Following disease progression in motor neuron disorders with 3 motor unit number estimation methods.

INTRODUCTION: The objective of this study was to evaluate a recently developed motor unit number estimation (MUNE) method, MScanFit MUNE (MScan), as a measure of disease progression in amyotrophic lateral sclerosis (ALS) compared with compound muscle action potential (CMAP) amplitude and 2 traditional MUNE methods. METHODS: ALS patients were evaluated clinically using the ALS Functional Rating Scale-Revised (ALSFRS-R). MScan, multiple-point stimulation MUNE (MPS), and motor unit number index (MUNIX) were performed in the abductor pollicis brevis (APB) muscle at baseline (27 patients), 4 month

[Motor evacuatory disorders of the digestive tract in the early periods after stomach operations (1. Motor evacuatory disorders of the stomach)].

The method of telemetry was used for studying the motor function of the stomach in 60 patients with ulcer disease before operation and in 48 patients at early terms after truncal vagotomy or resection of the stomach. An analysis of 318 cases of similar operations on the stomach was also made. The states of normo-, hypo- and adynamia were documented. A pronounced inhibition of motility of the stomach and its stump were noted during 14 and more days after vagotomy and resection. Under conditions of hypo- and adynamia the evacuatory function was maintained by the positive gastrointestinal gradien

Mixed Motor Disorder: Essential Tremor Families With Heterogeneous Motor Phenomenology.

BACKGROUND AND OBJECTIVES: Essential tremor (ET) is one of the most prevalent movement disorders. Because ET is so common, individuals with other neurologic disorders may also have ET. There is evidence, however, that the cooccurrence of ET with Parkinson disease (PD) and/or dystonia is not merely a chance cooccurrence. We have observed combinations of these 3 movement disorders within individuals and across individuals within families containing multiple individuals with ET. This observation has a number of implications. Our objective is to present 4 ET families in whom motor phenomenology wa

Aversive stimuli exacerbate defensive motor behaviour in motor conversion disorder.

Conversion disorder or functional neurological symptom disorder (FND) can affect the voluntary motor system, without an organic cause. Functional symptoms are thought to be generated unconsciously, arising from underlying psychological stressors. However, attempts to demonstrate a direct relationship between the limbic system and disrupted motor function in FND are lacking. We tested whether negative affect would exacerbate alterations of motor control and corresponding brain activations in individuals with FND. Ten patients and ten healthy controls produced an isometric precision-grip contrac

Olfactory disorder in motor neuron disease.

In Parkinson's disease and Alzheimer's disease there is profound disorder of olfaction. The extent to which this modality is involved in motor neuron disease (MND) has been studied little. To address this further we assessed olfaction by three methods-a smell identification test ("UPSIT") in 58 patients and 135 controls; olfactory-evoked response (OEP) to H2S in 15 patients, and pathological examination of olfactory bulbs obtained from 8 cadavers. It was found that smell identification compared with the controls was slightly worse overall in the MND group as a whole, but only the bulbar patien

Clinical features of adolescent-onset functional motor disorders in tertiary movement disorders centers.

BACKGROUND: Functional Motor Disorders (FMDs) represent a diagnostic and therapeutic challenge in pediatric neurology, particularly among adolescents. Their clinical presentation is common but often nonspecific, leading to frequent misdiagnoses and diagnostic delays. We aimed to characterize FMDs in adolescents and to examine the frequency of isolated and combined phenotypes and their associations with demographic and clinical variables. METHODS: In this observational study, data were obtained from the Italian Registry of FMDs, including patients with a clinically definite diagnosis of FMD co

Stuttering: a dynamic motor control disorder.

UNLABELLED: The purpose of this review is to determine what neural mechanisms may be dysfunctional in stuttering. Three sources of evidence are reviewed. First, studies of dynamic inter-relationships among brain regions during normal speech and in persons who stutter (PWS) suggest that the timing of neural activity in different regions may be abnormal in PWS. Second, the brain lesions associated with acquired stuttering are reviewed. These indicate that in a high percentage of cases, the primary speech and language regions are not affected but lesions involve other structures, such as the basa

[Motor-evacuation disorders of the digestive tract in the early period after stomach surgery. 3. Motor-evacuation disorders of the large intestine].

The balloonographic method was used for studying the motor function of the sigmoid in 15 healthy volunteers, 15 patients with peptic ulcer before operation and in 25 patients at early terms after truncal vagotomy or resection of the stomach. During the first 2-3 days independent of the type of surgical intervention and of the degree of extra-organic denervation of the digestive tract, the motor activity of the sigmoid has the autonomic regimen of regulation, pathognomic structure of the motor cycle and is characterized by a sufficiently high contractile activity at the expense of frequent and

Central Motor Conduction Time and diffusion tensor imaging metrics in children with complex motor disorders.

OBJECTIVES: To explore potential correlations between Diffusion Tensor Imaging (DTI) metrics and Central Motor Conduction Time (CMCT) in a cohort of children with complex motor disorders. METHODS: For a group of 49 children undergoing assessment for potential Deep Brain Stimulation (DBS) surgery, CMCT was derived from the latency of MEPs invoked by transcranial magnetic stimulation of the contralateral motor cortex and from peripheral conduction times. Tract-Based Spatial Statistics (TBSS) was used to compare Diffusion Tensor Imaging (DTI) metrics between children with normal and abnormal CMC

Motor unit firing in amyotrophic lateral sclerosis and other upper and lower motor neurone disorders.

OBJECTIVE: Motor unit recruitment order and firing rate was investigated in healthy subjects, and in small numbers of patients 50years ago. We aimed to investigate firing rate in different disorders, testing the same target muscle with normal strength, to evaluate possible application in diagnosing upper motor neuron (UMN) lesion. METHODS: We studied motor unit firing in the tibialis anterior muscle in six groups of subjects; normal subjects (n=45), patients with amyotrophic lateral sclerosis (ALS) (n=36), primary lateral sclerosis (PLS) (n=21), progressive muscular atrophy (PMA) (n=14), vari

Motor and non-motor outcomes after a rehabilitation program for patients with Functional Motor Disorders: A prospective, observational cohort study.

BACKGROUND: Rehabilitation has proven effective in improving motor symptoms (i.e., weakness, tremor, gait and balance disorders) in patients with Functional Motor Disorders (FMDs). Its effects on non-motor symptoms (NMSs) such as fatigue, pain, depression, anxiety and alexithymia, have not been explored yet. OBJECTIVE: To explore the effects of a validated inpatient 5-day rehabilitation program, followed by a home-based self-management plan on functional motor symptoms, NMSs, self-rated perception of change, and quality of life (QoL). METHODS: 33 FMD patients were enrolled. Measures for moto

Inherited Paediatric Motor Neuron Disorders: Beyond Spinal Muscular Atrophy.

Paediatric motor neuron diseases encompass a group of neurodegenerative diseases characterised by the onset of muscle weakness and atrophy before the age of 18 years, attributable to motor neuron loss across various neuronal networks in the brain and spinal cord. While the genetic underpinnings are diverse, advances in next generation sequencing have transformed diagnostic paradigms. This has reinforced the clinical phenotyping and molecular genetic expertise required to navigate the complexities of such diagnoses. In turn, improved genetic technology and subsequent gene identification have en

Occurrence of eye movement disorders in motor neuron disease.

The diagnosis of amyotrophic lateral sclerosis (ALS) relies on symptoms and signs related to upper and lower motor neuron injury. Preservation of normal ocular motor movements is an important criterion for making this diagnosis as oculomotility pathways are classically spared in ALS. However, some authors report eye disturbances resulting from nuclear and supranuclear ophthalmoplegia in autopsy-proven ALS. Here, we report a case in which eye movement disorders were an early sign associated with a bulbar-onset ALS. The association of progressive ophthalmoplegia, dysexecutive syndrome and automa

Neurodevelopmental movement disorders - an update on childhood motor stereotypies.

AIM: The term 'stereotypies' encompasses a diverse range of movements, behaviours, and/or vocalizations that are repetitive, lack clear function, and sometimes appear to have a negative impact upon an individual's life. This review aims to describe motor stereotypies. METHOD: This study reviewed the current literature on the nature, aetiology, and treatment of motor stereotypies. RESULTS: Motor stereotypies occur commonly but not exclusively in autistic spectrum disorders. Similar movements are also found in otherwise healthy children and those suffering sensory impairment, social isolation,

Current treatment of vestibular, ocular motor disorders and nystagmus.

Vertigo and dizziness are among the most common complaints with a lifetime prevalence of about 30%. The various forms of vestibular disorders can be treated with pharmacological therapy, physical therapy, psychotherapeutic measures or, rarely, surgery. In this review, the current pharmacological treatment options for peripheral and central vestibular, cerebellar and ocular motor disorders will be described. They are as follows for peripheral vestibular disorders. In vestibular neuritis recovery of the peripheral vestibular function can be improved by treatment with oral corticosteroids. In Men

TDP-43 in differential diagnosis of motor neuron disorders.

Motor neuron disorders are clinically and pathologically heterogeneous. They can be classified into those that affect primarily upper motor neurons, lower motor neurons or both. The most common disorder to affect both upper and lower motor neurons is amyotrophic lateral sclerosis (ALS). Some forms of motor neuron disease (MND) affect primarily motor neurons in the spinal cord or brainstem, while others affect motor neurons at all levels of the neuraxis. A number of genetic loci have been identified for the various motor neuron disorders. Several of the MND genes encode for proteins important f

Sensory-motor control of ligaments and associated neuromuscular disorders.

The ligaments were considered, over several centuries, as the major restraints of the joints, keeping the associated bones in position and preventing instability, e.g. their separation from each other and/or mal-alignment. This project, conducted over 25 years, presents the following hypothesis: 1. Ligaments are also major sensory organs, capable of monitoring relevant kinesthetic and proprioceptive data. 2. Excitatory and inhibitory reflex arcs from sensory organs within the ligaments recruit/de-recruit the musculature to participate in maintaining joint stability as needed by the movement ty

Transcranial magnetic stimulation: studying motor neurophysiology of psychiatric disorders.

RATIONALE: Transcranial magnetic stimulation (TMS) is a noninvasive tool that directly stimulates cortical neurons by inducing magnetic and secondary electric fields. Traditionally TMS has been used to study the motor neurophysiology of healthy subjects and those with neurological disorders. OBJECTIVE: Given the known motor dysfunctions in many psychiatric disorders supplemental usage of TMS to study the underlying pathophysiology of certain psychiatric disorders and to assess treatment outcomes is underway. Such studies include examination of motor neuronal membrane, corticospinal and intrac
