Trajectories of Disease Accumulation Using Electronic Health Records.

Multimorbidity is a major problem for patients and health services. However, we still do not know much about the common trajectories of disease accumulation that patients follow. We apply a data-driven method to an electronic health record dataset (CPRD) to analyse and condense the main trajectories to multimorbidity into simple networks. This analysis has never been done specifically for multimorbidity trajectories and using primary care based electronic health records. We start the analysis by evaluating temporal correlations between diseases to determine which pairs of disease appear signif

System failures in care continuity and coordination for people with multiple long-term conditions: an umbrella review of patient, carer, and professional perspectives.

BACKGROUND: The growing number of patients with multiple long-term conditions (MLTCs) presents major challenges for continuity and coordination of care across patients, carers, professionals, and healthcare systems. OBJECTIVES: To identify and contrast key problem areas in continuity and coordination of care from the perspectives of patients, carers, and healthcare professionals (HCPs), and define requirements for a system design toolbox to address them. METHODS: We searched nine databases (MEDLINE, Embase, HMIC, PsycInfo, SPP, CINAHL, AgeLine, AMED, CDAS) from inception to 12 February 2024.

Assessing Progress Toward the Vision of a Comprehensive, Shared Electronic Care Plan: Scoping Review.

BACKGROUND: Care plans are central to effective care delivery for people with multiple chronic conditions. But existing care plans-which typically are difficult to share across care settings and care team members-poorly serve people with multiple chronic conditions, who often receive care from numerous clinicians in multiple care settings. Comprehensive, shared electronic care (e-care) plans are dynamic electronic tools that facilitate care coordination and address the totality of health and social needs across care contexts. They have emerged as a potential way to improve care for individuals

Systemic treatment of HCC in special populations.

Recent years have seen significant progress in the systemic treatment of hepatocellular carcinoma (HCC), including the advent of immunotherapy. While several large phase III trials have provided the evidence for a multi-line treatment paradigm, they have focused on a highly selected group of patients by excluding potentially confounding comorbidities. As a result, high quality evidence for the systemic treatment of HCC in patients with various comorbidities is missing. This review summarises current knowledge on the use of approved medicines in patients with HIV, autoimmune disease, cardiovasc

Understanding transitional care programs for older adults who experience delayed discharge: a scoping review.

BACKGROUND: Many hospitalized older adults cannot be discharged because they lack the health and social support to meet their post-acute care needs. Transitional care programs (TCPs) are designed to provide short-term and low-intensity restorative care to these older adults experiencing or at risk for delayed discharge. However, little is known about the contextual factors (i.e., patient, staff and environmental characteristics) that may influence the implementation and outcomes of TCPs. This scoping review aims to answer: 1) What are socio-demographic and/or clinical characteristics of older

Preoperative pathways in adults with multiple long-term conditions undergoing major surgery: a systematic review.

OBJECTIVES: Adults with multiple long-term conditions (MLTCs) undergoing surgery have a higher risk of postoperative complications compared with adults without MLTCs. No preoperative care interventions, pathways or protocols exist to specifically support people with MLTCs. Our aims were to: (i) summarise current preoperative interventions targeted at people with MLTCs, (ii) report outcomes of preoperative interventions specifically designed for MLTC populations and (iii) identify future research into interventions resulting in better surgical outcomes for this population. DESIGN: Systematic r

Research on prevalence of secondary conditions in individuals with disabilities: an overview.

Individuals with disabilities may be at increased risk of a number of secondary conditions. Secondary conditions influence health status and quality of life. Many of these conditions are preventable and their consequences can be managed. Knowledge of the prevalence, course, and association with age of secondary conditions is needed for better prevention and management. The aim of present study was to review the available literature on the prevalence of secondary conditions in individuals with disability. We performed searches of electronic databases for studies published between 1980 and 2017

Therapeutic Exercise.

Physicians often overlook exercise as a treatment or prophylactic measure for many common diseases and ailments. It can be used to treat comorbidities including obesity, cardiovascular disease, chronic obstructive pulmonary disease, diabetes mellitus, osteoporosis, osteoarthritis, cancer, and low back pain. Education on the general physical activity guidelines as well as easy exercise prescription methods can improve the ability of physicians to prescribe exercise as a therapeutic option. In addition, identifying barriers to compliance with exercise and ways to overcome these barriers is also

Gut microbiota: A player in aging and a target for anti-aging intervention.

Aging-associated alterations in composition, diversity and functional features of intestinal microbiota are well-described in the modern literature. They are suggested to be caused by an age-related decline in immune system functioning (immunosenescence) and a low-grade chronic inflammation (inflammaging), which accompany many aging-associated pathologies. The microbiota-targeted dietary and probiotic interventions have been shown to favorably affect the host health and aging by an enhancement of antioxidant activity, improving immune homeostasis, suppression of chronic inflammation, regulatio

Analysis of multiple chronic disease characteristics in middle-aged and elderly South Koreans by exercise habits based on association rules mining algorithm.

BACKGROUND: The term, "multiple chronic diseases" (MCD), describes a patient with two or more chronic conditions simultaneously at the same time. Compared with general chronic diseases, it is linked to poorer health outcomes, more difficult clinical management, and higher medical expenses. Several existing MCD guidelines support a healthy lifestyle including regular physical activities but do not include specific exercise therapy recommendations. This study aimed to understand the prevalence and model of MCD in middle-aged and elderly South Koreans by comparing MCD characteristics with exercis

[New Trains of Thoughts About Acupuncture Analgesia-Acupuncture Analgesia Feb Involve Multi-dimensional Regulation of Pain].

With the development of pain study, researchers gradually recognized that pain is composed of three main dimensions, namely "sensory-discriminative" "affective-motivational" and "cognitive-evaluative" which influence each other and are also independent from each other. Pain study has shifted away from focusing on the single mode of nociception to the multi-dimensional mode of sensory-affection-cognition. It is held early in traditional Chinese medicine that "when there is a stoppage, there is a pain" and a worsening disease Feb induce depression, which has already shown a multi-dimensional rec

Nonpharmacologic management of chronic pain in the United States' Medicare population: a scoping review protocol.

OBJECTIVE: This objective of this review is to describe the scope of the literature on the access to and use of nonpharmacologic therapies to manage chronic pain among people with disabilities and older adults. INTRODUCTION: Clinical guidelines recommend nonpharmacologic interventions as first-line therapy for chronic pain management. The importance of nonpharmacologic management is magnified in populations with a high pain burden and multiple chronic conditions, such as people living with a disability and/or older adults, many of whom are enrolled in Medicare. Understanding the utilization o

Public Perceptions of Multiple Long-Term Conditions: An Analysis of Mass Observation Accounts.

INTRODUCTION: The growing number of people living with two or more long-term conditions presents challenges to medicine and society. While the terminology of 'multiple long-term conditions' (MLTC) or 'multimorbidity' has increasingly been adopted in research, policy and clinical practice, how the public responds to 'MLTC' remains underexplored. Given the important implications for research inclusion and intervention uptake, we aimed to address this gap by investigating public perceptions of MLTC. METHODS: Mass Observation Project respondents were invited to share their views on MLTC. We colla

Do hospital-to-home transitions work for older adults with multiple long-term conditions including dementia? A realist review.

BACKGROUND: Hospital-to-home transitions involve multiple providers and are particularly complex for older adults with dementia, who often live with additional conditions. Frequent transitions increase the risk of errors, miscommunication, and treatment delays, compromising patient safety and leading to potentially increased mortality, morbidity, and preventable readmissions. Understanding what works and does not work in these processes is essential to improving outcomes. AIM: This realist review synthesised existing literature to explore how, for whom, and to what extent hospital-to-home tra

Chronic comorbidities in children and adolescents with perinatally acquired HIV infection in sub-Saharan Africa in the era of antiretroviral therapy.

Globally, 1·7 million children are living with HIV, of which 90% are in sub-Saharan Africa. The remarkable scale-up of combination antiretroviral therapy has resulted in increasing numbers of children with HIV surviving to adolescence. Unfortunately, in sub-Saharan Africa, HIV diagnosis is often delayed with children starting antiretroviral therapy late in childhood. There have been increasing reports from low-income settings of children with HIV who have multisystem chronic comorbidities despite antiretroviral therapy. Many of these chronic conditions show clinical phenotypes distinct from th

Managing older people with atrial fibrillation and preventing stroke: a review of anticoagulation approaches.

INTRODUCTION: Oral anticoagulants (OACs) are the cornerstone of stroke prevention in atrial fibrillation (AF), but prescribing decisions in older people are complicated. Clinicians must assess the net clinical benefit of OAC in the context of multiple chronic conditions, polypharmacy, frailty and life expectancy. The under-representation of high-risk, older adult sub-populations in clinical trials presents the challenge of choosing the right OAC, where a 'one-size-fits-all' approach cannot be taken. AREAS COVERED: This review discusses OAC approaches for stroke prevention in older people with

The role of senolytics in osteoporosis and other skeletal pathologies.

The skeletal system undergoes irreversible structural deterioration with aging, leading to increased fracture risk and detrimental changes in mobility, posture, and gait. This state of low bone mass and microarchitectural changes, diagnosed as osteoporosis, affects millions of individuals worldwide and has high clinical and economic burdens. Recently, pre-clinical studies have linked the onset of age-related bone loss with an accumulation of senescent cells in the bone microenvironment. These senescent cells appear to be causal to age-related bone loss, as targeted clearance of these cells lea

Association between physical activity behaviors and hypertension with comorbid multiple chronic diseases in South Korean middle-aged and elderly: an analysis based on association rule.

BACKGROUND: The increasing prevalence of multiple chronic diseases in aging populations presents a serious public health concern. Hypertension, as one of the most common chronic conditions, is frequently observed alongside other chronic diseases such as diabetes, obesity, and dyslipidemia. Physical activity is widely acknowledged to be associated with the prevention and management of chronic diseases. However, few studies have examined how different Physical activity behaviors are associated with specific multimorbidity patterns involving hypertension. This study aimed to identify the patterns
