A few studies have reported a variety of nonspecific histological lesions in patients with IgA monoclonal gammopathies and polyneuropathy. In our case, using electron microscopy, we observed widenings of the myelin lamellae identical to those commonly described in IgM neuropathies with anti-myelin-associated glycoprotein activity. Using immunoelectron microscopy, we demonstrated a direct involvement of IgA in myelin lesions. The search for a direct link between monoclonal dysglobulinemia, regardless of type, and polyneuropathy is important and may influence treatment.

We describe 2 adult patients with a subepidermal bullous dermatosis with exclusively linear IgA depositions along the epidermal basement membrane zone that were deposited in the sublamina densa zone as witnessed by direct immunoelectron microscopy. Two patients with dermatitis herpetiformis and one with linear IgA disease were examined. Two of the patients had oral lesions and all three showed IgA deposits detected by direct immunofluorescence in apparently normal buccal mucosa. A 38-year-old man with systemic lupus erythematosus had nodular cutaneous mucinosis, leukocytoclastic vasculitis, and a vesiculobullous eruption. Immunopathologic study of a bullous lesion revealed granular deposits of IgA and IgG along the basement membrane zone. Electron microscopy is an essential component of myopathology, both in diagnostics and research of neuromuscular diseases. Carnosine-immunoreactive primary olfactory nerve terminals are demonstrated in the glomerular layer of the rat olfactory bulb by immunoelectron microscopy. Asymmetrical synapses between dendrites of mitral/tufted cells and that of periglomerular cells could be observed.
