High-proportion breast milk feeding is associated with a reduction in the incidence of IVH in very preterm infants.

OBJECTIVE: To investigate the protective effect of high-proportion breast milk feeding (>50%) on intraventricular hemorrhage (IVH) in very preterm infants (VPIs). METHODS: This was a retrospective secondary analysis of a prospective multi-center study, which included 604 VPIs from six hospitals in eastern China between September 2019 and December 2020. The 604 VPIs were divided into two groups according to whether IVH occurred. High-proportion breast milk feeding was defined as breast milk accounting for 51-100% of the total feeding amount both within 7 days and throughout the hospitalization

Impact of microbiological testing protocols on discard rates in donor human milk with a focus on Bacillus cereus.

Microbiological testing protocols are key components of human milk bank operations, yet the lack of consensus on screening schedules and microbiological criteria leads to variability in discard rates and uncertainty about safety standards. This study provides a large-scale characterization of pre-pasteurization donor human milk microbiota and evaluates the impact of testing protocols on operational outcomes. Over 59 months, 7,921 donor human milk batches were analysed. Pre-pasteurization analyses were dominated by skin staphylococci, with environmental Gram-negative bacilli and enteric genera

[Variability in enteral feeding practices of preterm infants among hospitals in the SEN1500 Spanish neonatal network].

INTRODUCTION: Proper nutrition is one of the primary objectives in the management of preterm infants. However, lack of evidence on the best strategy to achieve this objective has led to a great variability in feeding practices. This variability may be related to the differences in the incidence of complications, such as necrotising enterocolitis (NEC). OBJECTIVE: The aim of this study is to assess the variability in clinical practice regarding enteral feeding in SEN-1500 Spanish network. METHOD: An observational study was conducted using a questionnaire sent out in 2013 requesting informatio

High Temperature-Short Time Pasteurization Has a Lower Impact on the Antiviral Properties of Human Milk Than Holder Pasteurization.

Holder pasteurization (62. 5°C for 30 min) is recommended by all international human milk bank guidelines to prevent infections potentially transmitted by donor human milk. A drawback is that it affects some human milk bioactive and nutritive components. Recently, High Temperature-Short Time (HTST) pasteurization has been reported to be a valuable alternative technology to increase the retention of some biological features of human milk. Nevertheless, to date, few data are available about the impact of pasteurization methods other than Holder on the antiviral activity of human milk. The presen

[The value of human milk for preterm infants-overview and practical aspects].

Over the last decades the immense benefit of human milk on the nutrition of preterm infants has become increasingly evident. Research has confirmed that human milk has significant advantages for the preterm infant in terms of host defense, gastrointestinal development and maturation, neurological development, reduction of necrotizing enterocolitis, retinopathy of prematurity and chronic lung disease as well as mental and physical benefits for the mother. Computing these factors into a health-cost-benefit equation, positive economic consequences for a national public health system were demonstr

Receiving screened donor human milk for their infant supports parental wellbeing: a mixed-methods study.

BACKGROUND: Access to donor human milk (DHM) has primarily been based on the health and development outcomes of premature infants but there has been little examination of the broader impact of an infant receiving it upon parental mental health. Breastfeeding and mental health are closely tied with women who experience breastfeeding difficulties or are unable to meet their own breastfeeding goals often experiencing feelings of guilt, sadness and anger, alongside an increased risk of postnatal depression. The aim of the current study was to explore how experience of receiving DHM for their baby

The Impact of Donating Human Milk on the Health of the Donor and Their Infant: Evidence from Two Systematic Reviews.

Using human milk has been associated with decreased morbidity and mortality in preterm/low birth weight infants. Donor human milk is recommended when maternal milk is unavailable. The benefits of donor human milk for the recipient are well documented, but the impact of donation on donors and their infants is not clear. We aimed to evaluate the effects of donation human milk on donors and their infants. Literature searches were conducted (April 2024) to identify studies (observational, quasi-experimental, and randomized control trials) assessing the impact of human milk donation on donor health

Care of the growth-restricted newborn.

With the first 1,000 days of life proving to be a critical window of opportunity for physical and cognitive growth and development, an optimal intrauterine environment is vital. If fetus needs are compromised prenatally, there is an increased risk of intrauterine growth restriction (IUGR), and infants being born premature, low birth weight (LBW), or small-for-gestational age (SGA). Specialized care of these high-risk infants is necessary in terms of preconception interventions, resuscitation, thermoregulation, nutritional support and kangaroo mother care. Significant evidence supports exclusiv

The Contactless Milk Depot-Pioneering Round-the-Clock Human Milk Donation.

BACKGROUND: The KK Human Milk Bank provides pasteurized donor human milk to vulnerable infants across Singapore who lack sufficient mothers' own milk. However, donors and their designated family members were previously limited to depositing frozen milk only during office hours, creating inconvenience and restricting timely access. These challenges were amplified during the COVID-19 pandemic, when hospital entry limitations and concerns about viral exposure further discouraged on-site milk drop-offs. OBJECTIVE: To develop a contactless, round-the-clock milk depot that enables donors to securel

The Use of Saliva Samples to Test for Congenital Cytomegalovirus Infection in Newborns: Examination of False-Positive Samples Associated with Donor Milk Use.

A universal screening research study was conducted in six hospitals to identify the clinical sensitivity of polymerase chain reaction (PCR) testing on newborn dried blood spots (DBSs) versus saliva specimens for the diagnosis of congenital cytomegalovirus (cCMV). CMV DNA positive results from DBSs or saliva were confirmed with urine testing. Findings of several false-positive (FP) saliva PCR results prompted an examination of a possible association with donor milk. Documentation of the frequency of positive saliva results, including both true-positive (TP) and FP status from clinical confirmat

Bacillus Pre-Pasteurization Screening Protocol Reduces Donor Human Milk Loss.

Introduction: Human milk is the optimal source of nutrition and bioactives for infants, particularly preterm infants, who are at higher risk of morbidity and mortality. Many premature infants require pasteurized donor human milk (PDHM) to supplement their diet. Given the increased susceptibility of preterm infants to infections, PDHM is strictly regulated to ensure safety. Pasteurization of human milk eliminates most pathogens. However, spore-forming bacteria, notably Bacillus cereus and other Bacillus species, are capable of surviving pasteurization. These bacteria contribute significantly to

Microbiological safety and immunoglobulin A retention in bottle-agitated Holder pasteurization of human milk.

BACKGROUND: National guidelines for donor human milk pasteurization in Peru and most low to middle income countries use Holder Pasteurization (HoP; 62.5 °C, 30 min) and indicate that the bottles containing milk should be agitated manually every 5 min during the 62.5 °C plateau. The guidelines also assume indirect control of the temperature inside the bottles through adjustments in the setpoint temperature of the water bath. Automatic agitation of the bottles could improve the dynamic temperature patterns to be closer to the target temperature. The aim of this study was to describe the implemen
