Today advances in techniques and materials for rotator cuff surgery allow the repair of a large variety of types or extensions of cuff lesions in patients from a wide range of age groups who have different kinds of jobs and participate in different kinds of sports, and who have widely different expectations in terms of recovery of functions and pain relief. A large number of factors must be taken into account before implementing a rehabilitation protocol after rotator cuff surgery. These mainly include the technique (materials and procedure) used by the surgeon. Moreover, tissue quality, retra

Context: We sought to describe our experience with the Hybrid Assistive Limb® (HAL®) for active knee extension and voluntary ambulation with remaining muscle activity in a patient with complete paraplegia after spinal cord injury. Findings: A 30-year-old man with complete paraplegia used the HAL® for 1 month (10 sessions) using his remaining muscle activity, including hip flexor and upper limb activity. Electromyography was used to evaluate muscle activity of the gluteus maximus, tensor fascia lata, quadriceps femoris, and hamstring muscles in synchronization with the Vicon motion capture syst

Treatment by continuous passive movement at home is an alternative to immobilisation in a cast after surgery for club foot. Compliance with the recommended treatment, of at least four hours daily, is unknown. The duration of treatment was measured in 24 of 27 consecutive children with a mean age of 24 months (5 to 75) following posteromedial release for idiopathic club foot. Only 21% (5) of the children used the continuous passive movement machine as recommended. The mean duration of treatment at home each day was 126 minutes (11 to 496). The mean range of movement for plantar flexion improved

Chronic patellar tendon rupture is a rare disabling injury that is technically difficult to repair. Many different surgical methods have been reported for the reconstruction of chronic patellar tendon ruptures. We are reporting the use of contralateral bone-tendon-bone (BTB) autograft for chronic patellar tendon rupture reconstruction followed by double-wire loop reinforcement and without postoperative immobilization. One year after the operation, our patient had full knee extension and up to 130 degrees of flexion. He had good quadriceps strength, and isokinetic muscle testing showed no defic

AIM: The present study aimed to investigate whether the response variability of infants to modified constraint-induced movement therapy and bimanual therapy are associated with different types of brain lesions.
METHOD: Infants with unilateral cerebral palsy (N = 22) ages 8-15 months (mean = 10.95, standard deviation = 2.15 months) were grouped according to having either a periventricular brain lesion or a middle cerebral artery infarct lesion. Improvement in hand function was analyzed based on the mini-Assistive Hand Assessment results.
RESULTS: Infants with periventricular brain lesion disp

BACKGROUND: Spinal cord injury (SCI) induces alterations in cardio-autonomic control of which autonomic dysreflexia (AD), a condition characterized by life-threatening hypertension, is arguably the most insidious. Passive hind-limb cycling represents a low-cost therapeutic intervention with demonstrable cardiovascular, sensory, and motor benefits.
OBJECTIVE: To investigate the effect of passive hind-limb cycling on AD in rodents with T3 SCI.
METHODS: Forty-five male Wistar rats were evenly assigned to either uninjured control (CON), SCI, or SCI plus hind-limb cycling exercise (SCI-EX). At th

BACKGROUND: Knee dislocation is a severe but relatively uncommon injury caused by violent trauma that can result in long-term complications, such as arthrofibrosis, stiffness, instability, and pain. Perhaps owing in part to its rarity, treatment of this injury is controversial. We therefore describe a treatment approach for these complex cases involving a novel dynamic knee external fixator.
DESCRIPTION OF TECHNIQUE: We performed open PCL reconstruction when possible and/or repair of other associated lesions. At the end of the surgical procedure, the surgeon applied an external fixator that r

Adhesive capsulitis is characterized by painful, gradual loss of active and passive shoulder motion resulting from fibrosis and contracture of the joint capsule. Other shoulder pathology can produce a similar clinical picture, however, and must be considered. Management is based on the underlying cause of pain and stiffness, and determination of the etiology is essential. Subtle clues in the history and physical examination can help differentiate adhesive capsulitis from other conditions that cause a stiff, painful shoulder. The natural history of adhesive capsulitis is a matter of controversy

OBJECTIVE: Most children with cerebral palsy have increased muscle tension in the lower extremities, which inevitably leads to abnormal gait characteristics. Proper management of excessive muscle tension is therefore of great importance. The aim of this study was to explore the effects of continuous passive motion on children with cerebral palsy, regarding their lower extremity spastic hypertonia and ambulatory function.
DESIGN: A repeated measures design.
SUBJECTS: Sixteen children with cerebral palsy.
METHODS: Intervention was applied to the subject's knees with a continuous passive motio

An injury to the spinal accessory nerve is mostly reported after surgical procedures performed in the posterior triangle of the neck. In addition, it may be caused by fractures in the jugular foramina, traumas or skull base tumors. Clinically, paralysis of the trapezius muscle leads to weakness, downward rotation of the scapulae and falling down of the shoulder girdle. A 38- year-old male with left shoulder pain, scapular deviation and weakness in the left upper extremity, whose symptoms developed over a two-year period following a traffic accident, is presented herein. In the electromyography

The Washington-regimen for the rehabilitation of flexor tendon injuries (Chow et al., 1987) represents a combination of the established Kleinert-method and the controlled passive motion of Duran and Houser. This paper presents the results of a study which was carried out in 99 patients with 113 injured fingers treated in the Department of Burns, Plastic and Hand Surgery of the Accident Hospital Ludwigshafen. 55 patients with injuries of the fingers and 29 patients with injuries of the thumb were evaluated according to functional and subjective criteria and compared to a group of 15 patients tr

METHODS: 60 patients with THFs were randomly and equally divided into the CPM group and non-CPM group. Both groups immediately received CPM and conventional physical therapies during hospitalization. After discharge, the non-CPM group was treated with conventional physical therapy alone, while the CPM group received conventional physical training in combination with CPM treatment. At 6 weeks and 6 months postoperatively, the primary outcome which was knee ROM and the secondary outcome which was knee functionality and quality of life were evaluated.
RESULTS: The CPM group had a significantly i

The so-called "marrow stimulating" technique of microfracture uses an awl to penetrate the subchondral bone in cartilage defects. Disruption of the subchondral bone induces fibrin clot formation in the area of the chondral defect. This clot contains pluripotent, marrow-derived mesenchymal stem cells, which are able to differentiate into fibrochondrocytes, resulting in a fibrocartilage repair with varying amounts of type I, II, and III collagen content. Microfracture is a single-stage procedure that is ideally suited for small, well-contained, Outerbridge grade 3 to 4 cartilage lesions. Most cl

Regeneration of ligaments and tendons is a slow process, compared with the healing of other connective tissues (e.g., bone). Healing starts from the surrounding soft tissues ("extrinsic healing"), but also from the ligament or tendon itself ("intrinsic healing"). Regeneration is poor when there is little surrounding soft tissue available, as is the case in the anterior cruciate ligament. Regeneration of the medial collateral ligament of the knee is examined best. Mechanical and ultrastructural properties are not completely normal even 1 year after dissection of the ligament irrespective of whe

Eleven patients (with 12 supracondylar femur fractures) with knee extension contractures were followed for an average of 24.8 months (range 12-45) after Judet quadricepsplasty. An increase in motion of 53 degrees with active postoperative flexion of 91 degrees was noted. There was an average loss of 14 degrees of flexion from maximum intraoperative correction. One patient had an extensor lag of 30 degrees, which did not improve after quadricepsplasty, but no other patients developed an extensor lag, infection, or skin breakdown after the procedure.

In this study, the use of the dual force system to correct recent or relatively longstanding knee deformities in ten patients is described. (Nine of the patients had severe haemophilia and one had severe von Willebrand's disease.) The mean duration of deformity in these patients was 10 months. The mean range of movement at the affected knee joints increased from 50 degrees at pre-intervention to 110 degrees following 6 weeks of application of the dual force system. In nine of ten patients (90%) the residual flexion deformity ranged from 0 degrees to 10 degrees. The dual force system offers an

BACKGROUND: Continuous local antibiotics perfusion (CLAP) involves placing a bone marrow needle and a double-lumen tube in the infected area, through which an antibiotic at an appropriate concentration is continuously administered. CLAP has shown good outcomes in patients with bone and soft tissue infections. However, the use of CLAP for late infection after total knee arthroplasty (TKA) is rarely reported. We addressed the gap in the treatment of prosthetic joint infection (PJI) using intra-joint antibiotics perfusion (iJAP) combined with debridement, antibiotics, and implant retention (DAIR)

BACKGROUND: Knees with a limited range of motion caused by intraarticular scars benefit from arthroscopic arthrolysis. Usually these scars result from previous surgery, severe trauma with damage of intraarticular structures. Less frequent the reason is primary arthrofibrosis. Improvement of range of motion is achieved by arthroscopic release of scar tissue and removal of the fibrotic Hoffa fat pad.
OBJECTIVES: To improve the patients' range of motion which is necessary for activities of daily living and labour is the aim of this surgery. Scar tissue is debrided and resected arthroscopically w

Functional recovery is typically poor after facial nerve transection and surgical repair. In rats, whisking amplitude remains greatly diminished after facial nerve regeneration, but can recover more completely if the whiskers are periodically mechanically stimulated during recovery. Here we present a robotic "whisk assist" system for mechanically driving whisker movement after facial nerve injury. Movement patterns were either preprogrammed to reflect natural amplitudes and frequencies, or movements of the contralateral (healthy) side of the face were detected and used to control real-time mir

OBJECTIVE: Comparison of cycling interventions to reduce spastic muscle tone increase in patients with spinal cord injury.
SETTING: Neuroprosthetic outpatient clinic in a university hospital.
METHODS: Five patients with spinal cord injury took part in a crossover study in which the lower limbs (1) were stimulated by functional neuromuscular electrical stimulation (FES) to induce leg cycling movements and (2) were passively moved by an ergometer machine. Patients sat in a comfortable chair fastened to the ergometer while FES was done to induce leg cycling (active session). During the passive
