Syndesmosis procedure: a non-osteotomy approach to metatarsus primus varus correction.

BACKGROUND: Hallux valgus and metatarsus primus varus deformities usually are the result of failure of the supporting soft tissues rather than bone deformities. Since soft-tissue procedures have been shown to only be suitable for mild deformities, first metatarsal osteotomy has become an integral part of correcting moderate to severe deformities. A soft-tissue technique referred to as the "syndesmosis procedure" by the author was evaluated for its effectiveness in correcting metatarsus primus varus in feet with hallux valgus. METHODS: This is a retrospective clinical and radiographic study of

Metatarsus primus varus: including various clinicoradiologic feautres of the female foot.

Metatarsus primus varus occurs in feet which have a normal 1--2 intermetatarsal angle. Fifty-two Caucasian females, average age 18 years, were analyzed radiologically for 1--2 and 1--5 intermetatarsal angles and for metatarsophalangeal and first interphalangeal joint angles. Measurement of the 1--5 rather than the 1--2 intermetatarsal angel is a more reliable indicator of first metatarsal varus. Valgus at the first interphalangeal joint is normally greater than that at the first metatarsophalangeal joint and excessive first interphalangeal valgus (valgus interphalangeus) is significantly linke

Correction of hallux valgus and metatarsus primus varus. Using the Cedell technique.

The Cedell procedure consists of: a proximal valgus osteotomy of the first metatarsal bone to correct the metatarsus primus varus, a soft tissue plasty at the first metatarsophalangeal joint with lateral release, and tightening of the medial capsule to correct the hallux valgus. A total of 45 operations in 31 patients (2 men and 29 women) were performed. Forty-one of these operations were followed. The average age at surgery was 24 years (range 16 to 43 years), and the average follow-up time was 23 months (range 8 to 56 months). The postoperative decrease in the intermetatarsal angle averaged

The pathology of congenital metatarsus varus. A post-mortem study of a newborn infant.

A case of congenital metatarsus varus from an autopsy of a newborn infant was explored. Contracture of the anterior tibialis muscle was present as well as alterations in size and shape of the first cuneiform bone. Furthermore, subluxations in the fore- and midfoot were observed, especially round the first cuneiform bone. It is concluded that congenital metatarsus varus may be classified as a subluxation followed by secondary bone changes and contractures in the soft tissues.

Instrumented correction of metatarsus adductus with hallux valgus - A multicenter radiographic assessment.

Failure to address metatarsus adductus (MTA) with coexisting hallux valgus (HV) has resulted in mixed outcomes and increased deformity recurrence. The purpose of this study was to investigate early radiographic and clinical outcomes of a novel instrumented 2nd and 3rd TMT corrective arthrodesis combined with instrumented triplanar 1st TMT arthrodesis with early weightbearing. Radiographs and medical records from subjects undergoing this surgical approach were retrospectively reviewed. Forty-three subjects (N = 43 feet) with a mean ± SD age of 41.6 ± 14.2 (range 15 to 62) years were treated at

A Simplified Preoperative Radiographic Assessment for Metatarsus Adductus Associated With Hallux Valgus.

Radiographic measurements for the assessment of metatarsus adductus (MTA) have a broad range of interpretation without a consensus regarding surgical indications. The "Plumbline" (PL) radiographic assessment method helps identify MTA and determines if physical space is available to align the first metatarsal to the longitudinal foot axis without the need to realign the lesser metatarsals. Forty-five neutral weight-bearing anterior-posterior (AP) radiographs of patients scheduled for surgical intervention for isolated hallux valgus (HV) or combined MTA/HV deformities were reviewed. The cohort w

Correction of Hallux Valgus Deformity in Association With Underlying Metatarsus Adductus Deformity.

UNLABELLED: Presence of underlying metatarsus deformity is known to negatively affect the outcome of hallux valgus corrective surgery. Yet there are many other factors that also affect final outcomes. In order to address the potential relationships between variables, we attempted to evaluate the association of metatarsus adductus with surgical outcomes, independent of other common factors used in evaluation of hallux valgus deformity. We enrolled 154 patients who had undergone hallux valgus corrective surgery and recorded their pre- and postoperative intermetatarsal (IM12) and hallux valgus (H

Foot scan assessment of metatarsus adductus: A useful adjunct to Bleck's classification.

UNLABELLED: To determine the severity of metatarsus adductus (MA) comparing with Bleck's classification as a commonly acceptable method for assessing MA, static foot scan has been used. In this cross-sectional descriptive research study, 100 subjects were equally divided into four groups according to Bleck's classification. The feet were scanned and MA severity (MAS) index was measured on the obtained foot scan images. The MAS index was the ratio of the transverse deviation of the forefoot from the lateral border heel line to the width of the ball of the foot. The mean of the MAS index in norm

[A rare shortening of the first metatarsus of the foot and its treatment with distraction arthrodesis].

Multiple toes may develop on the medial or lateral side of the forefoot or in its middle. This may involve duplication of only the distal phalanx or the whole hallux including the metatarsal. In a duplicated big toe, the phalanx to be maintained, because short muscles of the leg are attached to it, is usually localized medially. A rare case of dysplasia of the first metatarsus, following a resection indicated in childhood for a hallux triples, is described in a 20-year-old girl. In order to remove hallux insufficiency and an overloading of the middle metatarsals, the authors decided to carry o

The crescentic first metatarsal basilar osteotomy for correction of metatarsus primus varus.

The crescentic basilar first metatarsal osteotomy has been largely abandoned by the podiatric community in recent years in favor of proximal wedge-type osteotomies for the correction of metatarsus primus varus with large intermetatarsal angles. In most cases, this was due to the inherent instability of the osteotomy and difficulties with fixation. However, the crescentic osteotomy has the ability to correct in all three planes with less shortening than the wedge-type osteotomies. New fixation techniques, such as the small cannulated screw systems, have allowed for less technical difficulty in

The Evans calcaneal osteotomy: postoperative care and an evaluation on the metatarsus adductus angle.

A retrospective study of two aspects of the Evans calcaneal osteotomy with bone graft is presented. First, the postoperative casting time can be decreased significantly to 3 weeks of nonweightbearing and 3 weeks of weightbearing. Second, the metatarsus adductus angle remains relatively unaltered after the procedure.

Metatarsus adductus and selected radiographic measurements of the first ray in normal feet.

Radiographic evaluation of hallux abducto valgus frequently involves the measurement of the metatarsus adductus angle, first-second intermetatarsal angle, hallux abductus angle, and proximal articular set angle. While the concept that there is a relationship between untreated metatarsus adductus and hallux abducto valgus deformity is not new, a quantifiable relationship between the metatarsus adductus angle and intermetatarsal angle, hallux abductus angle, and the proximal articular set angle in normal feet is relatively undocumented. The purpose of this study is to document relationships betw

The Heyman-Herndon tarsometatarsal capsulotomy for metatarsus adductus: results in 48 feet.

Forty-eight Heyman-Herndon tarsometatarsal capsulotomies were performed over an 18-year period for idiopathic metatarsus adductus and residual adductus of clubfoot. There was a 41% overall failure rate and a 50% incidence of painful dorsal prominence at the surgical scar. Sixty-eight percent of the patients had difficulty with proper shoe fitting. Those with associated orthopedic anomalies were at high risk for a poor result. We believe that tarsometatarsal capsulotomy is of questionable benefit in treatment of forefoot adduction deformities of children.

Metatarsus varus corrected by open wedge osteotomy of the first cuneiform bone.

The persistence of metatarsus adductus varus has been a problem in management. We have treated a series of selected patients with this problem and believe that our results have been better than with the procedure we have used in the past. Fowler has described a procedure which seems ideal for the patients in our series. Through personal communication the procedure and its application were discussed and the series was started eight years ago. Our series is small because our patients are responding to other forms of treatment at an earlier age. The few that do not respond are now considered for

Radiological characteristics of hallux valgus with metatarsus adductus: A matched case-control study.

BACKGROUND: No evidence has been found to support the hypothesis that there is a correlation between hallux valgus (HV) and intermetatarsal (IM) angles in HV with metatarsus adductus (MA) and that IM angle in HV with MA is lower than that in HV without MA. The present study aimed to analyze the radiographic characteristics of HV with MA compared to matched controls and to clarify the differences between HV with MA and without MA. METHODS: Preoperative radiographs of 126 female patients (164 feet) who underwent hallux valgus surgery were reviewed. The HV, IM, and MA angles were measured. The M
