Effects of haloperidol on morphine-induced antinociception morphine tolerance and withdrawal in hyperprolactinaemic rats.

Male rats with hyperprolactinaemia, induced by adenohypophyseal homografts under the kidney capsule, were injected with haloperidol for 3 days and then subjected to treatment with morphine, administered twice in a day at increasing doses for 12 days. At the first treatment, morphine induced an antinociceptive effect that was more prolonged in homografted rats than in sham-operated controls. The pretreatment with haloperidol did not change the prolongation of morphine-induced antinociception in homografted rats. After 12 days of treatment with morphine, all animals were tested for the developme

The release of morphine from the mast cells by 48/80 in morphine perfused dogs.

The aim of this study was to determine whether or not mast cells store or bind morphine. Injection of a potent mast cell depleter, compound 48/80 to dogs, 10 minutes after completion of a perfusion of 30 mg/kg of morphine produced a large concomitant increase of both morphine and histamine in blood samples drawn from the aorta. This increase is transient and statistically significant up to 1 minute following the injection of the drug. Morphine and histamine levels decrease rapidly as they reach the general circulation and are subjected to distribution. Injection of compound 48/80 one hour afte

[Use of anti-morphine antibodies to determine low concentrations of morphine in human blood].

The authors have developed an immunochemical method for determining low concentrations of morphine in human serum based on using the reaction of passive hemagglutination inhibition. The method is highly sensitive and practical, does not require special equipment, expensive reagents, and preliminary treatment of the biological fluid. The method has already been used for determining the morphine level in narcomaniacs with the opium form of morphinism.

The effects of acute and chronic morphine treatment and of morphine withdrawal on rat brain in vivo.

1. The effects of morphine, nalorphine, acetazolamide, and 10% CO(2) on brain metabolite concentrations of 24h-starved rats were studied. 2. A single dose of morphine (20mg/kg body wt.) caused an increase in brain glucose concentration (42%) and decreased concentrations of lactate (24%), pyruvate (29%), citrate (20%), alpha-oxoglutarate (16%), malate (14%) and creatine phosphate (10%) after 30min. No changes were found in adenine nucleotide concentrations. 3. The same dose of morphine increased arterial CO(2) from 5.07 to 7.60 kN/m(2) (38 to 57 Torr), decreased the pH from 7.41 to 7.31 and dec

Morphine Antinociception Restored by Use of Methadone in the Morphine-Resistant Inflammatory Pain State.

The antinociceptive effect of methadone in the morphine-resistant inflammatory pain state was described in the paw-withdrawal test using the complete Freund's adjuvant (CFA)-induced mouse inflammatory pain model. After intraplantar (i.pl.) injection of CFA, thermal hyperalgesia was observed in the ipsilateral paw. The antinociceptive effects of subcutaneous (s.c.) injection of morphine, fentanyl, and oxycodone against thermal hyperalgesia in the inflammatory pain state were reduced in the ipsilateral paw 7 days after CFA pretreatment. On the contrary, the antinociceptive effect of s.c. injecti

Self administration of heroin and cocaine in morphine-dependent and morphine-withdrawn rhesus monkeys.

RATIONALE: Dependence can develop during chronic opioid use, and the emergence of withdrawal might promote drug taking. OBJECTIVE: This study examined how chronic morphine administration or withdrawal modified self administration of heroin or cocaine. METHODS: Four monkeys responded under a fixed ratio 10 schedule to receive i.v. infusions of heroin (0.56-560 microg/kg/infusion) or cocaine (1-100 microg/kg/infusion). Monkeys received morphine twice daily; the final dose was 10 mg/kg/12 h. Dose-effect curves for heroin or cocaine were determined in 150-min sessions throughout morphine adminis

Formation of morphine tolerance in offspring of morphine-tolerant animals: neurochemical and neuroimmune correlates.

We carried out a complex physiological, neurochemical, and neuroimmunologic study of the formation of tolerance to analgetic effect of morphine and analyzed enkephalinase A activity in different brain structures and serotonin antibodies in the serum. More early development of morphine tolerance and a sharp increase in serum antibody titer was found in the offspring of morphine-tolerant rats. This points to an imbalance in the neurotransmitter system and can serve as a diagnostic marker of endogenous opioid system pathology.

Analgesia from morphine and ketamine. A comparison of infusions of morphine and ketamine for postoperative analgesia.

Ketamine 4 micrograms/kg/minute produced pain relief similar to that from morphine 33 micrograms/minute in a double-blind study that compared analgesia from constant-rate intravenous infusions of the two drugs in 60 patients. The analgesic efficacy of the infusions, as assessed by pain scores and the requirement for supplementary self-administered morphine, was poor. Ventilatory depression, the most significant side effect, occurred only in patients who received morphine infusion. The low dose ketamine infusion did not provide clinically useful analgesia even though adequate plasma concentrati

Comparison of oral controlled release morphine and epidural morphine in the management of postoperative pain.

Oral controlled release morphine (CRM) was compared in a double-blind study with epidural morphine (EM) for postoperative pain relief in 20 patients undergoing knee arthrotomy under epidural anesthesia. Ten patients received 30 mg CRM orally and saline epidurally (CRM group), and ten patients received placebo tablets orally and 4 mg morphine epidurally (EM group), both at the time of skin incision and then every 8 hr for 48 hr during which patients evaluated pain intensity using a visual analog scale. Nine of the ten patients in the EM group had good relief of pain throughout the study period.

Rectally administered morphine: plasma concentrations in children premedicated with morphine in hydrogel and in solution.

Seventeen healthy children undergoing eye surgery in general anaesthesia were given morphine rectally for premedication. Six patients (Group I) were given morphine 0.15 mg X kg-1 in a propylene glycol solution also containing diazepam and hyoscine. Eleven patients (Group II) were given morphine 0.5 mg X kg-1 in a starch hydrogel preparation. The plasma concentrations of morphine (Groups I and II) and diazepam (Group I) were studied. After the lower dosage of morphine the plasma concentrations never exceeded 10 ng X ml-1, whereas the mean peak plasma concentration was 25.3 +/- 5.5 ng X ml-1 aft

Morphine metabolism revisited: I. Metabolic activation of morphine to a reactive species in rats.

Treatment of fasted rats with relatively high doses of morphine rapidly results in depletion of hepatic glutathione (GSH) content and marked elevation of serum transaminase activity. Such morphine-induced response has been generally attributed to central nervous system mediated effects of the drug. We now report that this response might be due to a direct effect of the drug in the liver. That is, its metabolic activation to reactive electrophilic metabolite(s), by the hepatic cytochrome P-450-dependent mixed function oxidase system. Structure-activity relationships of morphine and its congener

Use of sustained release oral morphine as a bridge in withdrawal of morphine in patients on high doses of oral immediate release morphine for cancer pain.

According to World Health Organization (WHO), cancer pain can be controlled effectively with oral morphine in up to 90% of patients. Due to advancement in anticancer therapy and early presentation of cancer patients, the likelihood of cure is on an increasing trend. Awareness and education in the use of oral morphine, and easier regulations in procurement of oral morphine for use in cancer pain has lead to prescription of oral morphine to more patients earlier in pain therapy. In many patients, resolution of disease occurs and it becomes necessary to withdraw morphine. Guidance for starting me

Plasma concentrations of morphine, morphine-6-glucuronide and morphine-3-glucuronide and their relationship with analgesia and side effects in patients with cancer-related pain.

Morphine, the recommended drug for the management of moderate to severe cancer pain, is metabolized predominantly to the glucuronides morphine-6-glucuronide (M6G) and morphine-3-glucuronide (M3G). The quantitative clinical importance of these metabolites following the administration of oral morphine is unclear. This study investigates the relationship between plasma concentrations of morphine (M), M6G, M3G and clinical effects in patients receiving sustained release oral morphine for cancer-related pain. Peak and trough plasma concentrations of morphine and its metabolites were determined by h

The oral-to-intravenous equianalgesic ratio of morphine based on plasma concentrations of morphine and metabolites in advanced cancer patients receiving chronic morphine treatment.

To provide additional pharmacokinetic evidence for the oral-to-parenteral relative potency ratio of 1:2 to 1:3 for chronic morphine use in a palliative care setting, we determined the plasma concentrations of morphine and its major metabolites, morphine-3-glucuronide (M3G) and morphine-6-glucuronide (M6G), in hospitalized advanced cancer patients maintained on long-term oral or intravenous morphine. There were significant linear correlations between daily doses of morphine and plasma concentrations (molar base) of morphine, M3G and M6G for both routes of administration. The oral-to-intravenous

Routine drug monitoring of serum concentrations of morphine, morphine-3-glucuronide and morphine-6-glucuronide do not predict clinical observations in cancer patients.

The clinical importance of routine drug monitoring of serum concentrations of morphine, morphine-6-glucuronide (M6G) and morphine-3-glucuronide (M3G) during chronic morphine therapy is not established. We measured morphine, M6G and M3G serum concentrations in cancer pain patients receiving oral (n = 263, median dose 80 mg/24 hours) or subcutaneous (sc) (n = 35, median dose 110 mg/24 hours) morphine. Regression analyses were performed to investigate if serum concentrations of morphine, M3G and M6G predicted pain intensity (Brief Pain Inventory), health-related quality-of-life variables (EORTC Q

Plasma concentrations of morphine, morphine-3-glucuronide, and morphine-6-glucuronide after intravenous and oral administration to healthy volunteers: relationship to nonanalgesic actions.

Healthy volunteers were given morphine as an i.v. infusion (10 mg), immediate release (IR) tablets (3 x 10 mg), and as a new controlled release (CR) tablet (30 mg) on separate occasions. Venous blood samples were analyzed for morphine, morphine-3-glucuronide (M3G), and morphine-6-glucuronide (M6G) using high-performance liquid chromatography (HPLC). Pupil size, salivation, and central nervous system (CNS) effects were evaluated serially. Pharmacokinetic parameters, calculated using a two-compartment model, were in accordance with previous results for i.v. administration of morphine. The absolu

Nicorandil reduces morphine withdrawal symptoms, potentiates morphine antinociception, and ameliorates liver fibrosis in rats.

AIMS: Chronic liver disease (CLD) is a serious medical condition affecting patients globally and pain management poses a unique challenge. ATP-sensitive potassium channels (KATP) are expressed in nociceptive neurons and hepatic cells. We tested the hypothesis whether morphine and nicorandil, KATP channel opener, alone and in combination possess hepatoprotective, antinociceptive effect and alter morphine physical dependence. MAIN METHODS: Intraperitoneal injection (i.p.) of carbon tetrachloride (CCl4) induced liver fibrosis in male Wistar rats. Nicorandil (15 mg/kg/day) was administered per os

Morphine with adjuvant ketamine vs higher dose of morphine alone for immediate postthoracotomy analgesia.

BACKGROUND: Thoracotomy is associated with severe pain. We hypothesized that the concomitant use of a subanesthetic dose of ketamine plus a two-third-standard morphine dose might provide more effective analgesia with fewer side effects than a standard morphine dose for early pain control. METHODS: We conducted a 6-month randomized, double-blind study in patients undergoing thoracotomy for minimally invasive direct coronary artery bypass or for lung tumor resection. After extubation, when objectively awake (>or= 5/10 visual analogue scale [VAS]) and complaining of pain (>or= 5/10 VAS), patient
