The cutaneous microcirculation: ultrastructure and microanatomical organization.

The cutaneous microcirculation is organized as two horizontal plexuses. One is situated 1-1.5 mm below the skin surface, and the other is at the dermal-subcutaneous junction. Ascending arterioles and descending venules are paired as they connect the two plexuses. From the upper layer, arterial capillaries arise to form the dermal papillary loops that represent the nutritive component of the skin circulation. There are sphincter-like smooth muscle cells at the point where the ascending arterioles divide to form the arteriolar component of the upper horizontal plexus. At the dermal subcutaneous

Modulation of the release and activity of neuropeptides in the microcirculation.

Electrical stimulation of the sensory saphenous nerve leads to neurogenic edema formation in the innervated area of the paw of the anesthetized rat. Evidence suggests that the edema formation is the result of increased microvascular permeability mediated via neurokinin NK1 receptors and increased blood flow mediated via calcitonin gene related peptide CGRP1 receptors. Results indicate that selective receptor antagonists will only inhibit the response mediated by the specific receptor they antagonise. In the case of neurogenic inflammation, where it is common for more than one biologically acti

The damaging potential of leukocyte activation in the microcirculation.

Traditionally leukocytes have been regarded as beneficial cells, owing to their immunologic and antimicrobial activity. Recent evidence suggests, however, an additional role of granulocytes and monocytes as mediators of cardiovascular complications, such as ischemia, reperfusion, diabetes, physiologic shock, venous ulceration, and other conditions. Granulocytes and monocytes have a large volume, and stiff cytoplasm; they have the ability to adhere to endothelium and to other substrates; and they exert several forms of cytotoxicity. Granulocytes and monocytes may be trapped in the microcirculat

[Cardiac function and the skin microcirculation in acromegaly].

The microcirculation was measured by nail-fold capillary video microscopy in 21 patients (12 men, 9 women; mean age 54.7 [29-74] years) with acromegaly. Levels of growth hormone (12.0-71.7 microU/ml) and of somatomedin C (2.4-10.5 IU/ml) were elevated in 10 patients, despite preceding treatment. Eleven patients had an increase in myocardial thickness and nine had impairment of left ventricular function, although only slight in most. Left ventricular hypertrophy was demonstrable even in the absence of hypertension. No patient had evidence of coronary heart disease. Nail-fold capillary video mic

Microcirculation of the juvenile knee in chronic arthritis.

In order to investigate pathogenetic factors in growth abnormalities of the knee in hemophilic arthropathy and juvenile rheumatoid arthritis, the hemodynamic changes of the knee following chronic synovial inflammation and elevated joint pressure were studied in puppies. Unilateral arthritis was induced by intraarticular injections of carrageenan solution. Regional blood flow (RBF) was measured by tracer microsphere technique. Microvascular volume (VV) was estimated from the distribution volumes of Cr51-labelled erythrocytes and I125 fibrinogen. Mean transit times (TT) of blood components were

Role of the microcirculation in the treatment and pathogenesis of psoriasis.

Controversy exists whether the initiating events in psoriasis are primarily epidermal or dermal (vascular). To study this point, serial biopsies from 6 patients were taken from the periphery of individual plaques before, and at 1 to 3 day intervals during, Goeckerman and PUVA treatments. Part of the biopsy was studied by electron microscopy to determine the fine structure of the capillary loops and part was incubated with tritiated thymidine to determine the labeling index (LI) of the basal cells. Normal appearing buttock skin of the 11 other psoriatic patients not under treatment was studied

Effect of ethaverine hydrochloride on cochlear microcirculation.

The effect of ethaverine hydrochloride on cochlear microcirculation was observed in short-term experiments on 11 guinea pigs. In these experiments, the endocochlear PO2, endocochlear potential (EP), cochlear microphonic (CM) potential, and blood pressure (BP) were recorded. Endocochlear PO2 was measured polarographically by gold electrodes in the scala media. Ethaverine hydrochloride was administered by intravenous infusion for ten minutes in doses of 2, 5, and 10 mg/kg. These experiments demonstrated that ethaverine hydrochloride in doses of 5 and 10 mg/kg caused significant elevation of endo

The effect of gastrointestinal hormones on the gastric microcirculation.

The purpose of the present study was to evaluate the effects of several gastrointestinal hormones on the gastric submucosal arterioles using an in vivo microscopy technique. Alteration in diameter of submucosal arterioles, 40 to 90 mu in diameter, in response to infusion of different agents into the celiac axis, was measured in the anesthetized cat. Pentagastrin, the octapeptide of cholecystokinin, natural secretin, and histamine produced arteriolar dilation. Only with histamine and the octapeptide of cholecystokinin did this occur with doses that might be considered physiological. Synthetic s

Microcirculation Monitoring in Septic Shock: Focused Review.

Background and Objectives: Septic shock is marked by profound circulatory and cellular dysfunction, with mortality rates of 25-40% despite guideline-based resuscitation. Normalization of macrohemodynamic variables often fails to restore tissue perfusion, a concept known as hemodynamic incoherence. Persistent microcirculatory dysfunction is associated with organ failure and poor outcomes, underscoring the limitations of systemic monitoring alone. This focused narrative review synthesizes current evidence on microcirculatory monitoring in septic shock, with emphasis on bedside and emerging optic

Beyond Coronary Artery Disease: Assessing the Microcirculation.

Ischemic heart disease (IHD) affects more than 20 million adults in the United States. Although classically attributed to atherosclerosis of the epicardial coronary arteries, nearly half of patients with stable angina and IHD who undergo invasive coronary angiography do not have obstructive epicardial coronary artery disease. Ischemia with nonobstructive coronary arteries is frequently caused by microvascular angina with underlying coronary microvascular dysfunction (CMD). Greater understanding the pathophysiology, diagnosis, and treatment of CMD holds promise to improve clinical outcomes of p

Microcirculation abnormalities provoked by Loxosceles spiders' envenomation.

Loxoscelism is caused by envenomation by spiders from Loxosceles genus. Clinical symptoms only appear a few hours after envenomation and can evolve in local reactions, such as dermonecrosis, and systemic reactions, including intravascular haemolysis, intravascular coagulation and renal failure. Considering that alterations in the microcirculatory network are involved in the pathogenesis of different diseases, including the inflammatory process, the aim of this study was to investigate the action of venoms of males and females of Loxosceles intermedia and Loxosceles laeta on the microcirculator

[Microcirculation disturbances in liver diseases (author's transl)].

Microcirculation was studied in liver biopsies of patients with viral hepatitis, Gilbert's syndrome, liver steatosis, chronic hepatitis, liver cirrhosis, obstructive jaundice and venous congestion. All these diseases were shown to be usually accompanied by morphologic changes of sinusoid, Disse's and intercellular spaces, cellular membranes of hepatocytes. Those changes causing microcirculation disturbances result in the functional insufficiency of the liver.

End organ perfusion and pediatric microcirculation assessment.

Cardiovascular instability and reduced oxygenation are regular perioperative critical events associated with anesthesia requiring intervention in neonates and young infants. This review article addresses the current modalities of assessing this population's adequate end-organ perfusion in the perioperative period. Assuring adequate tissue oxygenation in critically ill infants is based on parameters that measure acceptable macrocirculatory hemodynamic parameters such as vital signs (mean arterial blood pressure, heart rate, urinary output) and chemical parameters (lactic acidosis, mixed venous

Assessment of Coronary Microcirculation During Cardiac Catheterization.

Functional tests used in the catheterization laboratory have emerged as a very important adjunctive tool to coronary angiography that can identify patients with myocardial blood flow impairment. Fractional Flow Reserve (FFR) measurement is highly recommended for detection of ischemia-related coronary lesion(s) when objective evidence of vessel-related ischemia is not available. Recently, the much simpler instantaneous wave free ratio (iFR) was proposed as an alternative to FFR without the requirement for administration of vasodilators. More user-friendly techniques like iFR might further contr

Two-compartment modeling of tissue microcirculation revisited.

PURPOSE: Conventional two-compartment modeling of tissue microcirculation is used for tracer kinetic analysis of dynamic contrast-enhanced (DCE) computed tomography or magnetic resonance imaging studies although it is well-known that the underlying assumption of an instantaneous mixing of the administered contrast agent (CA) in capillaries is far from being realistic. It was thus the aim of the present study to provide theoretical and computational evidence in favor of a conceptually alternative modeling approach that makes it possible to characterize the bias inherent to compartment modeling
