Factors associated with late antiretroviral therapy initiation in Cameroon: a representative multilevel analysis.

OBJECTIVES: Many people living with HIV/AIDS in resource-limited settings begin antiretroviral therapy (ART) at low CD4 counts. Here, we investigated the simultaneous effect of individual-, facility- and regional-level factors on late ART initiation. METHODS: We conducted a survey in a nationally representative sample of 55 HIV treatment facilities in Cameroon. Medical records of 4935 patients >15 years of age who initiated ART in the month of October during the period 2007-10 were reviewed to gather individual characteristics. Late ART initiation was defined as CD4 count ≤ 100 cells/mm(3). F

The association of female circumcision with HIV status and sexual behavior in Mali: a multilevel analysis.

OBJECTIVE: In the regions of Africa where female circumcision (FC) is practiced, it is often regarded as a protective against HIV infection because it is believed to help women resist "illicit" sexual acts. This study examines the association between FC, HIV status, and sexual risk behavior in Mali, while taking into account multilevel factors. The following hypothesis was tested: FC is associated with HIV-positive status but not with decreased sexual behavior. DESIGN: The sample consists of 13,015 Malian women of reproductive age (15-49 years old). The sample is a nationally representative s

Analysis of a multilevel diagnosis decision support system and its implications: a case study.

Medical diagnosis can be performed in an automatic way with the use of computer-based systems or algorithms. Such systems are usually called diagnostic decision support systems (DDSSs) or medical diagnosis systems (MDSs). An evaluation of the performance of a DDSS called ML-DDSS has been performed in this paper. The methodology is based on clinical case resolution performed by physicians which is then used to evaluate the behavior of ML-DDSS. This methodology allows the calculation of values for several well-known metrics such as precision, recall, accuracy, specificity, and Matthews correlati

Herd and cow characteristics affecting the odds of veterinary treatment for disease - a multilevel analysis.

BACKGROUND: Research has indicated that a number of different factors affect whether an animal receives treatment or not when diseased. The aim of this paper was to evaluate if herd or individual animal characteristics influence whether cattle receives veterinary treatment for disease, and thereby also introduce misclassification in the disease recording system. METHODS: The data consisted mainly of disease events reported by farmers during 2004. We modelled odds of receiving veterinary treatment when diseased, using two-level logistic regression models for cows and young animals (calves and
