Blast injury with particular reference to recent terrorist bombing incidents.

The aetiology of primary blast lung is discussed with reference to the biodynamics of blast injury, and the clinical and pathological features of the condition are described. An analysis of casualties from bomb blast incidents occurring in Northern Ireland leads to the following conclusions concerning the injuries found in persons exposed to explosions: (1) there is a predominance of head and neck trauma, including fractures, lacerations, burns, and eye and ear injuries; (2) fractures and traumatic amputations are common and often multiple; (3) penetrating trunk wounds carry a grave prognosis;

Combat foot and ankle trauma.

Injury to the lower extremity is common in the current conflicts, often severely affecting the foot and ankle. Secondary to continued surgical advances, many lower extremities are able to undergo limb salvage procedures. However, scoring systems still do not reliably predict which patient will be best served with an amputation or limb salvage. Because of this, limb salvage should be attempted whenever possible, awaiting definitive treatment at a later time. Treatment begins at the time and location of injury with aggressive debridement, with reduction and external fixation of fractures when po

Risk factors for major amputation in hospitalised diabetic foot patients.

Diabetic foot ulcers are the main cause of non-traumatic lower extremity amputation. The objective of this study was to evaluate the risk factors for major amputation in diabetic foot patients. Eight hundred and sixty diabetic patients were admitted to the diabetic wound centre of the Korea University Guro Hospital for foot ulcers between January 2010 and December 2013. Among them, 837 patients were successfully monitored until complete healing. Ulcers in 809 patients (96·7%) healed without major amputation and those in 28 patients (3·3%) healed with major amputation. Data of 88 potential risk

Motorboat propeller-related head injuries: A systematic literature review with a case illustration.

BACKGROUND: Propeller-related injuries from motorboats are a major cause of injury in recreational water activities including severe and multiple lacerations that can promote scarring, blood loss, traumatic, or surgical amputations. The real incidence of these accidents is still unclear. The authors here present a systematic review of the literature, focusing on head injury, and related recommendations for its evaluation and management, also reporting a case of a female patient injured by a motorboat propeller. METHODS: A systematic literature review was conducted according to the preferred r

[Thumb reconstruction after amputation injuries].

Reconstruction of the thumb following amputation injuries: In this review of the different methods of thumb reconstruction following traumatic amputation, the procedures applicable in the primary care of the hand are first mentioned and demonstrated: replantation of a thumb, cover of a distal amputation with a neurovascular flap and salvage of a thumb with intermediate segmental loss. In most instances the thumb reconstruction is performed as a secondary procedure. The indication and the selection of the method depend upon the level of amputation, the dominance of the injured hand, and the pre

Severe filamentous fungal infections after widespread tissue damage due to traumatic injury: six cases and review of the literature.

We describe 6 cases of severe filamentous fungal infections after widespread tissue damage due to traumatic injury in previously healthy people. Additionally, we report 69 cases from an exhaustive 20-y review of the literature to investigate the epidemiological and clinical features, the prognosis and the therapeutic management of these post-traumatic severe filamentous fungal infections. Traffic (41%) and farm accidents (25%) were the main causes of injury, which involved either the limbs only (41%) or multiple sites (41%). Necrosis was the main symptom (60%) and Mucorales (72%) and Aspergill

Ultrasound guided posterior femoral cutaneous nerve block.

The posterior femoral cutaneous nerve (PFCN) is a branch of the sacral plexus. It needs to be implemented as a complementary block for anesthesia or in the surgeries necessitating tourniquet in the suitable cases. We consider target oriented block concept within the PFCN block in the anesthesia implementations with the emergence of ultrasonic regional anesthesia in the practice and with the better understanding of sonoanatomy.

Free-fillet flap harvested in 'severe, high-energy landmine explosion' injuries of lower extremity: a case report.

Fillet flaps harvested from the non-replantable or unsalvageable amputated segment can be used to cover tissue defects. We discuss the case of a patient who had suffered a severe high-energy landmine injury, including severe leg damage, resulting in a below-knee amputation and soft-tissue defect around the forearm region. We successfully harvested the fillet from the amputated part of the extremity to the forearm region. We conclude that harvesting of a fillet flap from severely injured lower extremity, resulting from a high-energy landmine explosion, is technically feasible.

[Replantation of segmental destructive amputation of multiple fingers].

OBJECTIVE: To discuss the indication of replantation of destructive amputation of multiple fingers for improvement of the function of injured fingers. METHODS: From February 1996 to August 1999, 23 amputated fingers in 8 cases were shortened and replanted. The crushed digital bones were fixed by Kirschner wires, flexor tendons repaired by Kessler suture technique, and digital extensor tendons repaired by mattress suture. The arteries and veins were anastomosed in each finger at the ratio of 1 to 2 or 2 to 3. The defect of blood vessels was repaired by free graft of autologous veins in 5 finge

Salvage of a foot by free transplants from the contralateral leg in a 5-year-old.

A 5-year-old girl had a traumatic amputation of her left thigh and on the right side loss of the posterior part of the talus and parts of the posterior tibial nerve and artery. These defects were treated with free transplantation of the corresponding part of the talus from the amputated left leg and a vascularized graft from the left posterior tibial nerve and artery. 2 years later the girl was able to walk without crutches, the ankle mobility was good, and so was the sensibility in the sole of the foot.

Total reconstruction of the auricle after traumatic amputation.

We report a technique of auricular replantation used in a case of traumatic amputation. The principal difficulties encountered in this type of replantation are mentioned, and ways of avoiding them are suggested. All the skin of the ear was removed except for that of the anterior surface of the conch. At this site, the cartilage received small, multiple perforations to allow for nutrition of the corresponding skin. The remainder of the auricular cartilage was covered by a skin flap undermined from the mastoid region. Three months later, the retro-auricular region was freed and covered with a to

The London nail bombings: the St. Thomas' Hospital experience.

At 18:00 h on 17 April 1999 a nail bomb exploded outside a supermarket in Brixton, London. This was the first of a series of three identical nail bombs. The injuries were due to penetrating nails, most were relatively minor, with only three patients requiring general anaesthesia for removal of nails and debridement. One 6-year-old child subsequently underwent onward referral for neurosurgical treatment. At 18:40 h on 30 April 1999, the third bomb exploded (there were no casualties at our hospital from the second bomb, that in Brick Lane on 24 April 1999) in the confined environment of the Admi

Are there still indications for the Krukenberg kineplasty? Report of two patients.

Krukenberg's operation was actually described by Vanghetti in 1989. Three cases, including one bilateral case, are reported. The first case was a 36-year-old patient from a developing country. At the sixth postoperative week, the patient had regained his independence and gained 1.5 kg. The second case was a child with multiple malformations, who presented, in addition to amputation of both legs, a high amputation of the left forearm and distal fold of the right forearm. Krukenberg's operation was performed at the age of 8 months. At the age of 2 years, the child was able to eat and drink indep

Prosthesis use among individuals with limb loss after targeted muscle reinnervation/regenerative peripheral nerve interface for symptomatic neuromas: A scoping review.

Neuromas are aberrant axonal growths that may develop after amputations and contribute to residual-limb and/or phantom-limb pain. Burgeoning surgical approaches for treating neuroma pain include targeted muscle reinnervation (TMR) and regenerative peripheral nerve interface (RPNI) surgery. However, little is known regarding functional outcomes postoperatively. The scoping review aimed to examine prosthesis outcomes among individuals with acquired amputations who have undergone TMR and/or RPNI surgery for neuromas. The study protocol was published on Open Science Framework. A search was conduct

How can a methadone and an opiate-positive immunoassay result be reconciled in a patient prescribed only OxyContin and Wellbutrin?

Appropriate management of patients in pain clinics can be complex and sometimes confusing because providers must correctly interpret multiple sources of patient information. The correct interpretation of laboratory results is essential to guide subsequent patient treatment and management; however, laboratory and clinical pictures can appear to be conflicting in cases of substance abuse. Incorrect interpretation of laboratory results can multiply negative impacts on clinical outcomes and may lead to patient harm or death. This report introduces the complex nature involved in understanding and i

Articular Cartilage After Toe Amputation-Protective Barrier or Source of Infection?: A Retrospective Chart Review.

BACKGROUND: Digital and ray amputations are common surgical treatments for infected bone and traumatic injuries in the foot. When disarticulated, the exposed articular cartilage can be addressed by either leaving the cartilage cap as a "protective barrier" to infection or by remodeling the exposed bone, removing the cartilage to bleeding bone to better fight infection. METHODS: Our objective is to provide the first study in the foot to determine whether leaving exposed articular cartilage after toe amputation results in more returns to surgery and delayed healing. We performed a review of the
