparent occlusion. The patient also suffered from septicemia caused by Enterococcus faecium. Therefore, a diagnosis of non-occlusive mesenteric ischemia (NOMI) induced by septicemia was supposed. Although treatment with antibiotics and papaverine hydrochloride was admi

Should mediastinal lymphadenectomy be performed during lung metastasectomy of renal cell carcinoma?

A best evidence topic was constructed according to a structured protocol. The question addressed was whether radical mediastinal lymphadenectomy should be performed during lung metastasectomy of renal cell carcinoma (RCC). Of the 13 papers found through a report search, seven represent the best evidence to answer this clinical question. The authors, journal, date, country of publication, study type, group studied, relevant outcomes and results of these papers are given. We conclude that on the whole, the seven-retrieved studies support the realization of systematic radical mediastinal lymphade

Hand-assisted thoracoscopic surgery for bilateral lung metastasectomy through sternocostal triangle access.

BACKGROUND: Complete resection of pulmonary metastases remains the standard of care for selected patients. An open approach causes criticism because of aggressiveness. Without manual palpation the video-assisted thoracic surgery procedure has the high probability of missing occult metastases. Transxiphoid bilateral lung metastasectomy surgery has not been widely adopted. We developed a hand-assisted thoracoscopic surgery for bilateral lung metastasectomy through natural sternocostal triangle access in 2001 to solve the problems mentioned previously. METHODS: We retrospectively evaluated 55 co

Prediction of prognosis and surgical indications for pulmonary metastasectomy from colorectal cancer.

BACKGROUND: Treatment of pulmonary metastases from colorectal cancer by excision has increased rapidly, but reports on indications and prognostic factors are inconsistent. We sought to identify poor prognostic factors preoperatively and to retrospectively evaluate preoperative clinical indications for surgery. METHODS: A total of 75 patients with colorectal cancer had pulmonary metastases excised from 1986 to 2003. Tumor size, number, laterality, hilar or mediastinal lymphadenopathy, and carcinoembryonic antigen level were possible risk factors for metastatic tumors, with primary site of colo

Efficacy of Metastasectomy for Metastatic Bladder Cancer: A Systematic Review and Meta-Analysis.

OBJECTIVES: It has been proposed that metastasectomy may cure some patients with a single metastasis. Hence, we considered it necessary to clarify the role of metastasectomy for metastatic bladder cancer (BCa). METHODS: We conducted a systematic review of published reports on the efficacy of metastasectomy for different sites of BCa metastasis. We searched English articles published before 7 June 2025 in three electronic databases: PubMed, Embase, and Cochrane Library. We then extracted authors, year of publication, country in which the study was conducted, study institution, study design, su
